E.3 | Principal inclusion criteria |
1. Patient is eligible for adjuvant TMZ treatment based on the current standard of care. 2. Men or women age between 18 and 80 years, inclusive. 3. Able and willing to give informed consent. 4. Grade IV glioma (GBM) confirmed through assessment of surgical specimens by a board-certified neuropathologist. 5. Maximal surgical debulking of the tumor was performed. 6. Combined radiation/TMZ treatment is completed based on the prescribed standard of care regimen. 7. Karnofsky rating 70-100 (See Appendix B). 8. Able to communicate during the Exablate BBBD procedure. 9. Life expectancy of at least 3 months. 10. Contraceptive use by both male and female subjects consistent with local regulations regarding the methods of contraception for those participating in clinical studies (See section 2.5 point 10). |
1. Il paziente è idoneo al trattamento adiuvante con TMZ in base all’attuale standard di cura. 2. Uomini o donne di età compresa tra i 18 e gli 80 anni, compresi. 3. Il paziente deve essere in grado e disposto a fornire il consenso informato. 4. E.4 | Principal exclusion criteria |
1. Patients presenting with the following imaging characteristics: i. Evidence of acute intracranial hemorrhage. 2. The sonication pathway to the tumor involves: i. More than 30% of the skull area traversed by the sonication pathway is covered by scars, scalp disorders (e.g., eczema), or atrophy of the scalp. 3. The subject presents with severe symptoms and signs of increased intracranial pressure (e.g., headache, nausea, vomiting, lethargy, and papilledema). 4. Sever midline shift or evidence of subfalcine, uncal, or tonsillar herniation on pre-procedure imaging. 5. Patients with cerebellar or brainstem tumors. 7. Significant depression not adequately controlled with medication and at potential risk of suicide. 8. Patients with brain tumors containing 1p/19q chromosomal co-deletion (consistent with oligodendroglioma). 9. Patient receiving bevacizumab (Avastin) therapy. 10. Patients receiving treatment with corticosteroid doses greater than dexamethasone 24mg daily (or equivalent). 11. Patients undergoing other concurrent therapies such as chemotherapy wafers, immunotoxins delivered by convection-enhanced delivery, regionally administered gene and viral therapies, immunotherapies, focal irradiation with brachytherapy, stereotactic radiosurgery, laser interstitial thermotherapy, and tumor treatment fields therapy. These regimens have been shown to cause contrast enhancement in the resection cavity boundary, which can be difficult to differentiate from true tumor recurrence [35] [36], [37-39]. 12. Anti-coagulant therapy, or medications known to increase risk of hemorrhage within washout period prior to treatment (i.e., antiplatelet or vitamin K inhibitor anticoagulants within 7 days, non-vitamin K inhibitor anticoagulants within 72 hours, or heparin-derived compounds within 48 hours of treatment). 13. History of a bleeding disorder and/or coagulopathy. 14. Abnormal level of platelets (< 100,000) or INR > 1.3. 15. Cerebral vasculopathy, including intracranial thrombosis, vascular malformation, cerebral aneurysm or vasculitis. 16. Evidence of new focal neurological deficits including, but not limited to, motor weakness or speech impairment within 7-14 days prior to the first BBBD procedure. 17. Severely impaired renal function with estimated glomerular filtration rate <30 mL/min/1.73m2 and/or on dialysis. 18. Right to left or bi-directional cardiac shunt. 19. Subjects with evidence of cranial or systemic infection. 20. Subjects with significant liver dysfunction, e.g., history of cirrhosis (hemochromatosis or severe alcohol abuse), or active hepatitis (autoimmune or infectious) with elevated AST, ALT INR or bilirubin (ALT: Male 21-72 units/L; Female 9-52 units/L; AST: Male 17-59 units/L, Female 14-36 units/L; INR >1.3; bilirubin >5 times lab normal). |
1. Pazienti che presentano le seguenti caratteristiche di imaging: i. Segni evidenti di emorragia intracranica acuta. 2. Il percorso della sonicazione al tumore coinvolge: 1. Oltre il 30% dell’area del cranio attraversata dal percorso di sonicazione è ricoperto da cicatrici, problemi del cuoio capelluto (ad es. eczema) o atrofia del cuoio capelluto. 3. Il soggetto presenta sintomi gravi e segni di aumento della pressione intracranica (ad es. emicrania, nausea, vomito, letargia e papilledema). 4. Grave spostamento della linea mediana o evidenza di ernia subfalcina, uncale o tonsillare su immagini pre-procedura. 5. Pazienti con tumori cerebrali o al tronco cerebrale. 6. Pazienti positivi al test dell’HIV. 7. Depressione significativa non adeguatamente controllata con farmaci e a potenziale rischio di suicidio. 8. Pazienti con tumori cerebrali con codelezione dei cromosomi 1p/19q (coerente con l’oligodendroglioma). 9. Pazienti sottoposti a la terapia con bevacizumab (Avastin). 10. Pazienti che assumono dosi di corticosteroidi superiori al desametasone 24 mg al giorno (o equivalente). 11. Pazienti sottoposti ad altre terapie concomitanti come la chemioterapia mediante ostie, immunotossine somministrate con metodica CED (convection-enhanced delivery), terapie geniche e virali somministrate a livello locale, immunoterapie, irradiazione focale con brachiterapia, radiochirurgia stereotassica, termoterapia interstiziale laser e terapia con campi TTF. È stato dimostrato che questi regimi causano un aumento del contrasto nel confine della cavità di resezione, che può essere difficile da differenziare dalla vera recidiva tumorale [35] [36], [37-39]. 12. Terapia anticoagulante o farmaci noti per aumentare il rischio di emorragia entro il periodo di wash-out prima del trattamento (ad es., antiaggreganti piastrinici o anticoagulanti inibitori della vitamina K entro 7 giorni, anticoagulanti non inibitori della vitamina K entro 72 ore o composti derivati dall’eparina entro 48 ore dal trattamento). 13. Storia di un disturbo emorragico e/o coagulopatia. 14. Livello anomalo di piastrine (< 100.000) o INR > 1,3. 15. Vasculopatia cerebrale, inclusi trombosi intracranica, malformazione vascolare, aneurisma cerebrale o vasculite. 16. Segni evidenti di nuovi deficit neurologici focali, tra cui a titolo esemplificativo debolezza motoria o disturbi del linguaggio, nei 7–14 giorni precedenti la procedura di rottura della BEE.
17. Funzionalità renale gravemente compromessa con tasso stimato di filtrazione glomerulare < 30 mL/min/1,73 m2 e/o in dialisi. 18. Shunt cardiaco da destra a sinistra o bidirezionale. 19. Safety: Safety of the BBBD procedure will be evaluated through patient examination and MRI assessments during the treatment and by their standard of care follow-up MRI scans and clinical visits. The standard of care follow-up MRI scans will be used to continue safety monitoring post-BBBD procedures and after adjuvant TMZ chemotherapy is completed.
Feasibility: Feasibility of BBBD will be evaluated through assessment of post-sonication contrast-enhanced MR imaging and comparing it with pre-sonication imaging. The relative increase in the amount of contrast uptake within the sonicated region will be used as a measure of the extent of BBB disruption due to sonication. A comparison of the MRI findings of changes in tumor progression compared to other similar tumor regions within the same patient will be evaluated using standard of care MR imaging. |
Sicurezza. La sicurezza della procedura di rottura della BEE verrà valutata attraverso l’esame del paziente e le valutazioni MRI durante il trattamento e attraverso le scansioni MRI e visite cliniche di follow-up previste dallo standard di cura. Le scansioni MRI di follow-up previste dallo standard di cura verranno utilizzate per proseguire il monitoraggio della sicurezza dopo le procedure di rottura della BEE e dopo il completamento della chemioterapia adiuvante con TMZ.
