1. Participant has histologically- or cytologically-confirmed diagnosis of Stage IV (M1a, M1b, or M1c per current AJCC criteria) squamous or nonsquamous NSCLC 2. Participant with nonsquamous NSCLC who is not eligible for an approved targeted therapy 3. Have confirmation that EGFR-, ALK-, or ROS1-directed therapy is not indicated as primary therapy (documentation of absence of tumor activating EGFR AND absence of ALK or ROS1 gene rearrangements). If participant’s tumor is known to have a predominantly squamous histology, molecular testing for EGFR mutation and ALK and ROS1 translocations will not be required, as this is not part of current diagnostic guidelines 4. Participant has measurable disease per RECIST 1.1 as assessed by the local site investigator/radiology assessment. Lesions situated in a previously irradiated area are considered measurable if progression has been known in such lesions 5. Participant is able to provide archival tumor tissue sample collected either within 5 years or within the interval from completion of last treatment but before entering the screening period; or newly obtained core or excisional biopsy of a tumor lesion not previously irradiated obtained within 90 days of treatment initiation. FFPE tissue blocks are preferred to slides. Newly obtained biopsies are preferred to archived tissue. Biopsies obtained before receipt of adjuvant/neoadjuvant chemotherapy will be permitted if recent biopsy is not feasible. 6. Participant has not received prior systemic treatment for their metastatic NSCLC. Participants who received adjuvant or neoadjuvant therapy are eligible if the adjuvant/neoadjuvant therapy was completed at least 12 months before the development of metastatic disease 7. Participants with PD-L1 TPS >=1% 8. Participant is Male or Female who is at least 18 years of age at the time of signing the informed consent 9. Participant has an ECOG performance status of either 0 or 1 as assessed within 7 days before initiation of study intervention 10. Participant is able to complete all screening procedures within the 35-day screening window. A participant may be rescreened after sponsor consultation 11. Participant has adequate organ function; all screening laboratory tests should be performed within 10 days of initiation of study intervention 12. A male participant must agree to use contraception and should refrain from donating sperm for at least 120 days after study interventions 13. A female participant is eligible to participate if she is not pregnant, not breastfeeding, and at least 1 of the following conditions applies: a. Not a WOCBP, OR b. A WOCBP who agrees to follow the contraceptive guidance during the treatment period and for at least 120 days after study intervention 14. The participant (or legally acceptable representative if applicable) has provided documented informed consent/assent for the study. The participant may also provide consent/assent for Future Biomedical Research. However, the participant may participate in the main study without participating in Future Biomedical Research |
1. Il partecipante presenta una diagnosi istologicamente o citologicamente confermata di NSCLC squamoso o non squamoso in stadio IV (M1a, M1b o M1c secondo gli attuali criteri AJCC). 2. Il partecipante presenta NSCLC non squamoso e non è idoneo a una terapia mirata approvata. 3. Conferma che la terapia mirata a EGFR, ALK o ROS-1 non è indicata come terapia primaria (documentazione dell'assenza di EGFR attivante del tumore E dell'assenza di riarrangiamenti del gene ALK o ROS1). Se è noto che il tumore del partecipante presenta un'istologia prevalentemente squamosa, non saranno richiesti test molecolari per la mutazione dell'EGFR e le traslocazioni di ALK e ROS1, poiché questo non fa parte delle attuali linee guida diagnostiche. 4. Il partecipante presenta malattia misurabile secondo RECIST 1.1, valutata dallo sperimentatore/radiologia del centro locale. Le lesioni situate in un'area precedentemente irradiata sono considerate misurabili se la progressione è stata dimostrata in tali lesioni. 5. Il partecipante è in grado di fornire un campione di tessuto tumorale di archivio prelevato negli ultimi 5 anni o entro l'intervallo dal completamento dell'ultimo trattamento, ma prima di iniziare il periodo di screening o dispone di una biopsia incisionale o escissionale ottenuta di recente da una lesione tumorale non precedentemente irradiata, prelevata nei 90 giorni precedenti l'inizio del trattamento. I blocchi di tessuto FFPE sono preferibili ai vetrini. Le biopsie di campioni freschi sono preferibili ai tessuti provenienti da archivio. Saranno consentite le biopsie ottenute prima della somministrazione della chemioterapia adiuvante/neoadiuvante se la biopsia recente non è fattibile. 6. Il partecipante non ha ricevuto un trattamento sistemico precedente per l'NSCLC metastatico. I partecipanti che hanno ricevuto una terapia adiuvante o neoadiuvante sono idonei se la terapia adiuvante/neoadiuvante è stata completata almeno 12 mesi prima dello sviluppo della malattia metastatica. 7. Partecipanti con punteggio percentuale tumorale (TPS) del PD-L1 >=1%. 8. Il partecipante è di sesso maschile o femminile e ha compiuto i 18 anni di età al momento della firma del consenso informato. 9. Il partecipante presenta un performance status secondo l'ECOG pari a 0 o 1, valutato nei 7 giorni precedenti l'inizio del trattamento dello studio. 10. Il partecipante è in grado di completare tutte le procedure di screening entro la finestra di screening di 35 giorni. Un partecipante può essere sottoposto a nuovo screening previo consulto con lo Sponsor. 11. Il partecipante presenta una funzione d'organo adeguata; tutte le analisi di laboratorio di screening devono essere eseguite nei 10 giorni precedenti l'inizio del trattamento dello studio. 12. Un partecipante di sesso maschile deve acconsentire a utilizzare la contraccezione e deve astenersi dalla donazione di sperma per almeno 120 giorni dopo i trattamenti dello studio. 13. Una partecipante è ritenuta idonea alla partecipazione se non è in gravidanza, non allatta al seno e soddisfa almeno 1 delle seguenti condizioni: a. Non è una donna in età fertile, O b. È una donna in età fertile che acconsente a seguire le indicazioni sui metodi contraccettivi durante il periodo di trattamento per almeno 120 giorni dopo il trattamento dello studio. 14. Il partecipante (o un rappresentante legalmente accettabile, se applicabile) ha fornito il consenso/assenso informato documentato per lo studio. Il partecipante può anche fornire il consenso/assenso a future ricerche biomediche. Tuttavia, il partecipante può prendere parte allo studio principale senza partecipare alle ricerche biomediche future. |

E.4 | Principal exclusion criteria |
1. Has a diagnosis of small cell lung cancer. For mixed tumors, if small cell elements are present, the patient is ineligible 2. Has a diagnosis of immunodeficiency or is receiving chronic systemic steroid therapy (in dosing exceeding 10 mg daily of prednisone equivalent) or any other form of immunosuppressive therapy within 7 days before the first dose of study drug. Participants with asthma that require intermittent use of bronchodilators, inhaled steroids, or local steroid injections would not be excluded from the study 3. Has a known additional malignancy that is progressing or has required active treatment within the past 2 years 4. Has known active CNS metastases and/or carcinomatous meningitis. Participants with previously treated brain metastases may participate provided they are radiologically stable (ie, without evidence of progression) for at least 2 weeks by repeat imaging (note that the repeat imaging should be performed during study screening), clinically stable, and without requirement of steroid treatment for at least 14 days before first dose of study intervention. Patients having untreated brain metastases will require treatment of the metastases to be eligible to enter the study 5. Has an active autoimmune disease that has required systemic treatment in the past 2 years (ie, with use of disease modifying agents, corticosteroids or immunosuppressive drugs). Replacement therapy (eg, thyroxine, insulin, or physiologic corticosteroid replacement therapy for adrenal or pituitary insufficiency) is not considered a form of systemic treatment and is allowed 6. Has a history of (non-infectious) pneumonitis that required steroids or has current pneumonitis. Lymphangitic spread of the NSCLC is not exclusionary 7. Has an active infection requiring systemic therapy 8. Has clinically significant cardiac disease, including unstable angina, acute myocardial infarction within 6 months from Day 1 of study drug administration, or New York Heart Association Class III or IV congestive heart failure. Medically controlled arrhythmia stable on medication is permitted 9. Has a known history of HIV infection (known HIV 1/2 antibodies positive) 10. Has a known history of Hepatitis B (defined as HBsAg reactive) or known active Hepatitis C virus (defined as HCV RNA [qualitative] is detected) infection 11. Has known psychiatric or substance abuse disorders that would interfere with cooperating with the requirements of the study 12. Has had major surgery (<3 weeks before first dose) 13. Is expected to require any other form of antineoplastic therapy while on study 14. Has received prior radiation therapy to the lung that is >30 Gy within 6 months of the first dose of study intervention. Participants must have recovered from all radiationrelated toxicities, not require corticosteroids, and not have radiation pneumonitis. A 1-week washout is permitted for palliative radiation (<=2 weeks of radiotherapy to CNS disease) 15. Has received a live vaccine within 30 days before the first dose of study intervention. 16. Has received any prior immunotherapy and was discontinued from that treatment due to a Grade 3 or higher irAE 17. Has had chemotherapy or biological cancer therapy within 4 weeks before the first dose of study treatment, or has not recovered to CTCAE Grade 1 or better from the AEs due to cancer therapeutics administered more than 4 weeks before the first dose of study treatment (including participants who had previous immunomodulatory therapy with residual irAEs). Participants receiving ongoing hormone replacement therapy for endocrine irAEs will not be excluded from participation in this trial. Participants with Grade 2 or lower neuropathy may be eligible

I partec con asma, che richiedono l'uso intermitt di broncodilat, steroidi per inalaz o iniez di steroidi locali, non saranno esclusi dallo stud 3. Il partec present un ulteriore tumor malign noto che sta progredendo o che ha richiesto un trattam attiv negli ultimi 2anni 4. Il partec present metast attiv note nel SNC e/o meningite carcinomat.I partec con metast cerebr precedent tratt possono partecip a condiz che siano radiolog stabili (ovvero senza evidenza di progress) per almeno 2sett con conferma mediante imaging ripetuto(si noti che l'imaging ripetuto deve essere eseguito durante lo screening dello stud), clinicam stabili e senza necess di trattam con steroidi per almeno 14gg prima della prima dos del trattam dello stud.I pz con metast cerebr non tratt avranno necessità di trattam delle metast per essere idonei a partecip allo stud 5. Il partec present una malatt autoimm attiva che ha richiesto un trattam sistem negli ultimi 2anni(ovvero con l'uso di agenti modif la malatt, corticoster o farmac immunosop). La terap sost(ad es. tiroxina, insulina o terap sost con corticoster fisiolog per insuff surrenalica o ipofisaria)non è considerata una form di trattam sistem ed è consentita 6.Il partec present un'anamnesi di polmon(non infettiva)che ha richiesto l'uso di steroidi o present polmon in atto. La diffus linfangitica dell'NSCLC non è un fattore di esclus 7. Il partec present un'infez attiv che richiede una terap sistem 8. Il partec present una malatt cardiaca clinicam significativ, inclusa angina instabile, infarto miocardico acuto entro 6mesi dal Gg 1 di somministr del farmac dello stud o insuff cardiaca congestizia di classe III o IV della New York Heart Association. È ammessa l'aritmia control dal punto di vista medico e stabiliz dai farmac 9. Il partec present un'anamnesi nota di infez da HIV(positiv accertata agli anticorpi HIV 1/2) 10. È consentito un washout di 1 sett per la radioterap palliativa(<=2 sett di radioterap a livello del SNC) 15. Il partec ha ricevuto un vaccino vivo nei 30gg prec la prima dos del trattam dello stud 16. Il partec ha ricevuto una prec immunoterap ed è stato sosp da quel trattam a causa di un evento avv immuno-correlato (irAE) di grado3 o > 17. Il partec ha ricevuto chemioterap o terap antitum biolog nelle 4sett prec la prima dos del trattam dello stud, oppure non si è ripreso fino al grado1 o migliore in base ai criteri CTCAE dagli AE dovuti a terapie antitum somminist più di 4sett prima della prima dos del trattam dello stud(inclusi i partec che hanno ricevuto una terap immunomod prec con irAE residui) I partec che ricevono una terap ormon sost in corso per irAE endocrini non saranno esclusi dalla partecip a questa speriment.I partec con neuropat di grado2 o < possono essere idonei
