Translational Study Objectives
1. To describe potential predictive biomarkers for PM14 and for the combination of PM14 with radiotherapy in tumor samples collected at baseline, by direct-transcriptomics, using HTG Molecular Oncology Biomarker panel (HTG Molecular Diagnostics; Tucson, AZ, USA). 2. To validate PM14 and PM14 plus radiotherapy predictive biomarkers at protein level by immunohistochemistry. 3. To explore the value of liquid biopsy (i.e. plasma) to detect and monitor soluble predictive biomarkers of PM14 and/or radiotherapy, using an Immune Monitoring 65-Plex Human Panel ProcartaPlex Immunoassay (ThermoFisher Scientific; Waltham, MA, USA). 4. To analyze the pharmacodynamics changes, induced by PM14, in terms of DNA damage and apoptosis levels (immunohistochemistry), using the paired tumor samples. 5. To evaluate the pharmacodynamics changes, induced by PM14, in HMG (i.e. HMGA1, HMGA2 and HMGB1) protein levels (ELISA assay), in plasma samples. 6. Objetivos del Estudio Traslacional
1. Describir biomarcadores predictivos potenciales de PM14 y de la combinación de PM14 con radioterapia en muestras tumorales recogidas al inicio del estudio, mediante transcriptómica directa, utilizando el panel de biomarcadores de oncología molecular HTG (HTG Molecular Diagnostics; Tucson, AZ, EE. UU.). 2. Validar biomarcadores predictivos en PM14 y PM14 en combinación con radioterapia a nivel de proteínas mediante inmunohistoquímica. 3. Explorar el valor de la biopsia líquida (es decir, plasma) para detectar y monitorizar biomarcadores predictivos solubles de PM14 y/o radioterapia, utilizando un inmunoensayo ProcartaPlex de panel humano 65-Plex de monitoreo inmunológico (ThermoFisher Scientific; Waltham, MA, EE. UU.). 4. Analizar los cambios farmacodinámicos, inducidos por PM14, en términos de daño del ADN y niveles de apoptosis (inmunohistoquímica), utilizando las muestras tumorales pareadas. 5. E.3 | Principal inclusion criteria |
All the cohorts: - The patient must voluntarily sign the informed consent before any study test is conducted that is not part of routine patient care. - Age: 18-75 years. - Measurable disease according to RECIST v1.1 criteria. - Performance status ≤1 (ECOG). - Men or women of childbearing potential must be using an effective method of contraception before entry into the study and throughout the same and for 3 months (men) and 6 months (women) after ending study treatment. Women of childbearing potential must have a negative serum or urine pregnancy test before study entry. - Normal cardiac function with a LVEF ≥ 50% by echocardiogram or MUGA. - HBV and HCV serologies must be performed prior to inclusion. If HbsAg is positive it is recommended to reject the existence of replicative phase (HbaAg+, DNA VHB+). If these were positives the inclusion is not recommended, remaining at investigators' discretion the preventive treatment with lamivudine. If a potential patient is positive for anti-HCV antibodies, presence of the virus should be ruled out with a qualitative PCR, or the patient should NOT be included in the study (if a qualitative PCR cannot be performed then patient will not be able to enter the study). - Patient must have a central venous catheter for PM14 treatment.
Cohorts A, B, E, and F: PM14 monotherapy in advanced disease - Patients must have a diagnosis of soft tissue sarcoma with metastasis, and not suitable for metastasectomy or surgery resection or not oncologically recommended metastasectomy. A centralized diagnosis confirmation will be performed and the tumor sample must be available and sent prior to inclusion to this end. - A centralized diagnosis of DD liposarcoma or mixoid/hypercellular liposarcoma or leiomyosarcoma must be confirmed for patients in cohort E. - A centralized diagnosis of other sarcomas includes: undifferentiated pleomorphic sarcoma (UPS), myxofibrosarcoma, synovial sarcoma, malignant peripheral nerve sheath tumors, sarcoma NOS, fibrosarcoma, pleomorphic rhabdomyosarcoma, pleomorphic liposarcoma, epithelioid sarcoma, clear cell sarcoma, dedifferentiated or aggressive features in solitary fibrous tumor, extraskeletal myxoid chondrosarcoma, angiosarcoma, epithelioid hemangioendothelioma, - Patients must have received a previous chemotherapy line in advanced disease unless contraindicated or not indicated. - Radiological disease progression must be documented within 6 months prior to study entry. - The patient must have been considered eligible for systemic chemotherapy. A maximum of two previous lines for advanced/metastatic disease are allowed.
Cohort C: Best scheme of PM14 plus RTP in advanced disease
- Patients must have a diagnosis of advanced soft tissue sarcoma, or recurrent head & neck suitable for reirradiation or other advanced or metastatic solid tumor not suitable for metastasectomy or surgery resection or not oncologically recommended metastasectomy. A centralized diagnosis will be performed and the tumor sample must be available and sent prior to inclusion to this end. For phase II part, only soft tissue sarcomas will be enrolled. - Patients must have received a previous chemotherapy line in advanced disease. - Disease distribution must allow meeting with normal tissue constrains of radiation therapy. Radiation oncologist must confirm this point at local sites. - Metastatic spread could be present in several organs (i.e. lungs and pelvic fossa) however, not all the locations have to be irradiated. - Those lesions considered for radiation therapy have to be related to symptoms (for phase II). - It is allowed that not all the lesions will be under radiation fields. As a general rule, the priority is to select, as target-irradiating lesions, those with greater increase in size and those largest lesions if related with symptoms. Irradiating pulmonary lesions with infiltration of pleural serosa is discouraged. - Radiological disease progression must be documented within 6 months prior to study entry. - The patient must have been considered eligible for systemic chemotherapy. A maximum of two previous lines for advanced/metastatic disease are allowed. - The following histological subtypes can be included for the phase II part (central pathology review is mandatory before accrual): undifferentiated pleomorphic sarcoma (UPS), leiomyosarcoma, angiosarcoma, epithelial hemangioendothelioma, liposarcoma and its variants (well differentiated, dedifferentiated, myxoid/round cell, pleomorphic), synovial sarcoma, fibrosarcoma and its variants (epithelial fibrosarcoma/low grade fibromyxoid sarcoma), solitary fibrous tumor, malignant peripheral nerve sheath tumor (MPNST), myxofibrosarcoma, epithelioid sarcoma and (NOS) unclassified sarcoma.

Común en todas las cohortes: - El paciente debe firmar voluntariamente el consentimiento informado antes de realizar cualquier prueba del estudio que no forme parte de la atención de rutina del paciente. - Edad: 18-75 años. - Enfermedad medible según los criterios RECIST v1.1. - Estado funcional ≤1 (ECOG). - Los hombres o mujeres en edad fértil deben utilizar un método anticonceptivo eficaz antes de la inclusión en el estudio y durante el mismo y durante 3 meses (hombres) y 6 meses (mujeres) después de finalizar el tratamiento del estudio. Las mujeres en edad fértil deben tener una prueba de embarazo en suero u orina negativa antes de la inclusión en el estudio. - Función cardíaca normal con FEVI ≥ 50% por ecocardiograma o MUGA. - Se deben realizar serologías para VHB y VHC antes de la inclusión. Si HbsAg es positivo se recomienda rechazar la existencia de fase replicativa (HbaAg +, DNA VHB +). Si estos fueran positivos no se recomienda la inclusión, quedando a criterio de los investigadores el tratamiento preventivo con lamivudina. Si un paciente potencial es positivo para anticuerpos anti-VHC, se debe descartar la presencia del virus con una PCR cualitativa, o el paciente NO debe ser incluido en el estudio (si no se puede realizar una PCR cualitativa, el paciente no podrá entrar al estudio). - El paciente debe tener un catéter central venoso para el tratamiento de PM14.
Cohortes A, B, E y F: - Los pacientes deben tener un diagnóstico de sarcoma de tejido blando con metástasis y no ser aptos para metastasectomía o resección quirúrgica o metastasectomía no recomendada oncológicamente. Se realizará una confirmación del diagnóstico centralizado y la muestra del tumor deberá estar disponible y enviarse antes de su inclusión para este fin. - Se debe confirmar un diagnóstico centralizado de liposarcoma DD o liposarcoma mixoide / hipercelular o leiomiosarcoma para los pacientes de la cohorte E. - Un diagnóstico centralizado de otros sarcomas incluye: sarcoma pleomórfico indiferenciado (SPI), mixofibrosarcoma, sarcoma sinovial, tumores malignos de la vaina del nervio periférico, sarcoma SAI, fibrosarcoma, rabdomiosarcoma pleomórfico, liposarcoma pleomórfico, sarcoma epitelioide, sarcoma de células claras o sarcoma de células claras agresivo, en tumor fibroso solitario, condrosarcoma mixoide extraesquelético, angiosarcoma, hemangioendotelioma epitelioide, - Los pacientes deben haber recibido una línea previa de quimioterapia en enfermedad avanzada a menos que esté contraindicado o no indicado. - La progresión de la enfermedad radiológica debe documentarse dentro de los 6 meses anteriores a la inclusión en el estudio. - El paciente debe haber sido considerado elegible para quimioterapia sistémica. Se permite un máximo de dos líneas previas para enfermedad avanzada/metastásica. - Función adecuada de la médula ósea (hemoglobina> 10 g/dL, neutrófilos ≥ 1.500 / mm^3, plaquetas ≥ 100.000/mm^3). Se aceptan pacientes con aclaramiento de creatinina ≥ 30 ml/min (fórmula de Cockcroft y Gault), transaminasas ≤ 3,0 veces el LSN, bilirrubina total ≤ LSN.
Cohorte C: Mejor esquema de PM14 más RTP en enfermedad avanzada - Los pacientes deben tener un diagnóstico de sarcoma de tejido blando avanzado o de cabeza y cuello recidivantes adecuados para reirradiación u otro tumor sólido avanzado o metastásico no apto para metastasectomía o resección quirúrgica o metastasectomía no recomendada oncológicamente. Se realizará un diagnóstico centralizado y la muestra del tumor deberá estar disponible y remitida antes de su inclusión para este fin. Para la parte de la fase II, solo se incluirán los sarcomas de tejidos blandos. - Los pacientes deben haber recibido una línea de quimioterapia previa en enfermedad avanzada. - La distribución de la enfermedad debe permitir cumplir con las limitaciones de tejido normal de la radioterapia. El oncólogo radioterapeuta debe confirmar este punto en los centros clínicos. - La diseminación metastásica puede estar presente en varios órganos (es decir, pulmones y fosa pélvica), sin embargo, no es necesario irradiar todas las ubicaciones. - Las lesiones consideradas para radioterapia deben estar relacionadas con los síntomas (para fase II). - Se permite que no todas las lesiones estén bajo campos de radiación. Como regla general, la prioridad es seleccionar, como lesiones diana de irradiación, las de mayor aumento de tamaño y aquellas de mayor tamaño si se relacionan con síntomas. Historia de otro cáncer con menos de 5 años sin enfermedad, con la excepción de carcinoma de células basales o cáncer de cuello uterino in situ adecuadamente tratado. 6. EPOC grave u otras enfermedades pulmonares graves. 7. Enfermedad cardiovascular significativa (por ejemplo, disnea> 2 NYHA). 8. Tratamiento previo con radioterapia. 3. Ubicación del tumor primario diferente a las indicadas en los criterios de inclusión. 4. Subtipos histológicos distintos de los indicados en los criterios de inclusión. 5. Tumor primario irresecable o inoperable. 6. Los pacientes que no den su consentimiento para las muestras biológicas obligatorias (incluidas las necesarias para el estudio traslacional) no pueden participar en el estudio. 7. Estado funcional ≥ 2 (ECOG). 8. Bilirrubina plasmática > LSN. 9. Creatinina > 1.6 mg/dL. 10.
