E.3 | Principal inclusion criteria |
1. >= 18 years of age. 2. Radiographic disease progression during or following at least 6 weeks of treatment with ICI for locally advanced or metastatic RCC with a clear cell component either in first- or second-line treatment. Patients must have progressed no longer than 6 months prior to randomization. An ICI is defined by anti-PD-L1 or anti-PD1 antibody including atezolizumab, avelumab, pembrolizumab, or nivolumab. Only patients who have 1 prior line including 1 PD-1 or PD-L1 will be allowed. a. Postoperative or adjuvant systemic therapy can be counted as a prior ICI therapy as long as recurrence is detected within 6 months of completion of treatment, in which case it will be counted as a prior therapy for metastatic disease. One or 2 prior therapies for metastatic disease are permissible but one of the treatments must have been an ICI. b. Subjects must be off all systemic anti-cancer therapy or radiotherapy for at least 2 weeks prior to Cycle 1 Day 1. 3. Subjects must have recovered from the AEs of prior therapy or returned to baseline. Controlled AEs such as hypothyroidism or hypertension are permitted. 4. Histologically or cytologically confirmed RCC with a clear cell component. 5. Measurable disease per the Response Evaluation Criteria in Solid Tumors (RECIST) criteria Version 1.1. 6. Eastern Cooperative Oncology Group (ECOG) performance status of 0 or 1. 7. Life expectancy >= 3 months. 8. If female and of childbearing potential, documentation of negative pregnancy test prior to enrollment. 9. Ability to give written informed consent and comply with protocol requirements. 10. Women of childbearing potential (WOCBP) must meet and/or agree to all of the contraception described in the Protocol, Inclusion Criteria section. 11. Men who are sexually active with WOCBP must meet and/or agree to all of the contraception described in the Protocol, Inclusion Criteria section. |
1. Età >=18 anni. 2. Progressione radiografica della malattia durante o dopo almeno 6 settimane di trattamento con ICI per CCR localmente avanzato o metastatico con una componente a cellule chiare nel trattamento di prima linea o in quello di seconda linea. I pazienti devono aver manifestato una progressione in un periodo non superiore a 6 mesi prima della randomizzazione. Un ICI è definito dalla presenza di anticorpi anti-PD-L1 o anti-PD1, tra cui atezolizumab, avelumab, pembrolizumab o nivolumab. Saranno ammessi solo i pazienti che hanno ricevuto 1 linea precedente, tra cui 1 PD-1 o PD-L1. a. La terapia sistemica postoperatoria o adiuvante può essere conteggiata come terapia ICI precedente, purchè venga rilevata una recidiva entro 6 mesi dal completamento del trattamento, nel qual caso sarà conteggiata come terapia precedente per la malattia metastatica. Sono consentite una o 2 terapie precedenti per la malattia metastatica, ma uno dei trattamenti deve essere stato un ICI. b. I soggetti non devono essere sotto trattamento con nessuna terapia antitumorale sistemiche o con radioterapia da almeno 2 settimane prima del Giorno 1 del Ciclo 1. 3. I soggetti devono essersi ripresi dagli EA della terapia precedente o essere tornati al basale. Sono consentiti EA controllati, come ipotiroidismo o ipertensione. 4. CCR confermato istologicamente o citologicamente con una componente a cellule chiare. 5. Malattia misurabile secondo i Criteri di valutazione della risposta nei tumori solidi (RECIST), versione 1.1. 6. Stato della prestazione pari a 0 o 1 secondo il Gruppo cooperativo orientale di oncologia (ECOG). 7. Aspettativa di vita = 3 mesi. 8. Se di sesso femminile e in età fertile, documentazione di test di gravidanza negativo prima dell’arruolamento. 9. Capacità di fornire il consenso informato scritto e di rispettare i requisiti del protocollo. 10. E.4 | Principal exclusion criteria |
1. More than 2 prior lines of therapy in the advanced or metastatic setting. 2. History of allergy or hypersensitivity to study drug or components. 3. History of life-threatening toxicity related to prior immune therapy (e.g. anti-CTLA-4 or anti-PD-1/PD-L1 treatment or any other antibody or drug specifically targeting T-cell co-stimulation or immune checkpoint pathways) except those that are unlikely to re-occur with standard countermeasures (e.g., hypothyroidism). 4. Active, known, or suspected autoimmune disease. Participants with type I diabetes mellitus, hypothyroidism only requiring hormone replacement, skin disorders (such as vitiligo, psoriasis, or alopecia) not requiring systemic treatment, or conditions not expected to recur in the absence of an external trigger are permitted to enroll. 5. Uncontrolled hypertension: systolic blood pressure > 150 mmHg or diastolic blood pressure > 100 mmHg on 2 or more antihypertensive medications, documented on 2 consecutive measurements taken at least 2 hours apart. Anti-hypertensives must be unchanged for 30 days prior to enrollment. 6. Condition requiring systemic treatment with either corticosteroids (> 10 mg daily prednisone equivalent) within 14 days of treatment initiation or other immunosuppressive medications within 30 days of randomization. Inhaled or topical steroids, and adrenal replacement steroid doses < 10 mg daily prednisone equivalent, are permitted in the absence of active autoimmune disease. 7. Serious or uncontrolled medical disorder 8. Treatment with any live or attenuated vaccine within 30 days of start of treatment. 9. Prior radiation therapy within 2 weeks prior to randomization of start of study treatment. Subjects should have recovered (i.e., Grade <= 1 or at baseline) from radiation-related toxicities. 10. Investigational agents for the treatment of RCC. Patients can be on a prior clinical trial if the specific agent is known (i.e., if any blind is broken). 11. Prior treatment with tivozanib 12. Active autoimmune disease as well as those that required discontinuation of prior IO therapy due to immune mediated AEs. 13. More than 1 prior line of therapy with a checkpoint inhibitor in the metastatic setting. 14. Known central nervous system (CNS) metastases other than stable, treated brain metastases. Subjects with previously treated brain metastasis will be allowed if the brain metastasis has been stable by neuroimaging without steroid treatment for at least 2 months following prior treatment (radiotherapy or surgery). 15. Any of the hematologic abnormalities mentioned in the protocol. 16. Any of the serum chemistry abnormalities mentioned in the protocol. 17. Significant cardiovascular disease. 18. Active peptic ulcer disease, inflammatory bowel disease, ulcerative colitis, or other gastrointestinal condition with increased risk of perforation; history of abdominal fistula, gastrointestinal perforation, or intra-abdominal abscess within 4 weeks prior to administration of first dose of study drug. 19. Serious/active infection or infection requiring parenteral antibiotics. 20. Inadequate recovery from any prior surgical procedure or major surgical procedure within 4 weeks prior to administration of first dose of study drug. 21. Significant thromboembolic or vascular disorders within 6 months prior to administration of first dose of study drug.

È consentito l’arruolamento di partecipanti con diabete mellito di tipo I, ipotiroidismo che richiede solo terapia ormonale sostitutiva, disturbi cutanei (come vitiligine, psoriasi o alopecia) che non richiedono un trattamento sistemico o condizioni che non si prevede si ripresentino in assenza di un fattore scatenante esterno. 5. Ipertensione incontrollata: valori di pressione sanguigna sistolica > 150 mmHg o pressione sanguigna diastolica > 100 mmHg con 2 o più farmaci antipertensivi, documentati in 2 misurazioni consecutive effettuate almeno a 2 ore di distanza l’una dall’altra. Gli antipertensivi devono rimanere invariati nel corso dei 30 giorni precedenti all’arruolamento. 6. Condizione che necessita di trattamento sistemico con corticosteroidi (> 10 mg una volta al giorno di prednisone-equivalente) entro 14 giorni dall’inizio del trattamento o con altri medicinali immunosoppressori entro 30 giorni dalla randomizzazione. Sono consentiti steroidi per via inalatoria o topica e dosi di steroidi surrenalici sostitutivi < 10 mg una volta al giorno di prednisone-equivalente in assenza di malattia autoimmune attiva. 7. Disturbo medico grave o non controllato 8. Trattamento con qualsiasi vaccino vivo o attenuato entro 30 giorni dall’inizio del trattamento. 9. Precedente radioterapia nelle 2 settimane precedenti la randomizzazione all’inizio del trattamento dello studio. I soggetti devono essersi ripresi (ovvero, grado <= 1 o al basale) dalle tossicità correlate alle radiazioni 10. Agenti sperimentali per il trattamento del CCR. I pazienti possono far parte di una precedente sperimentazione clinica se l’agente specifico è noto (ovvero, se viene tolto un eventuale oscuramento). 11. Precedente trattamento con tivozanib 12. Malattia autoimmune attiva, nonché malattie che hanno richiesto l’interruzione della precedente terapia immunoncologica (IO) a causa di EA immuno-mediati 13. Più di 1 precedente linea di terapia con un inibitore del checkpoint nel contesto metastatico. 14. Metastasi note nel sistema nervoso centrale (SNC) diverse dalle metastasi cerebrali trattate e stabili. I soggetti con metastasi cerebrali precedentemente trattate saranno ammessi se la metastasi cerebrale è rimasta stabile al neuroimaging senza trattamento steroideo per almeno 2 mesi dopo il precedente trattamento (radioterapia o intervento chirurgico). 15. Una qualsiasi delle anomalie ematologiche menzionate nel protocollo. 16. Una qualsiasi delle anomalie ematochimiche (siero) menzionate nel protocollo. 17. Significativa malattia cardiovascolare. 18. Ulcera peptica attiva, malattia infiammatoria intestinale, colite ulcerosa o altra condizione gastrointestinale con aumento del rischio di perforazione; anamnesi di fistola addominale, perforazione gastrointestinale o ascesso intra-addominale nelle 4 settimane precedenti la somministrazione della prima dose del farmaco dello studio. 19. Infezione grave/attiva o infezione che richiede antibiotici parenterali. 20.
