    El objetivo del estudio actual es investigar la eficacia, seguridad y tolerabilidad de BI10773 (25 mg una vez al día) en comparación con glimepirida (1-4 mg al día) administrado durante 104 semanas en pacientes con diabetes mellitus de tipo II y un control insuficiente de la glucemia pese a la metformina.

    The objective of the current study is to investigate the efficacy, safety and tolerability of BI10773 (25mg once daily) compared to glimepriride (1-4mg daily) given for 104 weeks in patients with type 2 diabetes mellitus and insufficient glycaemic control despite metformin.

    E.3Principal inclusion criteria
    1. Diagnóstico de diabetes mellitus de tipo 2 antes del consentimiento informado.
    2. Hombres y mujeres con un plan de dieta y ejercicio y tratados previamente con una dosis máxima tolerada de metformina (preferiblemente >/= 1500 mg) y no modificada durante 12 semanas antes de la aleatorización.
    3. HbA1c de >/= 7,0% y &#8804; 10% en la visita 1 (selección).
    4. Edad >/= 18 años
    5. IMC (índice de masa corporal) >/= 45 kg/m2 en la visita 1 (selección)
    6. Haber firmado el consentimiento informado por escrito cuando se realiza la visita 1, de conformidad con la BPC y la legislación local.

    1. Diagnosis of type 2 diabetes mellitus prior to informed consent
    2. Male and female patients on diet and exercise regimen who are pre-treated with a stable maximum tolerated dose of metformin unchanged for 12 weeks prior to randomisation. (Preferably >/= 1500mg/day metformin)
    3. HbA1c of more than or equal to 7.0% and less or equal to 10.0% at Visit 1 (screening)
    4. Age more than or equal to 18yrs
    5. BMI more or than or equal to 45 kg/m2 (Body Mass Index) at Visit 1 (Screening)
    6. Signed and dated written informed consent by date of Visit 1 in accordance with GCP and local legislation
    E.4Principal exclusion criteria
    1. Hiperglucemia no controlada, con niveles de glucosa >240 mg/dl (>13,3 mmol/l) tras una noche en ayunas durante la pre-inclusión con placebo y confirmada por una segunda determinación (no el mismo día).
    2. Cualquier otro medicamento antidiabético dentro de las 12 semanas previas a la aleatorización, excepto metformina
    3. Infarto de miocardio, ictus o AIT en los 3 meses anteriores a la obtención del consentimiento informado.
    4. Indicación de enfermedad hepática, definida por niveles séricos de ALT (SGPT), AST (SGOT) o fosfatasa alcalina superiores a 3 veces el límite superior de la normalidad (LSN) según se determine durante la fase de selección o pre-inclusión
    5. Alteración de la función renal, definida como FGe < 60 ml/min (insuficiencia renal moderada o grave utilizando fórmula de MDRD).
    6. Cirugía bariátrica en los dos últimos años y otras cirugías gastrointestinales que provoquen una malabsorción crónica.
    7. Antecedentes médicos de cáncer (excepto el carcinoma basocelular) o tratamiento para el cáncer en los 5 últimos años.
    8. Discrasias sanguíneas o cualquier otro trastorno que provoque hemólisis o inestabilidad de los eritrocitos (p. ej., malaria, babesiosis, anemia hemolítica).
    9. Contraindicaciones, incluida la hipersensibilidad comprobada a la metformina o sulfonilureas según el prospecto local.
    10. Tratamiento con fármacos contra la obesidad (p. ej., sibutramina, orlistat) 3 meses antes del consentimiento informado, o cualquier otro tratamiento en el momento de la selección (por ejemplo, cirugía, régimen alimentario drástico, etc.) que cause inestabilidad del peso corporal.
    11. Tratamiento actual con esteroides sistémicos en el momento del consentimiento informado, o cambio en la posología de las hormonas tiroideas en las 6 semanas previas al consentimiento informado o cualquier otro trastorno endocrino no controlado, excepto DMT2.     12. Mujeres pre-menopáusicas (última menstruación </= 1 año antes del consentimiento informado) que:
    - estén en período de lactancia o embarazadas, o
    - sean de edad fértil y no utilicen un método anticonceptivo aceptable o no planeen continuar utilizando este método durante todo el estudio y no accedan a someterse a una prueba de embarazo periódica durante su participación en el estudio. Los métodos anticonceptivos aceptables incluyen la ligadura de trompas, parches transdérmicos, dispositivos/sistemas intrauterinos (DIU/SIU), anticonceptivos orales, implantables o inyectables, abstinencia sexual, método de doble barrera y pareja vasectomizada.
    13. Abuso de alcohol o fármacos en los 3 meses previos al consentimiento informado que interfiera en la participación en el estudio, o cualquier otro trastorno que dificulte el cumplimiento de los procedimientos del estudio o la toma del fármaco del estudio.
    14. Participación en otro estudio con un fármaco experimental en los 30 días anteriores a la obtención del consentimiento informado.
    15. Cualquier otra patología que pusiera en peligro la seguridad de los pacientes durante su participación en este estudio clínico.

    1. Uncontrolled hyperglycaemia with a glucose level >240 mg/dl (>13.3 mmol/L) after an overnight fast during placebo run-in and confirmed by a second measurement (not on the same day).
    2. Any other antidiabetic drug within 12 weeks prior to randomisation except metformin
    3. Myocardial infarction, stroke or TIA within 3 months prior to informed consent
    4. Indication of liver disease, defined by serum levels of either ALT (SGPT), AST (SGOT), or alkaline phosphatase above 3 x upper limit of normal (ULN) as determined at Visit 1
    5. Impaired renal function, defined as eGFR<60 ml/min (moderate to severe impairment calculated using MDRD formula)
    6. Bariatric surgery within the past two years and other gastrointestinal surgeries that induce chronic malabsorption
    7. Medical history of cancer (except for basal cell carcinoma) and/or treatment for cancer within the last 5 years
    8. Blood dyscrasias or any disorders causing haemolysis or unstable Red Blood Cell (e.g. malaria, babesiosis, haemolytic anaemia)
    9. Contraindications including hypersensitivity known to metformin or sulfonylureas according to the local label
    10. Treatment with glitazones, insulin or GLP- analogues or agonists 3 months prior to randomisation
