The participant has received ERT (Cerezyme or other ERT; as deemed appropriate by local regulations) for at least 3 years prior to enrollment, on a stable dose for at least 6 months, is deemed clinically stable for at least 1 year by the Investigator and is within the therapeutic goals as all of the following: - Hemoglobin level of >=11.0 g/dL for females and >=12.0 g/dL for males - Platelet count >=100 000/mm3 - Spleen volume <10 multiples of normal (MN) - Liver volume <1.5 MN - No bone crisis and free of symptomatic bone disease such as bone pain attributable to osteonecrosis and/or pathological fractures within 3 months prior to screening - Adult participant is >=18 years of age - Pediatric participant is >=12 years <18 years of age - The participant has a clinical diagnosis of GD3 and a documented deficiency of acid beta-glucosidase activity confirming this diagnosis. - The participant has a modified SARA score of 1 or above. - The presence of gaze palsy, predominantly horizontal, with slow or absent saccades. - If the participant has a history of seizures, they are well controlled under appropriate medication not identified as a strong or moderate inducer or inhibitor of CYP3A. - Participants >=30 kg of weight - Contraception for sexually active male participants or female patient; not pregnant or breastfeeding; no sperm donating for male participant - Signed written informed assent/consent |
Il/La paziente ha ricevuto TES (Cerezyme o altra TES; come ritenuto appropriato dalle normative locali) per almeno 3 anni prima dell’arruolamento, a una dose stabile per almeno 6 mesi, è considerato/a clinicamente stabile per almeno 1 anno dallo sperimentatore ed entro gli obiettivi terapeutici come tutti i seguenti: - Livello di emoglobina >=11,0 g/dl per i soggetti di sesso femminile e >=12,0 g/dl per i soggetti di sesso maschile - Conta piastrinica >=100.000/mm3 - Volume della milza <10 multipli del normale (MN) - Volume del fegato <1,5 MN - Nessuna crisi ossea e nessuna malattia ossea sintomatica, come dolore osseo attribuibile a osteonecrosi e/o fratture patologiche nei 3 mesi precedenti lo screening. - Paziente adulto/a di età >=18 anni - Paziente pediatrico/a di età compresa tra >=12 anni e <18 anni - Il/La paziente presenta una diagnosi clinica di GD3 e un deficit documentato di attività della beta-glucosidasi acida a conferma di questa diagnosi. - Il/La paziente presenta un punteggio SARA modificato di 1 o superiore. - La presenza di paralisi dello sguardo, prevalentemente orizzontale, con saccadi lente o assenti. - Se il/la paziente presenta un’anamnesi di crisi convulsive, queste sono ben controllate con il farmaco appropriato, non classificato come induttore o inibitore forte o moderato dell’enzima CYP3A. - Pazienti con peso >=30 kg - Contraccezione per partecipanti maschi sessualmente attivi o pazienti di sesso femminile; se non incinta o non sta allattando al seno; evitare la donazione di sperma per i partecipanti maschi - in grado di fornire un assenso/consenso informato firmato |

E.4 | Principal exclusion criteria |
Participants are excluded from the study if any of the following criteria apply: - The participant is blood transfusion-dependent. - Prior esophageal varices or liver infarction or current liver enzymes (alanine aminotransferase [ALT]/ aspartate aminotransferase [AST]) or total bilirubin >2 times the upper limit of normal, unless the participant has a diagnosis of Gilbert Syndrome. - The participant has any clinically significant disease, other than GD, including cardiovascular (congenital cardiac defect, coronary artery disease, valve disease or left sided heart failure; clinically significant arrhythmias or conduction defect), hepatic, gastrointestinal, pulmonary, neurologic, endocrine, metabolic (eg, hypokalemia, hypomagnesemia) or psychiatric disease, other medical conditions, or serious intercurrent illnesses that may preclude participation in the opinion of the Investigator. - The participant has renal insufficiency, as defined by an estimated glomerular filtration rate <30 mL/min/1.73m2 at the screening visit. - The participant has a history of cancer, except for basal cell carcinoma. - The participant has progressive myoclonic epilepsy. - The participant is pregnant (has a positive serum beta-human chronic gonadotropin [ß-hCG]) or lactating. - The participant requires use of invasive ventilatory support. - The participant requires use of noninvasive ventilator support while awake for longer than 12 hours daily. - The participant is scheduled for in-patient hospitalization including elective surgery, during the study. - The participant has had a major organ transplant (eg, bone marrow or liver). - A history of drug and/or alcohol abuse within the past year prior to the screening visit. - Chaperone therapy within 6 months, substrate reduction therapy other than venglustat within 6 months or venglustat substrate reduction therapy prior to enrollment. - Exposure to any investigational drug (including venglustat) within the last 30 days or 5 half-lives from screening, whichever is longer - The participant has received strong or moderate inducers or inhibitors of CYP3A within 14 days or 5 half-lives from screening, whichever is longer, prior to screening. This also includes the consumption of grapefruit, grapefruit juice, or grapefruit containing products within 72 hours of starting venglustat. The participant is unwilling to abstain from consumption of grapefruit, grapefruit juice, or grapefruit containing products for the duration of the treatment period. - The participant, in the opinion of the investigator, is unable to adhere to the requirements of the study or unable to undergo study assessments (eg, contraindication for MRI). - Type of participant and disease characteristic: the participant has had a total splenectomy prior to enrollment. The patient had a partial splenectomy within 3 years prior to randomization. - Participant not suitable for participation, whatever the reason, as judged by the Investigator, including medical or clinical conditions, or participants potentially at risk of noncompliance to study procedures - Sensitivity to any of the study interventions, or components thereof, or drug or other allergy that, in the opinion of the Investigator, contraindicates participation in the study |
In presenza di uno dei seguenti criteri i/le pazienti saranno esclusi dallo studio: - Il/La paziente è dipendente dalla trasfusione di sangue. - Pregresse varici esofagee o infarto del fegato o attuali enzimi epatici (alanina aminotransferasi [ALT]/ aspartato aminotransferasi [AST]) o bilirubina totale >2 volte il limite superiore della norma, a meno che il/la paziente non presenti una diagnosi di sindrome di Gilbert. - Il/la paziente presenta qualsiasi malattia clinicamente significativa, diverso dalla GD, anche cardiovascolare (difetto cardiaco congenito, coronaropatia, malattia valvolare o insufficienza cardiaca sinistra; aritmie clinicamente significative o difetto della conduzione); epatica, gastrointestinale, polmonare, neurologica, endocrina, metabolica (per es., ipokaliemia, ipomagnesiemia) o malattia psichiatrica, altre condizioni mediche, o malattie intercorrenti gravi che potrebbero precludere la partecipazione secondo l’opinione dello sperimentatore. - Il/La paziente presenta insufficienza renale, definita da una velocità di filtrazione glomerulare stimata <30 ml/min/1,73 m2 alla visita di screening. - Il/La paziente presenta un’anamnesi di tumore, a eccezione del carcinoma basocellulare. - Il/La paziente presenta epilessia mioclonica progressiva. - La paziente è incinta (presenta una beta-gonadotropina cronica umana positiva [ß-hCG]) o in allattamento. - Il/La paziente richiede l’uso di un supporto ventilatorio invasivo. - Il/La paziente richiede l’utilizzo di un supporto ventilatorio non invasivo durante la veglia per più di 12 ore al giorno. - Per il/la paziente è stato programmato un ricovero ospedaliero, compresa chirurgia elettiva, durante lo studio. - Il/La paziente è stato sottoposto/a a un trapianto di organo maggiore (per es., midollo osseo o fegato). - Anamnesi di abuso di droghe e/o alcol nell’ultimo anno precedente la prima visita di screening. - Terapia con chaperone entro 6 mesi, terapia di riduzione del substrato diversa da venglustat entro 6 mesi o terapia di riduzione del substrato di venglustat prima dell’arruolamento. - Esposizione a qualsiasi farmaco sperimentale (incluso venglustat) negli ultimi 30 giorni o 5 emivite dallo screening, a seconda di quale periodo sia più lungo. Il/La paziente ha ricevuto induttori forti o moderati o inibitori del CYP3A entro 14 giorni o 5 emivite dallo screening, a seconda di quale periodo sia più lungo, prima dello screening. Ciò comprende anche il consumo di pompelmo, succo di pompelmo o prodotti contenenti pompelmo entro 72 ore dall’inizio della somministrazione di venglustat. Il/La paziente non è disposto ad astenersi dal consumo di pompelmo, succo di pompelmo o prodotti contenenti pompelmo per tutta la durata del periodo di trattamento. - Il/La paziente, a giudizio dello sperimentatore, non è in grado di aderire ai requisiti dello studio o di sottoporsi alle valutazioni dello studio (per es., controindicazione alla RM). - Tipo di paziente e caratteristica della malattia: il/la paziente è stato sottoposto a splenectomia totale prima dell’arruolamento. Il/La paziente è stato sottoposto a splenectomia parziale nei 3 anni precedenti la randomizzazione. - Pazienti non idonei/e alla partecipazione, a giudizio dello sperimentatore, a prescindere dal motivo, comprese condizioni mediche o cliniche, o pazienti potenzialmente a rischio di non aderenza alle procedure dello studio Sensibilità a uno qualsiasi dei trattamenti dello studio o a componenti degli stessi, o al farmaco o altre allergie che, secondo l’opinione dello sperimentatore, rappresentano una controindicazione alla partecipazione al presente studio |
