E.3 | Principal inclusion criteria |
Patients must meet all of the following inclusion criteria to be eligible for participation in this study (no waivers for patient eligibility will be offered or permitted): 1) Female or male patients, regardless of race and/or ethnic group, who are18 years of age or older, able to understand and give written informed consent 2) Patients with locally advanced inoperable or metastatic TNBC who have not received previous systemic therapy for advanced disease and whose tumors are PD-L1 positive at screening. 3) Have provided representative formalin-fixed paraffin-embedded (FFPE) tumor specimen in blocks (preferably) or have at least 20 to 25 freshly sectioned unstained slides from fresh biopsy tissue (preferred) or archival tissue block for central testing of ER, progesterone receptor, HER2, PD-L1, Trop-2, and additional biomarker testing. A baseline biopsy is required if archival tissue is not available and this procedure must be performed prior to the first dose of study treatment and after the patient provides written informed consent. Fine needle aspirates and bone biopsies are not suitable samples. Note: Tumor tissue quality must be confirmed by the central laboratory. Submission of another tumor specimen may be required if provided specimen is not adequate for assessment. 4) ECOG performance status score of 0 or 1 (Appendix 5). 5) Life expectancy = 3 months. 6) Recovered from major surgery for = 2 weeks. 7) Adequate hematologic counts without transfusional or growth factor support within 2 weeks of study treatment initiations (hemoglobin = 9 g/dL, ANC = 1500/mm3, and platelets = 100,000/µL). 8) Adequate hepatic function (bilirubin = 1.5 × ULN, AST and ALT = 2.5 × ULN or = 5 × ULN if known liver metastases, and serum albumin > 3 g/dL). 9) Creatinine clearance = 30 mL/min as assessed by the Cockcroft-Gault equation {Cockcroft 1976}. 10) International normalized ratio (INR)/PT and PTT or aPTT = 1.5 ULN unless patient is currently receiving therapeutic anticoagulant therapy. 11) Male patients and female patients of childbearing potential who engage in heterosexual intercourse must agree to use protocol-specified method(s) of contraception as described in the protocol Appendix 3. 12) Patients with HIV must be on antiretroviral therapy (ART) and have a well-controlled HIV infection/disease. |
I pazienti devono soddisfare tutti i seguenti criteri di inclusione per essere idonei a partecipare a questo studio (non saranno offerte o consentite deroghe per l'idoneità dei pazienti): 1) Pazienti di sesso maschile o femminile, senza distinzione di razza e/o gruppo etnico, di età pari o superiore a 18 anni, in grado di comprendere e fornire il consenso informato scritto. 2) Pazienti con TNBC localmente avanzato, non operabile o metastatico, che non abbiano ricevuto una precedente terapia sistemica per la malattia avanzata e i cui tumori siano positivi a PD-L1 al momento dello screening. 3) Hanno fornito un campione tumorale rappresentativo fissato in formalina e incluso in paraffina (FFPE) in blocchi (preferibilmente) o hanno almeno 20-25 vetrini appena sezionati non colorati di tessuto bioptico fresco (preferibile) o un blocco di tessuto d'archivio per l'analisi centrale di ER, recettore del progesterone, HER2, PD-L1, Trop-2 e per ulteriori analisi di biomarcatori. Se il tessuto d'archivio non è disponibile, è necessaria una biopsia basale; questa procedura deve essere eseguita anticipatamente rispetto alla prima dose di trattamento dello studio e dopo che il/la paziente abbia fornito il consenso informato scritto. Gli aspirati con ago sottile e le biopsie ossee non sono campioni adatti. Nota: la qualità del tessuto tumorale deve essere confermata dal laboratorio centrale. Se il campione fornito non è adeguato per la valutazione, può essere richiesta la presentazione di un altro campione tumorale. 4) Punteggio ECOG di performance status pari a 0 o 1 (Appendice 5 del Protocollo di studio). 5) Aspettativa di vita = 3 mesi. 6) Devono essere trascorse almeno due settimane di convalescenza da un intervento chirurgico maggiore. 7) Adeguata conta ematologica senza supporto trasfusionale o di fattori di crescita entro 2 settimane dall'inizio del trattamento di studio (emoglobina = 9 g/dL, ANC = 1500/mm3 e piastrine = 100.000/µL). 8) Funzionalità epatica adeguata (bilirubina = 1,5 × ULN, AST e ALT = 2,5 × ULN o = 5 × ULN in caso di metastasi epatiche note e albumina sierica > 3 g/dL). 9) Clearance della creatinina = 30 mL/min secondo l'equazione di Cockcroft-Gault {Cockcroft 1976}. 10) Rapporto internazionale normalizzato (INR)/PT e PTT o aPTT = 1,5 ULN a meno che il paziente non sia attualmente in terapia anticoagulante. 11) I pazienti maschi e le pazienti femmine con potenziale fertile che hanno rapporti eterosessuali devono accettare di utilizzare i metodi contraccettivi previsti dal protocollo, come descritto nell'Appendice 3 del protocollo. 12) I pazienti con HIV devono essere in terapia antiretrovirale (ART) e avere un'infezione/malattia da HIV ben controllata. |

E.4 | Principal exclusion criteria |
Patients who meet any of the following exclusion criteria are not eligible to be enrolled in this study (no waivers for patient eligibility will be offered or permitted): 1) Positive serum pregnancy test (Appendix 3) or women who are lactating. 2) Known or severe (= Grade 3) hypersensitivity or allergy to SG, pembrolizumab, and/or the chemotherapy regimen of choice in the TPC arm (eg, nab paclitaxel, paclitaxel, gemcitabine, or carboplatin), their metabolites, or formulation excipient. 3) Have received prior therapy with an agent directed to another stimulatory or coinhibitory T-cell receptor (eg, CTLA-4, OX-40, CD137) 4) Requirement for ongoing therapy with or prior use of any prohibited medications listed in Section 5.6.1. 5) Patients may not have received systemic anticancer treatment within the previous 6 months or radiation therapy within 2 weeks prior to enrollment. Patients must have recovered from AEs due to a previously administered agent to = Grade 1 or baseline at the time of study entry. 6) Patients may not be participating in a study with an investigational agent or investigational device within 4 weeks prior to randomization. Patients participating in observational studies are eligible. 7) Have previously received topoisomerase 1 inhibitors or antibody drug conjugates containing a topoisomerase inhibitor. 8) Have an active second malignancy. 9) Have known active central nervous system (CNS) metastases and/or carcinomatous meningitis. Patients with previously treated brain metastases may participate (with the exception of those treated with chemotherapy) provided they have stable CNS disease (defined as radiographic stability demonstrated with a minimum of 2 posttreatment brain imaging assessments; one performed during screening) for at least 4 weeks prior to enrollment and all neurologic symptoms have returned to baseline, have no evidence of new or enlarging brain metastases, and have also been clinically stable for at least 2 weeks while taking = 10 mg/day of prednisone or its equivalent. All patients with carcinomatous meningitis are excluded regardless of clinical stability. 10) Have undergone an allogenic tissue or solid organ transplant. 11) Met any of the following criteria for cardiac disease: a) Myocardial infarction or unstable angina pectoris within 6 months of enrollment. b) History of serious ventricular arrhythmia (ie, ventricular tachycardia or ventricular fibrillation), high-grade atrioventricular block, or other cardiac arrhythmias requiring antiarrhythmic medications (except for atrial fibrillation that is well controlled with antiarrhythmic medication); history of QT interval prolongation. c) New York Heart Association Class III or greater congestive heart failure or known left ventricular ejection fraction of < 40%. 12) Have active chronic inflammatory bowel disease (ulcerative colitis, Crohn's disease) or GI perforation within 6 months of enrollment. 13) Have active serious infection requiring systemic antimicrobial therapy. 14) Patients positive for HIV-1 or 2 with a history of Kaposi sarcoma and/or Multicentric Castleman Disease. 15) Have active HBV (defined as having a positive HBsAg test) or HCV. 16) Have other concurrent medical or psychiatric conditions that, in the investigator's opinion, may be likely to confound study interpretation or prevent completion of study procedures and follow-up examinations.

CTLA-4, OX-40, CD137). 4) Necessità di una terapia in corso con uno dei farmaci vietati elencati nella sezione 5.6.1 del Protocollo o uso precedente di tali farmaci. 5) I pazienti non devono aver ricevuto un trattamento antitumorale sistemico nei 6 mesi precedenti o una radioterapia nelle 2 settimane precedenti l'arruolamento. I pazienti devono essere guariti dagli effetti indesiderati dovuti a un agente precedentemente somministrato ed essere tornati a un grado = 1 o al livello base al momento dell'ingresso nello studio. 6) I pazienti non possono partecipare a uno studio con un farmaco sperimentale o un dispositivo sperimentale nelle 4 settimane precedenti la randomizzazione. I pazienti che partecipano a studi osservazionali sono eleggibili. 7) Hanno ricevuto in precedenza inibitori della topoisomerasi 1 o coniugati anticorpo-farmaco contenenti un inibitore della topoisomerasi. 8) Hanno una seconda neoplasia attiva. 9) Hanno metastasi attive note al sistema nervoso centrale (SNC) e/o meningite carcinomatosa. I pazienti con metastasi cerebrali precedentemente trattate possono partecipare (con l'eccezione di quelli trattati con chemioterapia) a condizione che abbiano una malattia del SNC stabile (definita come stabilità radiografica dimostrata con un minimo di 2 valutazioni di imaging cerebrale post-trattamento, una delle quali eseguita durante lo screening) da almeno 4 settimane prima dell'arruolamento e che tutti i sintomi neurologici siano tornati al livello basale, non abbiano evidenza di metastasi cerebrali nuove o in espansione e siano stati clinicamente stabili per almeno 2 settimane durante l'assunzione di = 10 mg/die di prednisone o di un suo equivalente. Tutti i pazienti con meningite carcinomatosa sono esclusi, indipendentemente dalla stabilità clinica. 10) Sono stati sottoposti a un trapianto allogenico di tessuto o di organo solido. 11) Soddisfano uno dei seguenti criteri per patologie cardiache: a) Infarto miocardico o angina pectoris instabile entro 6 mesi dall'arruolamento. b) Anamnesi di aritmia ventricolare grave (cioè tachicardia ventricolare o fibrillazione ventricolare), blocco atrioventricolare di grado elevato o altre aritmie cardiache che richiedono farmaci antiaritmici (ad eccezione della fibrillazione atriale che è ben controllata con farmaci antiaritmici); storia di prolungamento dell'intervallo QT. c) Insufficienza cardiaca congestizia di classe III o superiore della New York Heart Association o frazione di eiezione ventricolare sinistra nota inferiore al 40%. 12) Malattia infiammatoria cronica intestinale attiva (colite ulcerosa, morbo di Crohn) o perforazione gastrointestinale entro 6 mesi dall'arruolamento. 13) Infezione grave attiva che richiede una terapia antimicrobica sistemica. 14) Pazienti positivi all'HIV-1 o 2 con una storia di sarcoma di Kaposi e/o malattia di Castleman multicentrica. 15) Hanno HBV attivo (definito come test HBsAg positivo) o HCV. 16) Presenta altre condizioni mediche o psichiatriche concomitanti che, a giudizio dello sperimentatore, potrebbero confondere l'interpretazione dello studio o impedire il completamento delle procedure di studio e degli esami di follow-up.
