General Inclusion Criteria for Part 1 and Part 2: 1. Age 18 years or older; 2. Histologically confirmed and/or documented advanced or metastatic tumors who have exhausted the available standard treatment(s) of the respective country and/or progressing from at least one previous systemic therapy and not eligible to further available therapy; 3. At least one measurable or evaluable disease according to RECIST v1.1 Appendix 1); 4. Performance status of ECOG 0-2 (Appendix 3); 5. Estimated life expectancy of at least 12 weeks; 6. Toxicities incurred as a result of previous anti-cancer therapy resolved to ≤ Grade 1 (as defined by NCI CTCAE v5.0, Appendix 2), except for alopecia, lymphopenia assessed as non-clinically significant, sensory neurotoxicity and erectile dysfunction that could be ≤ G2; 7. At least a 4-week interval between the last received radiotherapy and the first scheduled day of dosing with RVU120 (SEL120) (with the exception of palliation radiotherapy which is allowed up to 2 weeks prior the first scheduled day of dosing); 8. Complete recovery from major surgery (stable and < Grade 2 toxicity sequela acceptable); 9. At least 2 weeks beyond high dose systemic corticosteroids (however, low dose corticosteroids < 20 mg prednisone or equivalent daily are permitted); 10. Laboratory values at Screening and or at D1C1 pre-dose: a. Absolute neutrophil count ≥1.5 x 109/L without colony stimulating factor support; b. Platelets 100 x 109/L; c. Hemoglobin ≥9 g/dL (or ≥2.2 mmol/L) without RBC transfusion within 4 weeks; d. Serum albumin ≥ 30g/L (3.0g/dL); e. Total bilirubin <1.5 times the upper limit of normal (ULN); f. AST (SGOT) ≤3 x ULN; ALT (SGPT) ≤3 x ULN; (≤5 x ULN for patients with advanced solid tumors with liver metastases); patients with confirmed bone metastases will be permitted on study with isolated elevations in ALP >5 x ULN; g. Creatinine clearance ≥60 mL/min (Cockroft-Gault formula Appendix 4); h. Normal coagulation (elevated INR, prothrombin time or APTT <1.3 x ULN acceptable); i. Urine protein ≤1+(as measured by dipstick) or urinary protein <1 g24/hrs. 11. Left ventricular ejection fraction> 50% by echocardiogram or MUGA; 12. Able to provide an archival or fresh tumor biopsy sample at Screening. For patients in Part 2 of study, baseline tumor biopsy samples from progressive disease lesions, where feasible, are required; 13. For females of childbearing potential (FCBP), a negative pregnancy test must be confirmed before enrolment. FCBP must commit to using two medically accepted forms of effective contraception during study participation and until 6 months after the last dose of study drug. Females must also refrain from donating blood or egg (ovum) during the same time-period; 14. For males, an effective barrier method of contraception must be used during study participation until 6 months after the last dose of study drug, if the patient is sexually active with a FCBP. Males must also refrain from donating blood or sperm during the same time-period; 15. Ability to give written, informed consent prior to any study-specific Screening procedures, with the understanding that the consent may be withdrawn by the patient at any time without prejudice; 16. Capable of understanding the mandated and optional protocol requirements, is willing and able to comply with the study protocol procedures and has signed the main informed document prior to any study specific procedure. For any optional biopsy sampling (tissue and/or blood) and long-term sample storage, additional consent is required; 17. Patients must have been off anti-cancer treatment and prohibited concomitant medications, for 2 weeks or 5 half-lives, whichever is longer;
Specific Additional Inclusion Criteria for Part 2 (Group A: TNBC) 18. Histologically or cytologically confirmed TNBC based on the most recent analyzed biopsy or other pathology specimen. Triple negative defined as <1% expression for estrogen receptor (ER) and progesterone receptor (PR) and negative for human epidermal growth factor receptor 2 (HER2) by a diagnostic in situ hybridization method. |
Criterios generales de inclusión en la Parte 1 y Parte 2 : 1. Mayor de 18 años; 2. General Exclusion Criteria for both Part 1 and Part 2: Any of the following will exclude a patient from enrolment: 1. Active brain metastasis [patients with treated, non-progressive brain metastases, off high-dose steroids (>20 mg prednisone or equivalent) for at least 4 weeks can be enrolled in the trial]; 2. Prior history of or planned organ or hematopoietic stem cell transplant; 3. Evidence of ongoing and uncontrolled systemic bacterial, fungal, or viral infection and acute inflammatory conditions (including pancreatitis); 4. Known seropositivity or history of active viral infection with human immunodeficiency virus (HIV); 5. Ongoing significant liver disease such as cirrhosis, drug-induced liver injury, active hepatitis or chronic persistent hepatitis B and/or C; 6. Impairment of gastrointestinal function or gastrointestinal disease that may significantly alter the absorption of RVU120 (SEL120) (e.g., active inflammatory bowel disease, ulcerative disease, malabsorption syndrome, short bowel syndrome, uncontrolled nausea, persistent vomiting or diarrhea); 7. Ongoing drug-induced pneumonitis; 8. Concurrent participation in another investigational clinical trial; 9. Taking any medications, herbal supplements or other substances (including smoking) that are known to be strong inhibitors or strong inducers or sensitive substrates of CYP1A2; with the exception of antibiotics, antifungals, and antivirals that are used as the standard of care or to prevent or treat infections and other such drugs that are considered absolutely essential for the care of the patient and no suitable or available alternative could be found, with prior approval of the Sponsor Study Medical Director (Appendix 5); 10. Mean QTcF or QTcB of >470 msec on triplicate electrocardiograms (ECGs) performed within 5 minutes of each other, using QTcF (Fredericia) or QTcB (Bazett) formulation; 11. Currently taking drugs that are documented in the drug package insert, to have risk of causing prolonged QTc or torsades de pointes (TdP) (unless these can be changed to acceptable alternatives or discontinued). This includes: Myocardial infarction or unstable angina < 6 months prior to Screening; NYHA Grade III or greater congestive heart failure (Appendix 7); cerebrovascular accident including transient ischemic attack within the past 6 months; Uncontrolled hypertension; Serious or uncontrolled cardiac arrhythmia; Personal history of Torsade de Pointe or syndrome of congenital QTc prolongation or QTc > 470 msec.; 13. Any other prior or current medical condition, intercurrent illness, surgical history, physical or electrocardiogram (ECG) findings, laboratory abnormalities, or extenuating circumstance (e.g., alcohol or drug addiction) that, in the Investigator’s opinion, could jeopardize patient safety or interfere with the objectives of the study; 14. Pregnant or breast-feeding females; 15. Known active ongoing infection with COVID19 virus or known persistent Positive COVID-19 Ag-based test;
Specific Exclusion Criteria for Part 2, Group A: 16. Prior history of malignancies other than TNBC, unless the patient has been free of the disease for 5 years or more prior to Screening. Exceptions to the ≥5-year time limit include history of the following: a. basal cell carcinoma of the skin; b. non-metastatic squamous cell carcinoma of the skin; c. carcinoma in situ of the cervix; d. carcinoma in situ of the breast; e. carcinoma in situ of the bladder; f. incidental histological finding of prostate cancer (Tumor/Node/Metastasis [TNM] stage of T1a or T1b). |

PART 2 Primary Efficacy Objective To explore the anti-tumor activity of RVU120 (SEL120) as single agent in patients with selected tumor indications at each study Group: • As assessed by RECIST v1.1 determining objective overall response rate (ORR) defined by patients who experienced a complete response (CR) or partial response (PR), duration of response (DoR), progression-free survival (PFS) and overall survival (OS); and clinical benefit rate (CBR). CBR is defined as (CR+PR+SD). Stable disease has to last at least 12 weeks. Complete Pathologic Response (CPR) will also be evaluated when applicable to a certain tumor indication, such as Breast cancer. • Imaging for RECIST v1.1 will be performed locally at each trial site using either MRI or CT scan, though whichever is used at baseline screening for a specific patient, must be used throughout the course of the study for that patient. During the study, RECIST v1.1 will be determined by the trial site. Images will be stored centrally by a third-party vendor who will perform a second review at the interim and final analyses. In the event of an inconsistency between the investigational site and the 3rd party reviewer, the result reported by the 3rd party, prevails. |

El CBR se define como (CR + PR + SD). La estabilización de la enfermedad debe persistir durante al menos 12 semanas. También se evaluará la respuesta patológica completa (CPR), según proceda, para indicaciones oncológicas específicas, tales como el cáncer de mama. • Las pruebas de imagen según los criterios RECIST ver.1.1 serán realizadas localmente, en cada centro de investigación, utilizando la RMN o TAC, y la prueba que se utilice primero durante la selección de un paciente determinado deberá seguir realizándose en ese paciente durante todo el ensayo. PART 1 Secondary Objectives Efficacy: To determine preliminary anti-tumor response to RVU120 (SEL120): • As assessed by RECIST v1.1 (Appendix 1) determining objective overall response rate (ORR) defined by patients who experienced a complete response (CR) or partial response (PR), duration of response (DoR), progression-free survival (PFS) and overall survival (OS); and clinical benefit rate (CBR). CBR is defined as (CR+PR+SD). Stable disease has to last at least 12 weeks to be considered as component of clinical benefit. Complete Pathologic Response (CPR) will also be evaluated when applicable to a certain tumor indication, such as Breast Cancer. • Imaging for tumor response evaluation based on RECIST v1.1, will be performed locally at each trial site using either MRI or CT scan, though whichever is used at baseline screening for a specific patient, must be used throughout the course of the study for that patient. During the study, tumor response evaluation will be determined by the trial site. Images will be stored centrally by a third-party vendor who will perform a second review, at the time of the interim and final analyses. In the event of an inconsistency between the investigational site and the 3rd party reviewer, the result reported by the 3rd party, prevails.
Pharmacokinetics: to determine pharmacokinetic profile of RVU120 (SEL120) single agent in plasma of patients with solid tumors: • PK variables including maximum concentration (Cmax), minimum concentration (Cmin), time to maximum concentration (Tmax), elimination and effective half-life (t1/2), AUCtau (area under the plasma concentration time curve over a dosing interval) will be measured at prespecified time points.
PART 2 Secondary Objectives Pharmacokinetics: to determine pharmacokinetic profile of RVU120 (SEL120) single agent in patients with solid tumors, at each study Group with sparse sampling (n=3 samples) for population PK modelling: • PK variables including maximum concentration (Cmax), minimum concentration (Cmin), time to maximum concentration (Tmax), elimination half-life (t1/2), AUCtau (area under the plasma concentration time curve over a dosing interval) will be measured if possible, using population PK modelling. • At selected subgroup of 6 patients: Determine the effect of food on the rate of absorption and exposure of RVU120 (SEL120) after a single dose. Determine the effect of RVU120 (SEL120), at steady state, on the exposure of the CYP1A2 probe substrate caffeine. |
PARTE 1 Objetivos secundarios Eficacia: Determinar la respuesta antitumoral preliminar tras la administración de RVU120 (SEL120). • Se evalúa según los criterios RECIST ver. 1.1 (Apéndice 1), que determinan el porcentaje total de respuestas objetivas (ORR), definidas por los pacientes que experimentaron respuesta completa (CR) o respuesta parcial (PR), la duración de la respuesta (DOR), la supervivencia libre de progresión (PFS) y la supervivencia global (OS), así como el porcentaje de beneficio clínico (CBR). El CBR se define como (CR + PR + SD). La estabilización de la enfermedad debe persistir durante al menos 12 semanas para que se considere un componente de beneficio clínico. También se evaluará la respuesta patológica completa (CPR), según proceda, para indicaciones oncológicas específicas, tales como el cáncer de mama. • Las pruebas de imagen para evaluar la respuesta del tumor según los criterios RECIST ver.1.1 serán realizadas localmente, en cada centro de investigación, utilizando la RMN o TAC, y la prueba que se utilice durante la selección de un paciente determinado deberá seguir realizándose en ese paciente durante todo el ensayo. Durante el ensayo, el centro de investigación determinará la respuesta del tumor al tratamiento recibido. Las imágenes serán almacenadas en una entidad central, por una empresa externa que las reevaluará a la hora de llevar a cabo los análisis intermedios y finales. En caso de discrepancia entre la evaluación del centro de investigación y la evaluación del empleado de la empresa externa, prevalecerá la puntuación proporcionada por la empresa externa.
Farmacocinética: determinar el perfil farmacocinético del fármaco RVU120 (SEL120) como agente único en el plasma de los pacientes con tumores sólidos • Las variables PK, incluyen: la concentración máxima (Cmax), la concentración mínima (Cmin), el tiempo hasta la concentración máxima (Tmax), eliminación y vida media efectiva (t1/2) y el AUCtau (área bajo la curva de concentración plasmática - tiempo durante el intervalo de dosis) se determinarán en puntos temporales predefinidos.
PARTE 2 Objetivos secundarios Farmacocinética: determinar, para cada grupo terapéutico, el perfil farmacocinético de RVU120 (SEL120) usado como fármaco único en pacientes con tumores sólidos, para crear un modelo poblacional de PK mediante muestreo disperso (n = 3 muestras) • Las variables PK, incluidas: la exposición total, la concentración máxima (Cmax), la concentración mínima (Cmin), el tiempo hasta la concentración máxima (Tmax), la semivida de eliminación (t1/2) y el AUCtau (área bajo la curva de concentración plasmática - tiempo durante el intervalo de dosis) se determinarán, a medida de lo posible, utilizando el modelo poblacional de PK. • En un subgrupo seleccionado de 6 pacientes: Determinar el efecto de los alimentos en la tasa de absorción y en la exposición a RVU120 (SEL120) tras la administración de una dosis única. • Blood samples for the assessment of the PK profile of RVU120 (SEL120) and its metabolites (as appropriate) in plasma will be taken at Cycle 1, Day 1 (at pre-dose and 0.5, 1, 2, 4, 6, 8, and 24-hours post-dose), at Cycle 1 Day 13 (at pre-dose and 0.5, 1, 2, 4, 6, 8, 24, and 48-hours post-dose). PK blood samples will also be taken at Cycle 1 Day 3 (pre-dose), and at Cycle 1 Day 7 (pre-dose) and 4 hours post-dose. From Cycle 2 onwards a pre-dose, 2-4-hour, 6-8 hour and 24-48-hour post-dose PK blood sample will be taken on Days 1 and 13, and a pre-dose on Day 3 |
