Patients are eligible to be included in the study only if all the following criteria are met (* is for screening part 1, all criteria for screening part 2) 1. Patients must voluntarily participate in this clinical study. Be willing and able to provide written informed consent form (ICF) prior to any study activity. 2. Male ≥18 years of age on the day of signing the ICF. 3. Patients must have histologically or cytologically confirmed prostate adenocarcinoma. 4. Patients must have received and failed at least one novel hormonal agent (NHA, such as abiraterone, enzalutamide, etc.) before or in mCRPC stage. In addition, patients are allowed to have received and failed up to one novel hormonal therapy in mCRPC stage. Treatment failure is judged by the investigator. 5. Patients confirmed by investigator to be at mCRPC stage when starting the immediate prior treatment with docetaxel. Metastatic disease documented by positive bone scan or metastatic lesions on CT or MRI. Metastatic lesions do not include disease spread to local pelvic lymph nodes or local disease recurrence (e.g., bladder and rectum). 6. Surgically or medically castrated, with serum testosterone levels of ≤50 ng/dL (≤1.73 nmol/L). If the patient is being treated with LHRH agonists/antagonists (patient who have not undergone orchiectomy), this therapy must be continued throughout the study. 7. Completed at least 6 cycles and a maximum of 8 cycles of immediate prior treatment with docetaxel (3 weeks a cycle) with no evidence of PD after last docetaxel treatment. The investigator must check whether the patient has PD according to the following four criteria, taking the baseline of docetaxel treatment as reference (if there is no radiographic tumor evaluation at baseline, the earliest tumor assessment after the first dose of docetaxel will serve as reference). Patients cannot be enrolled if they meet any of the four criteria for PD. a. Radiographic soft-tissue lesions progression according to RECIST v1.1. b. Radiographic bone progression according to Prostate Cancer Clinical Trials Working Group 3 (PCWG3) criteria. Bone progression= appearance of 2 or more new lesions (only positivity on the bone scan defines metastatic disease to bone). Ambiguous results will be confirmed by other imaging modalities (e.g., CT or MRI). c. PSA progression according to PCWG3 criteria. PSA progression is defined as: PSA progression ≥25% increase and ≥2 ng/mL increase from baseline of docetaxel treatment beyond 12 weeks, and which is confirmed by a second value ≥3 weeks later. d. Clinical PD judged by investigator, including but not limited to skeletal-related events (SREs, e.g., asymptomatic or symptomatic fractures, surgery or radiation therapy to bone, or spinal cord compression), tumor related pain progression and other tumor related symptoms or signs. 8. Documented germline/somatic mutation in at least one of the homologous recombination repair genes, including BRCA1, BRCA2, ATM, BARD1, BRIP1, CDK12, CHEK1, CHEK2, FANCL, PALB2, PPP2R2A, RAD51B, RAD51C, RAD51D, and RAD54L that is predicted to be deleterious or suspected deleterious (known or predicted to be detrimental/lead to loss of function) assessed by central laboratory using validated Next-Generation Sequencing (NGS) test from blood samples. 9. Patients have adequate organ functions, as indicated by the following laboratory values (had not received blood transfusion, apheresis infusion, erythropoietin, granulocyte colony- stimulating factor (G-CSF), and other relevant medical support within 14 days before the administration of study drug): a. Haemoglobin ≥9.0 g/dL. b. Absolute neutrophil count (ANC) ≥1.5×109/L. c. Platelet count ≥100×109/L. d. Total bilirubin (TBIL) ≤1.5×institutional upper limit of normal (ULN) (not applicable for patients with Gilbert’s syndrome, whose enrollment should be discussed with the sponsor). e. Aspartate aminotransferase (AST) (serum glutamic oxaloacetic transaminase [SGOT])/alanine aminotransferase (ALT) (serum glutamic pyruvate transaminase [SGPT]) ≤3×institutional ULN unless liver metastases are present in which cases, they must be ≤5×ULN. f. Creatinine clearance ≥45 mL/min estimated using the Cockcroft-Gault equation. g. International normalized ratio (INR), prothrombin time (PT), and activated partial thromboplastin time (APTT) ≤1.5×ULN. It is not applicable for patients with Gilbert’s syndrome, whose enrollment should be discussed with the sponsor. The INR only applies to patients who do not receive therapeutic anti-coagulation. 10. Planned start of first dose of study drug 2 to 8 weeks after the last dose of docetaxel. 11. Patients have an Eastern Cooperative Oncology Group (ECOG) performance status of 0 to 2. 12. Patients have a life expectancy of at least 16 weeks.

Les patients sont éligibles à l’inclusion dans l’étude uniquement s’ils répondent à tous les critères suivants (* concerne la sélection partie 1, tous les critères pour la sélection partie 2) : 1. *Participation volontaire des patients à cette étude clinique. Volonté et capacité à fournir un consentement éclairé écrit (FCE) avant toute activité de l’étude. 2. *Homme âgé de ≥ 18 ans le jour de la signature du FCE. 3. *Présence d’un adénocarcinome de la prostate confirmé sur le plan histologique ou cytologique. 4. Échec d’un traitement antérieur par au moins un nouvel agent hormonal (NAH, tel que : abiratérone, enzalutamide, etc.) avant ou pendant un CPRC au stade CPRCm. En outre, les patients pourront avoir reçu jusqu’à une nouvelle hormonothérapie au stade CPRCm et ne pas avoir répondu à ce traitement. L’échec thérapeutique est jugé par l’investigateur. 5. Stade CPRCm confirmé par l’investigateur au moment de l’instauration du traitement antérieur immédiat par docétaxel. Atteinte métastatique documentée par des résultats positifs à la scintigraphie osseuse ou présence de lésions métastatiques sur les examens TDM ou IRM. Les lésions métastatiques n’incluent pas la propagation de la maladie aux ganglions lymphatiques pelviens locaux ou une récidive locale de la maladie (par exemple, vessie et rectum). 6. Castration chirurgicale ou médicale, avec taux de testostérone ≤ 50 ng/dl (≤ 1,73 nmol/l). Si le patient est traité par agonistes/antagonistes de la LHRH (patient n’ayant pas subi d’orchidectomie), ce traitement doit être poursuivi pendant toute l’étude. 7. Patient ayant terminé au moins 6 cycles et un maximum de 8 cycles de traitement antérieur immédiat par docétaxel (3 semaines par cycle) sans signe de progression de la maladie après le dernier traitement par docétaxel. L’investigateur doit vérifier si le patient a présenté une progression de la maladie conformément aux quatre critères suivants, en prenant les valeurs initiales du traitement par docétaxel comme référence (s’il n’y a pas d’évaluation tumorale radiographique initiale pour référence, l’évaluation tumorale la plus précoce après la première dose de docétaxel servira de référence). Les patients ne pourront pas être recrutés s’ils ne répondent pas à un des quatre critères de progression de la maladie. a. Progression radiographique des tissu mous selon les critères RECIST v1.1. b. Progression osseuse radiographique selon les critères PCWG3 (Prostate Cancer Clinical Trials Working Group 3, Groupe de travail 3 sur le cancer de la prostate) Progression osseuse = apparition d’au moins 2 nouvelles lésions (seule une positivité sur la scintigraphie osseuse définit une atteinte métastatique osseuse). Les résultats ambigus seront confirmés par d’autres modalités d’imagerie (par exemple, TDM ou IRM). c. Progression du taux de PSA selon les critères PCWG3. La progression du taux de PSA est définie ainsi : augmentation de ≥ 25 % et ≥ 2 ng/ml au-delà de l2 semaines par rapport aux valeurs initiales relevées à l’instauration du traitement par docétaxel, confirmée par une deuxième valeur ≥ 3 semaines plus tard. d. Progression clinique de la maladie jugée par l’investigateur, y compris notamment événements liés au squelette (ELS, par exemple fractures asymptomatiques ou symptomatiques, chirurgie ou radiothérapie osseuse, compression médullaire), progression de la douleur liée à la tumeur et autres signes ou symptômes liés à la tumeur. 8. Mutation germinale/somatique documentée dans au moins un des gènes impliqués dans la réparation par recombinaison homologue (HRR), y compris BRCA1, BRCA2, ATM, BARD1, BRIP1, CDK12, CHEK1, CHEK2, FANCL, PALB2, PPP2R2A, RAD51B, RAD51C, RAD51D et RAD54L, prédite d’être délétère ou suspectée d’être délétère (mutation connue ou prédite pour être nocive/conduire à une perte de fonction) d’après l’évaluation du laboratoire central à l’aide d’un essai de séquençage de nouvelle génération (NGS, Next-Generation Sequencing) validé effectué sur des échantillons sanguins. 9. Fonction appropriée des organes, indiquée par les paramètres biologiques suivants (sans administration de transfusions sanguines, aphérèse, érythropoïétine, facteurs de croissance granulocytaire (G-CSF, Granulocyte-colony stimulating factor) et autre soutien médical significatif dans les 14 jours avant l’administration du traitement à l’étude).

Patients who meet any of the following criteria will be excluded from the study (* is for screening part 1) 1. *Previous allogenic bone marrow transplant or double umbilical cord blood transplantation (dUCBT). 2. *Prior treatment with a polyadenosine 5'diphosphoribose polymerisation (PARP) inhibitor, including Senaparib. 3. *In mCRPC stage, any prior systemic therapy except for docetaxel, NHA or necessary ADT with LHRH agonists/antagonist for mCRPC. 4. *Patients with a known hypersensitivity to Senaparib or any of the component of Senaparib. 5. Patients who have other new malignancies within 2 years prior to the first dose of Senaparib will be excluded, except for radically treated basal or squamous cell skin cancer. Patients with other malignancies which have been treated with no relapse within 2 years can be enrolled. 6. Initiating bisphosphonate/denosumab therapy or adjusting bisphosphonate/denosumab dose/regimen within 28 days prior to the first dose of study drug. Patients on a stable bisphosphonate/denosumab regimen are eligible and may continue. 7. Patients who have received chemotherapy (except for docetaxel), targeted therapy, immunotherapy, endocrine therapy (except for necessary ADT with LHRH agonists/antagonist for mCRPC), anti-tumor Chinese herbal medicine or proprietary Chinese medicine treatment or other anti-cancer systemic treatment within 5 half-lives or 28 days (whichever is longer), prior to the first dose of study drug. 8. Patients who have any acute toxicities due to prior chemotherapy and/or radiotherapy that are >2 Grade per National Cancer Institute-Common Terminology Criteria for Adverse Events (NCI-CTCAE) v5.0 with the exception of alopecia. 9. Patients who have participated in a study of an investigational agent and received study therapy or used an investigational device within 28 days of the first dose of study drug. For investigational drugs, the washout period should be 5 half-lives or 28 days whichever is longer. 10. Patients who have received strong inhibitors/inducers of CYP3A4 which cannot be discontinued 21 days prior to the first dose of study drug and withheld throughout the study drug treatment. Patients received phenobarbital/enzalutamide will require a 5-week washout prior to the first dose of study drug. 11. Patients who have undergone a major surgery or radical radiotherapy within 28 days prior to the first dose of study drug, or have undergone a palliative radiotherapy within 14 days prior to the first dose of study drug. 12. *Patients with MDS or AML, or with clinical features suggestive of MDS or AML. 13. Patients with serious acute or chronic infections. 14. Patients who have acute myocardial infarction, unstable angina, stroke, or transient ischemic attack within 6 months prior to the first dose of study drug; patients who have congestive heart failure (New York Heart Association [NYHA] Classification Class ≥II); patients who have severe or uncontrolled hypertension, arrhythmia. 15. Patients with known and symptomatic brain metastasis. Patients with asymptomatic, treated, stable brain metastases without steroids for at least 28 days are eligible for study entry. A radiographic imaging to confirm the absence of brain metastases is not required. 16. Patients with symptomatic or impending spinal cord compression, unless appropriately treated for this and clinically stable and asymptomatic for 28 days prior to the first dose of study drug. 17. Patients who are unable to swallow a whole capsule. Patients have gastrointestinal illnesses that may clinically and significantly affect the absorption of oral medication of study drug, e.g., ulcerative colitis, Crohn’s disease, chronic diarrhea with malabsorption, intractable nausea and vomiting, or have had a surgical procedure which could affect gastrointestinal absorption at discretion of investigators. 18. Patients who have received a live virus or bacterial or RNA vaccination within 28 days prior to the first dose of study drug. 19. Patients known to have a history of alcoholism or drug abuse within 2 years. 20. Patients considered a high risk due to a serious, uncontrolled medical disorder, non- malignant systemic disease at discretion of investigators. Examples included but not limited to, uncontrolled grand mal seizures, hepatic sclerosis, superior vena cava syndrome, extensive interstitial bilateral lung disease or any psychiatric disorder that prohibit obtaining ICF. 21. *Patients are unable or unwilling to abide by the study protocol or cooperate fully with the investigator or designee. |
Les patients répondant à n’importe lequel des critères suivants seront exclus de l’étude (* concerne la sélection partie 1) : 1. *Antécédents de greffe de moelle osseuse allogénique ou de double greffe de sang de cordon ombilical (dGSCO). 2. *Antécédents de traitement par inhibiteur de PARP (polyadenosine 5’diphosphoribose polymerisation, polymérisation du polyadénosine 5’diphosphoribose), y compris senaparib. 3. *Au stade CPRCm, tout traitement systémique antérieur sauf docétaxel, NAH ou ADT nécessaire par agonistes/antagonistes de LHRH pour un CPRCm. 4. *Hypersensibilité connue au senaparib ou à l’un des composants du senaparib. 5. Les patients ayant eu d’autres nouveaux cancers dans les 2 ans avant la première dose de senaparib seront exclus, sauf dans les cas de carcinome spinocellulaire ou de carcinome basocellulaire de la peau traité de manière radicale. Les patients ayant d’autres cancers ayant été traités et qui ne présentent pas de récidive dans les 2 ans peuvent être recrutés. 6. Instauration d’un traitement par bisphosphonates/dénosumab ou ajustement de la dose/du schéma posologique de bisphosphonates/dénosumab dans les 28 jours avant la première dose de traitement à l’étude. Les patients sous schémas posologique stable de bisphosphonates/dénosumab sont éligibles et pourront continuer. 7. Traitement antérieur par chimiothérapie (sauf docétaxel), traitement ciblé, immunothérapie, endocrinothérapie (sauf ADT nécessaire par agonistes/antagonistes de LHRH pour CPRCm), produit de phytothérapie antitumorale chinois ou médicament de médecine traditionnelle chinoise ou tout autre traitement anticancéreux systémique dans les 5 demi-vies ou 28 jours (selon la période la plus longue), avant la première dose de traitement à l’étude. 8. Présence d’une toxicité aiguë due à une chimiothérapie et/ou radiothérapie antérieure, qui est de grade > 2 selon les critères communs de terminologie des évènements indésirables, établis par l’Institut national du cancer (NCI-CTCAE, National Cancer Institute-Common Terminology Criteria for Adverse Events) v5.0 à l’exception d’une alopécie. 9. Participation antérieure à une étude sur un agent expérimental et ayant reçu le traitement étudié ou utilisé un dispositif expérimental dans les 28 jours avant la première dose de traitement à l’étude. Pour les traitements expérimentaux, la période de sevrage doit être de 5 demi-vies ou 28 jours, selon la période la plus longue. 10. Antécédents d’administration d’inhibiteurs/inducteurs puissants du CYP3A4 ne pouvant pas être arrêtés 21 jours avant la première dose de traitement à l’étude et suspendus pendant toute la durée du traitement par le traitement à l’étude. Les patients ayant reçu un traitement par phénobarbital/enzalutamide devront avoir un sevrage de 5 semaines avant la première dose de traitement à l’étude. 11. Antécédents de chirurgie majeure ou de radiothérapie radicale dans les 28 jours avant la première dose de traitement à l’étude ou ayant reçu une radiothérapie palliative dans les 14 jours avant la première dose de traitement à l’étude. 12. *Patients ayant un syndrome myélodysplasique (SMD) ou une leucémie aigüe myéloïde (LAM) ou présentant des caractéristiques cliniques évoquant un SMD ou une LAM. 13. Présence d’infections graves aiguës ou chroniques. 14. Antécédents d’infarctus aigu du myocarde, angor instable, accident vasculaire cérébral (AVC) ou accident ischémique transitoire dans les 6 mois avant la première dose de traitement à l’étude ; insuffisance cardiaque congestive (de classe ≥ II selon la NYHA [New York Heart Association]) ; hypertension, arythmie sévère et non contrôlée. 15. Métastases cérébrales connues et symptomatiques. Les patients ayant des métastases cérébrales asymptomatiques, traitées, stables sans corticoïdes depuis au moins 28 jours sont éligibles pour l’entrée dans l’étude. Un examen d’imagerie radiographique pour confirmer l’absence de métastases cérébrales n’est pas nécessaire. 16. Compression médullaire symptomatique ou imminente, sauf si traitée de façon appropriée, et cliniquement stable et asymptomatique depuis 28 jours avant la première dose de traitement à l’étude. 17. Incapacité à avaler une gélule en entier. Présence de pathologies gastro-intestinales susceptibles d’affecter cliniquement et significativement l’absorption du traitement à l’étude par voie orale, par exemple, rectocolite hémorragique, maladie de Crohn, diarrhée chronique avec malabsorption, nausées et vomissements réfractaires, ou antécédents d’intervention chirurgicale pouvant affecter l’absorption gastro-intestinale, à la discrétion des investigateurs. 18. Antécédents de vaccin antiviral ou antibactérien vivant ou ARN dans les 28 jours précédant la première dose de traitement à l’étude. 19. Antécédents connus d’alcoolisme ou de toxicomanie dans les 2 années précédentes.
