E.3 | Principal inclusion criteria |
1. Female, age at least 18 years.
2. Histologically diagnosed low-grade serous ovarian cancer, fallopian tube cancer, or primary peritoneal cancer.
3. Patients must have completed at least 1 previous course of platinum-containing therapy (e.g., combination with carboplatin or cisplatin. Maintenance therapy with bevacizumab and/or letrozole is allowed). Neoadjuvant chemotherapy in the first line therapy will be counted as one line
of therapy. Patients may, but are not required to, have a previous cytoreductive surgery in the first as well as in the second line.
4. Patients must have platinum sensitive disease, e.g. progression or recurrence after platinum-containing therapy, occurring no sooner than 6 months after completion of the last dose of platinum chemotherapy.
Definition Recurrence: Evidence of measurable or non-measurable tumor according to RECIST 1.1 criteria by imaging with or without increase of tumor marker CA-125 ≥ 2x ULN OR histologically confirmed tumor recurrence by biopsy or surgery.
5. Eastern Cooperative Oncology Group (ECOG) performance status 0 or 1.
6. Women of childbearing potential should not become pregnant while on the study therapy and should not be pregnant at the beginning of treatment. A pregnancy test should be performed on all women of childbearing potential prior to receiving the study therapy. Women of childbearing potential must agree to follow contraceptive guidance during the treatment period and for 6 months after receiving the last dose of the study therapy.
7. The participant provides written informed consent for the trial.
8. Have provided archival tumor tissue sample or newly obtained core or excisional biopsy of a tumor lesion not previously irradiated. Formalin-fixed, paraffin embedded (FFPE) tissue blocks are preferred to slides. Newly obtained biopsies are preferred to archived tissue.
Note: If submitting unstained cut slides, newly cut slides should be submitted to the testing laboratory within 14 days from the date slides are cut.
9. 1. Weiblich, mindestens 18 Jahre alt.
2. Histologisch diagnostiziertes geringgradiges seröses Ovarialkarzinom, Eileiterkrebs oder primärer Peritonealkrebs.
3. Die Patientinnen müssen mindestens eine vorherige platinhaltige Therapie (z.B. Kombination mit Carboplatin oder Cisplatin) abgeschlossen haben. Eine Erhaltungstherapie mit Bevacizumab und/oder Letrozol ist erlaubt. Die neoadjuvante Chemotherapie in der Erstlinientherapie wird als eine Linie gezählt der Therapie. Patienten können, müssen aber nicht, sowohl in der Erst- als auch in der Zweitlinie eine vorherige zytoreduktive Operation haben.
4. Die Patienten müssen eine platinsensitive Erkrankung haben, d.h. eine Progression oder ein Rezidiv nach einer platinhaltigen Therapie, die nicht früher als 6 Monate nach Abschluss der letzten Dosis einer Platin-Chemotherapie auftritt.
Definition Rezidiv: Nachweis eines messbaren oder nicht messbaren Tumors nach den Kriterien von RECIST 1.1 durch Bildgebung mit oder ohne Erhöhung des Tumormarkers CA-125 ≥ 2x ULN ODER histologisch gesichertes Tumorrezidiv durch Biopsie oder Operation.
5. Leistungsstatus der Eastern Cooperative Oncology Group (ECOG) 0 oder 1.
6. Frauen im gebärfähigen Alter dürfen während der Studientherapie nicht schwanger werden und sollten zu Beginn der Behandlung nicht schwanger sein. Ein Schwangerschaftstest sollte bei allen Frauen im gebärfähigen Alter vor Beginn der Studientherapie durchgeführt werden. Frauen im gebärfähigen Alter müssen sich verpflichten, während des Behandlungszeitraums und für 6 Monate nach Erhalt der letzten Dosis der Studientherapie empfängnisverhütende Maßnahmen zu ergreifen.
7. Die Studienteilnehmerin erteilt eine schriftliche Einwilligungserklärung für die Studie.
8. Patientin muss eine archivierte Tumorgewebeprobe oder eine neu gewonnene Kern- oder Exzisionsbiopsie einer Tumorläsion zur Verfügung gestellt haben, die zuvor nicht bestrahlt wurde. Formalin-fixierte, paraffineingebettete (FFPE) Gewebeblöcke werden gegenüber Objektträgern bevorzugt. Neu gewonnene Biopsien werden archiviertem Gewebe vorgezogen.
Hinweis: Bei der Einreichung von ungefärbten geschnittenen Objektträgern sollten neu geschnittene Objektträger innerhalb von 14 Tagen nach dem Schnitt der Objektträger beim Prüflabor eingereicht werden.
9. E.4 | Principal exclusion criteria |
1. High-grade ovarian cancer.
2. Persistent ≥Grade 2 non-hematologic toxicity from prior cancer therapy
3. Has received prior therapy with an anti-PD-1, anti-PD-L1, or anti PD L2 agent or with an agent directed to another stimulatory or co-inhibitory T-cell receptor (e.g., CTLA-4, OX 40, CD137).
4. Is eligible for carboplatin-based doublet therapy in combination with bevacizumab in the relapse situation, since no treatment with bevacizumab has been administered in the first line therapy.
5. Has received prior systemic anti-cancer therapy including investigational agents within 4 weeks.
6. Has received prior radiotherapy within 2 weeks of start of study treatment. Participants must have recovered from all radiation-related toxicities, not require corticosteroids, and not have had radiation pneumonitis. A 1-week washout is permitted for palliative radiation (≤2 weeks of radiotherapy) to non-CNS disease.
7. Has received a live vaccine or live-attenuated vaccine within 30 days prior to the first dose of study drug. Administration of killed vaccines is allowed.
8.Is currently participating in or has participated in a study of an investigational agent or has used an investigational device within 4 weeks prior to the first dose of study treatment.
Note: Participants who have entered the follow-up phase of an investigational study may participate as long as it has been 4 weeks after the last dose of the previous investigational agent.
9. Has a diagnosis of immunodeficiency or is receiving chronic systemic steroid therapy (in dosing exceeding 10 mg daily of prednisone equivalent) or any other form of immunosuppressive therapy within 7 days prior to the first dose of study drug.
10. Has a known additional malignancy that is progressing or has required active treatment within the past 3 years. Note: Participants with basal cell carcinoma of the skin, squamous cell carcinoma of the skin, or carcinoma in situ (e.g. breast carcinoma, cervical cancer in situ) that have undergone potentially curative therapy are not excluded
11. Has known active CNS metastases and/or carcinomatous meningitis. Participants with previously treated brain metastases may participate provided they are radiologically stable, i.e. without evidence of progression for at least 4 weeks by repeat imaging (note that the repeat imaging should be performed during study screening), clinically stable and without requirement of steroid treatment for at least 14 days prior to first dose of study treatment.

13. Has hypersensitivity to any of the study drugs the patient will be treated with and/or to any of the excipients of the study drugs the patient will be treated with.
14. Has hypersensitivity to platin-containing compounds other than carboplatin.
15. Has active autoimmune disease that has required systemic treatment in the past 2 years (i.e. with use of disease modifying agents, corticosteroids or immunosuppressive drugs). Replacement therapy (e.g., thyroxine, insulin, or physiologic corticosteroid replacement therapy for adrenal or pituitary insufficiency, etc.) is not considered a form of systemic treatment.
16. Has a history of (non-infectious) pneumonitis/interstitial lung disease that required steroids or has current pneumonitis/interstitial lung disease.
17. Has an active infection requiring systemic therapy.
18. Has active infection with SARS-CoV-2 (antigen test).
19. Has a history of Human Immunodeficiency Virus (HIV) infection (known HIV1/HIV2 antibodies positive, mandatory testing for HIV during screening is required)
20. Has a history of Hepatitis B (defined as Hepatitis B surface antigen [HBsAg] reactive) or active Hepatitis C virus (defined as HCV RNA [qualitative] has been detected) infection (mandatory testing for Hepatitis B and Hepatitis C during screening is required).
21. Has a known history of active TB (Bacillus Tuberculosis).
22. Has a history or current evidence of any condition, therapy, or laboratory abnormality that might confound the results of the study, interfere with the subject’s participation for the full duration of the study, or is not in the best interest of the subject to participate, in the opinion of the treating investigator.
23. Has had an allogenic tissue/ solid organ transplant.
24. Has known psychiatric or substance abuse disorders that would interfere with cooperation with the requirements of the trial.
25. Is pregnant or breastfeeding, or expecting to conceive within the projected duration of the study, starting with the screening visit through 6 months after the last dose of trial treatment.
26. Patients unable to be regularly followed for any reason (geographic, familiar, social, psychologic, housed in an institution e.g. prison because of a court agreement or administrative order).
27. Subjects that are depending on the sponsor/CRO or investigational site as well as on the investigator.

4. Kommt für eine Carboplatin-basierte Therapie in Kombination mit Bevacizumab in der Rezidivsituation in Frage, da in der Erstlinientherapie keine Behandlung mit Bevacizumab verabreicht wurde.
5. Hat zuvor innerhalb von 4 Wochen eine systemische Anti-Krebs-Therapie einschließlich Prüfpräparate erhalten.
6. Hat innerhalb von 2 Wochen nach Beginn der Studienbehandlung eine vorherige Strahlentherapie erhalten. Die Teilnehmer müssen sich von allen strahlenbedingten Toxizitäten erholt haben, keine Kortikosteroide benötigen und keine Strahlenpneumonitis gehabt haben.
7. Hat innerhalb von 30 Tagen vor der ersten Dosis des Studienmedikaments einen Lebendimpfstoff oder einen abgeschwächten Lebendimpfstoff erhalten. Die Verabreichung von Totimpfstoffen ist erlaubt.
8. Nimmt derzeit an einer Studie mit einem Prüfpräparat teil oder hat daran teilgenommen oder hat innerhalb von 4 Wochen vor der ersten Dosis der Studienbehandlung ein Prüfpräparat verwendet..
9. Hat die Diagnose einer Immunschwäche oder erhält eine chronische systemische Steroidtherapie
10. Hat eine bekannte zusätzliche Malignität, die fortschreitet oder innerhalb der letzten 3 Jahre eine aktive Behandlung erfordert hat. Hinweis: Teilnehmer mit Basalzellkarzinom der Haut, Plattenepithelkarzinom der Haut oder Carcinoma in situ (z.B. Mammakarzinom, Zervixkarzinom in situ), die sich einer potentiell kurativen Therapie unterzogen haben, werden nicht ausgeschlossen.
11. Hat bekannte aktive ZNS-Metastasen und/oder karzinomatöse Meningitis. Teilnehmer mit zuvor behandelten Hirnmetastasen können teilnehmen, vorausgesetzt, sie sind radiologisch stabil, d.h. ohne Anzeichen einer Progression für mindestens 4 Wochen durch wiederholte Bildgebung (beachten Sie, dass die wiederholte Bildgebung während des Studienscreenings durchgeführt werden sollte), klinisch stabil und ohne die Notwendigkeit einer Steroidbehandlung für mindestens 14 Tage vor der ersten Dosis der Studienbehandlung.
