1. Has relapsed or refractory DLBCL and have failed at least 2 lines of prior therapy, and have failed auto-SCT or are auto-SCT ineligible. Must have received prior rituximab/anti-CD20 monoclonal antibody. a Relapsed disease: progression after achieving an overall response of PR or CR in response to the most recent therapy. b Refractory disease: failure to achieve CR or PR to the most recent therapy. c Ineligibility for auto-SCT: Is >65 years old Has organ dysfunction or comorbidities precluding the use of HDT or auto-SCT Has not responded to salvage therapy Has refused auto-SCT Has an inability to successfully collect peripheral blood stem cells 2. Histologically confirmed diagnosis of DLBCL, according to the WHO classification of neoplasms of the hematopoietic and lymphoid tissues. 3. a. Has radiographically measurable DLBCL per the Lugano Response Criteria, with at least 1 nodal lesion (non-irradiated) that is >1.5 cm in the long axis, regardless of length of the short axis, AND/OR extranodal lesion of ≥1.0 cm in the long and short axis. and b. Has PET positive disease verified by BICR at Screening defined as 4-5 on a 5-point scale. 4. Life expectancy of at least 3 months, in the opinion of the investigator. 5. Is male or female, from 18 years of age inclusive, at the time of signing the informed consent. 6. Male participants are eligible to participate if they agree to the following during the intervention period and for at least the time needed to eliminate each study intervention after the last dose of study intervention. The length of time required to continue contraception for each study intervention is: • Zilovertamab vedotin: 110 days • Refrain from donating sperm PLUS either: • Be abstinent from heterosexual intercourse as their preferred and usual lifestyle and agree to remain abstinent OR • Must agree to use contraception unless confirmed to be azoospermic as detailed below: - Agree to use a male condom plus partner use of an additional contraceptive method when having penile-vaginal intercourse with a WOCBP who is not currently pregnant. • Contraceptive use by men should be consistent with local regulations regarding the methods of contraception for those participating in clinical studies. If the contraception requirements in the local label for any of the study interventions is more stringent than the requirements above, the local label requirements are to be followed. 7. A female participant is eligible to participate if she is not pregnant or breastfeeding, and at least one of the following conditions applies: • Is not a WOCBP OR • Is a WOCBP and: - Uses a contraceptive method that is highly effective, with low user dependency, or be abstinent from heterosexual intercourse as their preferred and usual lifestyle (abstinent on a long-term and persistent basis), during the intervention period and for at least the time needed to eliminate each study intervention after the last dose of study intervention and agrees not to donate eggs (ova, oocytes) to others or freeze/store for her own use for the purpose of reproduction during this period. The length of time required to continue contraception for each study intervention is as follows: ◦ Zilovertamab Vedotin 50 days The investigator should evaluate the potential for contraceptive method failure (ie, noncompliance, recently initiated) in relationship to the first dose of study intervention. -Contraceptive use by women should be consistent with local regulations regarding the methods of contraception for those participating in clinical studies. If the contraception requirements in the local label for any of the study interventions is more stringent than the requirements above, the local label requirements are to be followed. - Has a negative highly sensitive pregnancy test (urine or serum as required by local regulations) within 24 hours for urine and 72 hours for serum before the first dose of study intervention. If a urine test cannot be confirmed as negative (eg, an ambiguous result), a serum pregnancy test is required. In such cases, the participant must be excluded from participation if the serum pregnancy result is positive. - Has had her medical history, menstrual history, and recent sexual activity reviewed by the investigator to decrease the risk for inclusion of a woman with an early undetected pregnancy. 8. The participant (or legally acceptable representative) has provided documented informed consent/assent for the study. The participant may also provide consent/assent for FBR. However, the participant may participate in the study without participating in FBR. 9. Archival tumor tissue sample or newly obtained core or excisional biopsy of a tumor lesion not previously irradiated has been provided. 10. Have an ECOG performance status of 0 to 2 assessed within 7 days before time of enrollment. ECOG 2 will be capped at approximately 20%. 11. Have adequate organ function. |

Diagnóstico confirmado histológicamente de LDCBG. La célula de origen del LDCBG es conocida, sin especificar, de tipo linfocitos B de centros germinales o de tipo linfocitos B activados. El LDCBG con sobreexpresión de las proteínas MYC, BCL2 y/o BCL6 sin reordenamiento también se clasifica como LDCBG 3. a. Presencia de un LDCBG radiológicamente mensurable, con al menos una lesión ganglionar (no irradiada) que mida más de 1,5 cm en el eje mayor, con independencia de la longitud del eje menor, Y/O una lesión extraganglionar que mida ≥1,0 cm en los ejes mayor y menor y b. Mujeres participantes 7. En el estudio podrán participar mujeres que no estén embarazadas ni en período de lactancia y que cumplan al menos una de las condiciones siguientes: • No ser una mujer en edad fértil. O • Ser una MEF y: - Utilizar un método anticonceptivo muy eficaz, con baja dependencia de la usuaria, o practicar la abstinencia durante el período de intervención y durante el tiempo necesario para eliminar la medicación después de recibir la última dosis de la misma y comprometerse a no donar óvulos ni congelarlos/conservarlos durante este período (50 días). - El investigador deberá evaluar la posibilidad de fracaso del método anticonceptivo. El uso de anticonceptivos por las mujeres deberá cumplir la normativa local sobre métodos anticonceptivos para participantes en estudios clínicos. Si los requisitos de anticoncepción de la ficha técnica local de cualquiera de las intervenciones del estudio son más estrictos que los requisitos anteriores, deberán seguirse los requisitos de la ficha técnica local. - Dar negativo en una prueba de embarazo de alta sensibilidad realizada en las 24 horas en el caso de la orina, o las 72 horas en el caso del suero, previas a la primera dosis de la intervención del estudio. Cuando no pueda confirmarse que el resultado de una prueba en orina es negativo, será necesario hacer una prueba de embarazo en suero. - El investigador debe haber revisado los antecedentes médicos, los antecedentes menstruales y la actividad sexual reciente para reducir el riesgo de incluir a una mujer con un embarazo de poco tiempo no detectado. 8. El participante otorga su consentimiento informado documentado para el estudio. El participante también podrá otorgar su consentimiento para FBR. 1. Has received a diagnosis of PMBCL. 2. Has undergone solid organ transplant at any time. 3. Has clinically significant (ie, active) cardiovascular disease: cerebral vascular accident/stroke (<6 months prior to enrollment), myocardial infarction (<6 months prior to enrollment), unstable angina, congestive heart failure (New York Heart Association Classification Class ≥ II), serious cardiac arrhythmia requiring medication, arterial thromboembolism, cerebrovascular thromboembolism (<6 months prior to enrollment), uncontrolled Grade ≥3 hypertension (diastolic blood pressure ≥100 mm Hg or systolic blood pressure ≥160 mm Hg) despite antihypertensive therapy; or significant conduction system ECG abnormalities, including second-degree AV block type II, third-degree AV block, or Grade ≥2 bradycardia, or serious cardiac arrhythmia requiring medication. 4. Known history of liver cirrhosis. 5. Has pericardial effusion or clinically significant pleural effusion. 6. Has baseline peripheral neuropathy> Grade 1. 7. Has a history of a second malignancy, unless potentially curative treatment has been completed with no evidence of malignancy for 2 years. 8. Has a demyelinating form of Charcot-Marie-Tooth disease. 9. Has received prior therapy with a ROR1-directed therapy. 10. Has contraindication to any of the study intervention components. 11. Transformed DLBCL from indolent lymphoma. 12. In participants with prior allo-SCT, acute GVHD or ongoing evidence of chronic GVHD manifesting as Grade ≥2 serum bilirubin, Grade ≥3 skin involvement, or Grade ≥3 diarrhea or requiring treatment for their GVHD. 13. Has received prior systemic anticancer therapy, including investigational agents within 4 weeks prior to the first dose of study intervention. 14. Has received prior radiotherapy within 4 weeks of start of study intervention. Participants must have recovered from all radiation-related toxicities, not require corticosteroids, and not have had radiation pneumonitis. A 1-week washout is permitted for palliative radiation (≤2 weeks of radiotherapy) to non-CNS disease. 15. Has ongoing corticosteroid therapy (exceeding 30 mg daily of prednisone equivalent). Prednisone equivalent dosing must have been stable for at least 4 weeks prior to C1D1. 16. Has received a live or live-attenuated vaccine within 30 days before the first dose of study intervention. Administration of killed vaccines are allowed. 17. Has received a strong inhibitor or inducer of CYP3A4 (including itraconazole, ketoconazole, posaconazole, or voriconazole) within 7 days prior to C1D1 or expected requirement for chronic use of a strong CYP3A4 inhibitor or inducer during Cycle 1 of study therapy 18. Is currently participating in or has participated in a study of an investigational agent or has used an investigational device within 4 weeks before the first dose of study intervention. 19. Has known active CNS lymphoma involvement or active CNS involvement by lymphoma. Participants with prior CNS involvement are eligible if their CNS disease is in radiographic, cytological (for cerebrospinal fluid disease), and clinical remission. 20. Has an active infection requiring systemic therapy. 21. Has a known history of HIV infection. No HIV testing is required unless mandated by local health authority. 22. Has a known history of hepatitis B (defined as HBsAg reactive) or known active hepatitis C virus (defined as HCV RNA [qualitative] is detected) infection. 23. Has a history or current evidence of any condition, therapy, or laboratory abnormality that might confound the results of the study, interfere with the participant's participation for the full duration of the study, or is not in the best interest of the participant to participate, in the opinion of the treating investigator. 24. Has a known psychiatric or substance abuse disorder that would interfere with the participant’s ability to cooperate with the requirements of the study. Meals and Dietary Restrictions: Participants should avoid ingestion of grapefruit, grapefruit juice, Seville oranges, or starfruit (all of which contain CYP3A4 inhibitors) and should not use St. John’s wort (which is a potent CYP3A4 inducer). Otherwise, participants should maintain a normal diet unless modifications are required to manage an AE such as diarrhea, nausea, or vomiting. |

Antecedentes conocidos de cirrosis hepática. 5. Presencia de derrame pericárdico o derrame pleural clínicamente significativo. 6. Presencia de neuropatía periférica basal de grado >1. 7. Antecedentes de una segunda neoplasia maligna, a menos que se haya completado un tratamiento potencialmente curativo sin signos de neoplasia maligna durante dos años. 8. No será necesario realizar pruebas de VIH a menos que lo exijan las autoridades sanitarias locales. 22. Antecedentes de infección por el virus de la hepatitis B o de infección activa por el virus de la hepatitis C 23. Antecedentes o datos presentes de cualquier proceso, tratamiento o anomalía analítica que, en opinión del investigador responsable del tratamiento, pueda confundir los resultados del estudio, dificultar la participación durante la totalidad del estudio o motivar que la participación no sea lo más conveniente para el posible participante. 24. Comidas y Restriciones alimentarias Los participantes deberán evitar el consumo de pomelo, el zumo de pomelo, naranjas amargas, carambola (todo lo que pueda contener inhibidores de CYP3A4) y no podrán usar hierba de San Juan (que es un potente inductor del CYP3A4). Aparte de esto, los participantes podrán mantener una dieta normal, a no ser que se necesiten modificaciones para manejar un AA como diarrea, náuseas o vómitos. |
