E.3 | Principal inclusion criteria |
• The participant has a parent, caregiver or legal representative (hereinafter “caregiver”) who is reliable, competent and at least 18 years of age. The caregiver is willing and able to accompany the participant to clinic visits and to be available to the Investigational Site by phone or email if needed and who (in the opinion of the investigator) is and will remain sufficiently knowledgeable of participant’s ongoing condition to respond to any inquiries about the participant from personnel from the Study Site. • A caregiver must be able to consent for the participant according to International Council on Harmonisation (ICH) and local regulations. • Ability to comply with all study requirements. • Have adequate supportive psychosocial circumstances . • Able to tolerate blood draws. • Able to undergo LP and IT injection, under sedation or anesthesia if needed and as determined appropriate by the Investigator. • Stable medical status for at least 4 weeks prior to Screening and at the time of enrollment. • Bodyweight of = 7 kg. • Participant must be >= 1 to < = 12 years of age at the time of signing of the informed consent by the caregiver. • Clinical diagnosis of AS confirmed by a molecular diagnosis with genotypic classification of either: - UBE3A truncation mutation of maternal allele - Deletion on the maternally inherited chromosome 15q11q13 that includes the UBE3A gene and is less than 7 Mb in size . • Female Participants: A female participant is eligible to participate if she is not pregnant, not breastfeeding, and at least one of the following conditions applies: Women of non-childbearing potential or Women of childbearing potential who agree to remain abstinent (refrain from heterosexual intercourse) or use acceptable contraceptive methods during the treatment period and for at least 6 months after the final dose of RO7248824 • Male Participants: During the treatment period and for at least 6 months after the final dose of RO7248824, consent has to be provided to remain abstinent |
1. Presenza di un genitore del/della partecipante, di una persona che gli/le presta assistenza o di un suo rappresentante legale (in prosieguo “caregiver”) affidabile, competente e almeno maggiorenne (età =18 anni). Il caregiver deve dimostrare la volontà e la capacità di accompagnare il/la partecipante alle visite ospedaliere e, in caso di necessità, di rendersi disponibile per l’équipe del Centro sperimentale tramite telefono o e-mail, nonché di trovarsi nella condizione (a parere dello sperimentatore) di essere e rimanere sufficientemente a conoscenza dell’evoluzione delle condizioni del/della partecipante per rispondere a qualsiasi domanda sul/sulla partecipante posta dal personale del Centro sperimentale. 2. Capacità del caregiver di prestare il consenso in vece del/della partecipante in base al Consiglio internazionale sull’armonizzazione dei requisiti tecnici per i prodotti medicinali per uso umano (International Council on Harmonisation, ICH) e alle normative locali. 3. Capacità di soddisfare tutti i requisiti dello studio. 4. Disporre del sostegno di adeguate circostanze psicosociali . 5. Capacità di sopportare i prelievi di sangue. 6. Capacità di sottoporsi a una rachicentesi e a un’iniezione IT, sotto sedazione o anestesia, se necessario, e nelle modalità ritenute opportune dallo sperimentatore. 7. Condizioni cliniche stabili per almeno 4 settimane prima della Fase di screening e al momento dell’arruolamento. 8. Peso >= 7 kg 9. Età compresa tra 1 e 12 anni al momento della firma del consenso informato da parte del caregiver. 10. Diagnosi clinica di AS confermata da una diagnosi molecolare con classificazione genotipica di una delle seguenti: - mutazione tronca del gene UBE3A dell’allele materno - Delezione sul cromosoma materno 15q11q13 che include il gene UBE3A e ha dimensioni inferiori a 7 Mb . 11. Partecipanti di sesso femminile: sono ritenute idonee all’ingresso nello studio le partecipanti che non sono in stato di gravidanza né in fase di allattamento e per le quali trova applicazione almeno una delle seguenti condizioni: donne non in età fertile o donne in età fertile che acconsentono a praticare l’astinenza dai rapporti eterosessuali o a fare uso di metodi contraccettivi accettabili durante il Periodo di trattamento e per almeno 6 mesi dopo la somministrazione dell’ultima dose di RO7248824. 12. Partecipanti di sesso maschile: durante il Periodo di trattamento e per almeno 6 mesi dopo la somministrazione dell’ultima dose di RO7248824, i partecipanti devono acconsentire a praticare l’astinenza dai rapporti eterosessuali |

E.4 | Principal exclusion criteria |
• Clinically-significant laboratory, vital sign or electrocardiography (ECG) abnormalities at Screening. • Molecular diagnosis of AS with genotypic classification of: - Paternal UPD of 15q11-13 - UBE3A ID - A partial molecular diagnosis of AS that cannot exclude UPD or ID despite appropriate genetic testing. • Clinically relevant hematological, hepatic, cardiac or renal disease or event, in the judgement of the investigator. Pre-existing abnormal hepatic, renal or hematology lab tests must be discussed with the Sponsor Medical Monitor. • Any concomitant condition that might interfere with the clinical evaluation of AS and that is not related to AS. • Known history of human immunodeficiency virus (HIV) or hepatitis B virus (HBV) or hepatitis C virus (HCV). • Any condition that increases risk of meningitis. • History of bleeding diathesis or coagulopathy. • A medical history of brain or spinal disease that would interfere with the LP process, CSF circulation or safety assessment. • History of clinically significant post-lumbar-puncture headache of moderate or severe intensity and/or blood patch. • Malignancy within 5 years of Screening. • Hospitalization for any major medical or surgical procedure involving general anesthesia within 12 weeks of Screening or planned during the study. • Have any other conditions which, in the opinion of the Investigator, would make the participant unsuitable for inclusion or could interfere with the participant participating in or completing the study, including any contraindication to administration of intrathecal therapy. • Premature birth with gestational age at birth below 34 weeks. • History of hypersensitivity to the investigational medicinal product (IMP), antisense oligonucleotides, or any excipients. • Allowed sleep medications have not been stable for 4 weeks prior to screening and at the time of enrollment. • Allowed medications for treatment of epilepsy have not been stable for 12 weeks prior to screening and at the time of enrollment. • Use of antiplatelet or anticoagulant therapy for 2 weeks prior to screening and at the time of enrollment. • Concurrent psychotropic medications have not been stable for 4 weeks prior to screening and at the time of enrollment. • Received an investigational drug within 90 days or 5 times the half-life of the investigational drug (whichever is longer) or participation in a study testing an investigational medical device within 90 days prior to first dosing or if the device is still active. • Concurrent or planned concurrent participation in any clinical study (including observational and non-interventional studies) without approval of the Sponsor Medical Monitor. At the discretion of the Sponsor, participants may enroll into non-drug observational studies. • Previous participation in a cellular therapy, or gene therapy or gene editing clinical study. |
1. Anomalie clinicamente significative nei risultati di laboratorio, nei segni vitali o nei valori dell’ECG durante la Fase di screening 2. Diagnosi molecolare di AS con classificazione genotipica di: - disomia uniparentale paterna (Paternal Uniparental Disomy, UPD) della regione 15q11-13 - difetto del centro dell’imprinting (Imprinting center defect, ID) del gene UBE3A - diagnosi molecolare parziale di AS, che non permette di escludere UPD o ID nonostante l’appropriatezza dell’analisi genetica. 3. Malattia o evento di tipo ematologico, epatico, cardiaco o renale, clinicamente rilevante a giudizio dello sperimentatore. Anomalie preesistenti nei risultati dei test di laboratorio per la funzionalità epatica, renale o ematologica devono essere discusse con il Medical Monitor dello Sponsor. 4. Qualsiasi malattia concomitante che potrebbe interferire con la valutazione clinica dell’AS e che non è correlata a quest’ultima. 5. Anamnesi nota di virus dell’immunideficienza umana (Human Immunodeficiency Virus, HIV) o virus dell’epatite B (HBV) o dell’epatite C (HCV). 6. Qualsiasi malattia che aumenti il rischio di meningite. 7. Anamnesi di diatesi emorragica o coagulopatia. 8. Anamnesi medica di malattia cerebrale o midollare che interferirebbe con la procedura di rachicentesi, con la circolazione del LCR o con la valutazione della sicurezza 9. Storia di cefalea post-rachicentesi clinicamente significativa di intensità moderata o grave e/o procedura di “blood patch” epidurale 10. Neoplasia nei 5 anni precedenti la Fase di screening 11. Ricovero ospedaliero per qualsiasi procedura medica o chirurgica maggiore in anestesia generale nelle 12 settimane precedenti la Fase di screening o prevista durante lo studio. 12. Presenza di qualsiasi altra malattia che, a parere dello sperimentatore, renderebbe il/la partecipante inidoneo/a all’inclusione nello studio o che potrebbe interferire con la partecipazione allo studio o il completamento dello stesso da parte del/della partecipante, ivi inclusa qualsivoglia controindicazione alla somministrazione di terapia intratecale 13. Nascita prematura con età gestazionale alla nascita inferiore a 34 settimane. 14. Storia di ipersensibilità al medicinale sperimentale (IMP), oligonucleotidi antisenso o altri eccipienti. 15. Farmaci permessi per il sonno (vedere "Terapie consentite" di seguito) non stati stabili per 4 settimane prima dello screening e al momento dell'arruolamento 16. Farmaci consentiti per il trattamento dell'epilessia (vedere "Terapie consentite" di seguito) non stati stabili per 12 settimane prima dello screening e al momento dell'arruolamento. 17. Uso della terapia antipiastrinica o anticoagulante per 2 settimane prima dello screening e al momento dell'arruolamento (vedere "Terapie non consentite" di seguito). 18. Farmaci psicotropi concomitanti non stati stabili per 4 settimane prima dello screening e al momento dell'arruolamento. 19. Aver ricevuto un farmaco sperimentale entro 90 giorni o 5 volte l'emivita del farmaco sperimentale (a seconda di quale sia il periodo più lungo) o partecipazione a uno studio che testa un dispositivo medico sperimentale entro 90 giorni prima della prima somministrazione o se il dispositivo è ancora attivo 20. Partecipazione concomitante o previsione di una partecipazione concomitante a un’altra sperimentazione clinica (ivi inclusi studi osservazionali e non interventistici) senza l’approvazione del Medical Monitor dello Sponsor. A discrezione dello Sponsor, i partecipanti potranno essere arruolati in studi osservazionali non farmacologici. 21.
