E.3 | Principal inclusion criteria |
Inclusion criteria: Patients must meet all of the following criteria to be eligible for enrolment: 1. The patient has a definite diagnosis of Wolfram syndrome, as determined by the following: a. Documented diabetes mellitus diagnosed under 16 completed years according to WHO or ADA criteria OR documented optic atrophy diagnosed under 16 completed years AND b. Documented functionally relevant mutations on one or both alleles of the WFS1 gene based on historical test results (if available) or from a qualified laboratory at screening 2. The patient has sensorineural hearing loss of at least 20 db at 8 kHz in HFA 3. The patient is 13 years of age or older, and has a body-weight over 37.5 kg 4. Written informed consent for the principal study 5. Women of childbearing potential who are prescribed with sodium valproate must use effective contraception without interruption during the entire duration of treatment and at least 90 days after last administration . 6. Women with childbearing potential are required to have a confirmed negative blood pregnancy test before starting medication administration at baseline. Women with childbearing potential agree to repeat blood pregnancy tests during at each study visit. 7. Sexually active men with a female partner of childbearing potential must agree to the use of condoms and the use of a effective method of contraception by the female partner.

Les patients doivent satisfaire à tous les critères suivants pour pouvoir être inclus dans l’étude : 1. Patient ayant un diagnostic validé de syndrome de Wolfram, déterminé comme suit : a. diabète sucré documenté, diagnostiqué avant 16 ans révolus selon les critères de l’OMS ou de l’ADA OU atrophie optique documentée diagnostiquée avant 16 ans révolus ET b. des mutations fonctionnellement pertinentes documentées sur un ou deux allèles du gène WFS1 sur la base de résultats de tests précédents (le cas échéant) ou provenant d’un laboratoire qualifié lors de l’inclusion 2. Patient présentant une perte auditive neurosensorielle d’au moins 20 dB à une HFA de 8 kHz 3. Patient âgé d’au moins 13 ans et avec un poids corporel de plus de 37,5 kg 4. Consentement éclairé écrit pour l’étude principale 5. Les femmes en âge de procréer qui se voient prescrire du valproate de sodium doivent utiliser une méthode de contraception efficace sans interruption pendant toute la durée du traitement et au moins 90 jours après la dernière administration. 6. Les femmes en âge de procréer doivent présenter un test de grossesse sanguin négatif avant le début de l’administration du médicament à l’inclusion. Les femmes en âge de procréer acceptent que plusieurs tests de grossesse soient réalisés pendant toute la durée de l’étude. 7. Les hommes sexuellement actifs ayant pour partenaire une femme en âge de procréer doivent accepter d’utiliser des préservatifs et que leur partenaire emploie une méthode contraceptive efficace.

Patients will be excluded from the study for any of the following reasons: 1. The patient is unable or unwilling to comply with the protocol requirements 2. The patient has received treatment with any investigational drug within the 30 days prior to the screening visit 3. The patient is currently taking VPA 4. The patient has an history of allergy or hypersensitivity to VPA or its excipients/ingredients 5. The patient is known to be affected by a pathology for which the symptoms or associated treatments can alter the hearing function and/or affect the ear 6. The patient has clinically significant non-Wolfram related CNS involvement which is judged by the Investigator to be likely to interfere with the accurate administration and interpretation of protocol assessments 7. The patient has a contra-indication to VPA: mitochondrial disorders caused by mutations in the nuclear gene encoding the mitochondrial enzyme polymerase γ (POLG), e.g. Alpers-Huttenlocher Syndrome, active liver disease, personal or family history of liver dysfunction related to known genetic disorders, porphyria, lactose intolerance, the Lapp lactase deficiency, glucose- galactose malabsorption, urea cycle disorders… 8. Any other acute or chronic medical, psychiatric, social situation or laboratory result that, based on Investigator's judgment, would jeopardize patient safety during trial participation, cause inability to comply with the protocol, or affect the Trial participation 9. The patient has a known history of central apnea 10. An unwillingness on the part of male patients to use highly effective form of birth control if engaging in sexual intercourse with a woman who could become pregnant from the time of the first dose of study medication until completion of follow-up procedures. 11. An unwillingness on the part of female patients to use highly effective form of birth control if engaging in sexual intercourse and to have frequent pregnancy tests during treatment and until completion of follow-up procedures. 12. The patient is currently pregnant or breastfeeding 13. The patient has swallowing disorders

Les patients ne pourront être inclus s’ils présentent un des critères suivants 1) Le patient n’est pas apte ou ne souhaite pas respecter les exigences du protocole 2) Le patient a reçu un traitement par un médicament expérimental dans les 30 jours avant la visite de sélection 3) Le patient prend actuellement du VPA 4) Le patient a des antécédents d’allergie ou d’hypersensibilité au VPA ou à ses excipients/ingrédients 5) Le patient présente une pathologie connue pour laquelle les symptômes ou traitements associés peuvent modifier la fonction auditive et/ou affectent l’oreille 6) Le patient présente une implication du SNC cliniquement significative, non liée au syndrome de Wolfram, qui est considérée par l’investigateur comme étant susceptible d’interférer avec l’administration et l’interprétation exactes des évaluations du protocole 7) Le patient présente des contre-indications au VPA : troubles mitochondriaux provoqués par des mutations des gènes nucléaires codant pour la polymérase enzymatique mitochondriale γ (POLG), par exemple le syndrome d’Alpers-Huttenlocher, une maladie hépatique active, des antécédents personnels ou familiaux de dysfonctionnement hépatique d’origine génétique, une porphyrie, une intolérance au lactose, un déficit en lactase de Lapp, une malabsorption du glucose-galactose, des troubles du cycle de l’urée… 8) Toute autre affection médicale ou psychiatrique aiguë ou chronique, situation sociale ou résultats d’analyses qui, de l’avis de l’investigateur, nuirait à la sécurité du patient pendant sa participation à l’étude, entraînerait une incapacité à respecter le protocole, ou affecterait la participation à l’étude 9) Le patient présente des antécédents connus d’apnée centrale 10) Les patients de sexe masculin refusant d’utiliser une forme hautement efficace de moyen contraceptif s’ils ont des relations sexuelles avec une femme en âge de procréer depuis le moment de l’administration de la première dose du médicament à l’étude jusqu’à la fin des procédures de suivi. 11) Un refus de la part des patientes d’utiliser une forme hautement efficace de moyen contraceptif si elles ont des relations sexuelles avec des hommes et de pratiquer des tests de grossesse réguliers pendant le traitement et jusqu’à la fin des procédures de suivi. 12) La patiente est enceinte ou allaite 13) Le patient présente des troubles de la déglutition

- Safety Overall incidence of adverse events and serious adverse events, patient withdrawals as well as changes from baseline in laboratory safety assessments and vital signs parameters will be evaluated for each group and for the study as a whole.
- Efficacy: o Ventral Pons Volume measured and recorded in mm3 by standardised analysis of MRI at baseline, at visit 8 (Week 52) and at the final visit o Insulin and or desmopressin requirements will be assessed whenever the patient is under one or both treatments in order to document potential benefit from VPA on diabetes mellitus or diabetes insipidus o Visual acuity will be assessed using standard ETDRS measures and visual field recording at baseline, every six months during the first year of follow-up and at the final visit o Retinal nerve thikness measure by OCT measures at baseline, every six months during the first year of follow-up and at the final visit o Balance, measured by Mini-BESTest at baseline, at visit 8 (Week 52) and at the final visit o Sleep will be investigated by a specific Sleep questionnaire at baseline, at visit 8 (Week 52) and final visit: o For patients under 18 years: sleeping habits measured by the Pediatric Sleep Questionnaire (PSQ) Parent Questionnaire. o For adults: sleeping habits, measured by the Pittsburg Sleep Quality Index (PSQI) Self-Report.

- Tolérance L’incidence globale d’évènements indésirables et d’évènements indésirables graves ainsi que des analyses biologiques seront évaluées pour chaque groupe et pour l’étude dans son ensemble. - Efficacité o Volume du pont ventral mesuré et enregistré en mm3 par une analyse normalisée d’une IRM réalisée à l’inclusion, à la semaine 52 (visite 8) et lors de la visite finale o Les besoins en insuline et/ou desmopressine seront évalués si le patient prend l’un ou les deux traitements afin de documenter un bénéfice potentiel du VPA sur le diabète sucré ou le diabète insipide o L’acuité visuelle sera évaluée à l’aide de mesures ETDRS normalisées et le champ visuel enregistré à l’inclusion et tous les six mois durant la première année, ainsi qu’à la visite finale o Mesure de l’épaisseur du nerf optique par OCT à l’inclusion et tous les six mois durant la première année, ainsi qu’à la visite finale o L’équilibre, mesuré par Mini-BESTest à l’inclusion, à la semaine 52 (visite 8) et lors de la visite finale o Le sommeil sera évalué par un questionnaire sur le sommeil spécifique à l’inclusion, à la semaine 52 (visite 8) et lors de la visite finale : o Pour les patients de moins de 18 ans : les habitudes en matière de sommeil seront mesurées par le questionnaire sur le sommeil pédiatrique (PSQ). o Pour les adultes : les habitudes de sommeil seront mesurées par l’indice de qualité du sommeil de Pittsburg (PSQI) auto-évalué.
