E.3 | Principal inclusion criteria |
1. Histologically or cytologically confirmed diagnosis of NSCLC.
2. Documented evidence of advanced NSCLC not amenable to surgery or radiotherapy.
3. Confirmation of the presence or absence of epidermal growth factor receptor (EGFR) mutations prior to study enrolment in all subjects. Subjects with unknown EGFR mutational status and who have received prior treatment with an EGFR TKI can be enrolled but an archived tumor sample, or if unavailable, a tumor sample obtained prior to randomization, where feasible, must be made available to perform EGFR mutational analysis.
4. Subjects must have received at least two prior regimens for advanced NSCLC, which should have included a platinum-based regimen and, in all subjects with tumors harbouring EGFR mutations, an EGFR TKI.
5. Radiographic evidence of disease progression on, or after, the last anti cancer regimen prior to study entry.
6. Presence of measurable disease meeting the following criteria:
a) At least one lesion of at least 1.0 cm in the long-axis diameter for a non-lymph node or at least 1.5 cm in the short-axis diameter for a lymph node which is serially measurable according to RECIST 1.1 using either computerized tomography (CT) or magnetic resonance imaging (MRI). If there is only one target lesion and it is a non-lymph node, it should have a longest diameter of at least 1.5 cm.
b) Lesions previously treated with radiotherapy must show radiographic evidence of disease progression to be deemed a target lesion.
7. Eastern Cooperative Oncology Group performance status of 0, 1, or 2.
8. Adequate renal function, defined as calculated creatinine clearance greater than or equal to 50 mL/min using the Cockcroft and Gault formula.
9. Adequate bone marrow function, defined as:
a) Absolute neutrophil count (ANC) of at least 1500/mm3 or at least 1.5 x 109/L.
b) Platelet count at least 100,000/mm3 or at least 100 x 109/L.
c) Hemoglobin (Hb) at least 10 g/dL at Baseline (blood transfusions, hematopoietic growth factors and hematinics are allowed during the Pre-randomization Phase to correct Hb values less than 10 g/dL).
10. Adequate liver function, defined as:
a) Bilirubin less than or equal to 1.5 times the upper limit of normal (ULN), except for unconjugated hyperbilirubinemia of Gilbert’s syndrome.
b) Alkaline phosphatase (ALP), alanine aminotransferase, and aspartate aminotransferase less than or equal to 3 times the ULN. For total ALP values greater than 3 times the ULN, the ALP liver isoenzyme must be less than or equal to 3 times the ULN.
11. Female subjects of child-bearing potential must agree to use two forms of highly effective contraception from the last menstrual period prior to randomization, during the Treatment Cycles, and for 3 months after the final dose of study treatment.Female subjects exempt from this requirement are subjects who practice total abstinence. 12. Male subjects and their female partners who are of child bearing potential as defined in Inclusion 11, and are who not practicing total abstinence must agree to use two forms of highly effective contraception from the last menstrual period of their female partner prior to randomization, during the Treatment Cycles, and for 3 months after the final dose of study treatment. If currently abstinent, must agree to use a double barrier method of contraception if they become sexually active during Treatment Cycles, and for 3 months after the final dose of study treatment or use two forms of highly effective contraception 13. Voluntary agreement to provide written informed consent and the willingness and ability to comply with all aspects of the protocol.
14. Males or females aged at least 18 years (or any age greater than 18 years as determined by country legislation) at the time of informed consent. |
1. Diagnosi di NSCLC confermata istologicamente o citologicamente.

3. Conferma di presenza o assenza di mutazioni del recettore del fattore di crescita epidermico (EGFR, Epidermal growth factor receptor) prima dell'arruolamento nello studio per tutti i pazienti. I pazienti per i quali lo stato di mutazione dell'EGFR non sia noto e che siano stati trattati in precedenza con un EGFR-TKI possono essere arruolati, ma deve essere disponibile un campione di tumore archiviato o, se non disponibile, ne deve essere prelevato uno prima della randomizzazione, se possibile, per eseguire l'analisi dello stato di mutazione dell'EGFR.
4. I pazienti dovranno essere stati sottoposti ad almeno due regimi precedenti per l'NSCLC in stadio avanzato, tra cui obbligatoriamente un regime contenente platino e, in tutti i pazienti i cui tumori presentino mutazioni dell'ECFR, un EGFR-TKI.
5. Evidenza radiografica di progressione di malattia con o dopo l'ultimo regime antitumorale precedente all'arruolamento nello studio.
6. Presenza di malattia misurabile che soddisfi i seguenti criteri:
a) Almeno una lesione di almeno 1,0 cm di diametro nell'asse lungo per le sedi non linfonodali o di almeno 1,5 cm di diametro nell'asse corto per i linfonodi, misurabile in serie secondo i criteri RECIST 1.1 utilizzando la tomografia computerizzata (TAC) o la risonanza magnetica (RM). Se è presente una sola lesione target, non in sede linfonodale, il suo diametro maggiore deve essere di almeno 1,5 cm.
b) Per essere considerate lesioni target, le lesioni trattate in precedenza con radioterapia devono mostrare evidenza radiografica di progressione di malattia.
7. Performance status secondo l’Eastern Cooperative Oncology Group pari a 0, 1 o 2.
8. Funzionalità renale adeguata, definita come clearance calcolata della creatinina maggiore o uguale a 50 ml/min secondo la formula di Cockcroft e Gault.
9. Funzionalità adeguata del midollo osseo, definita come:
a) Conta assoluta dei neutrofili (ANC, Absolute neutrophil count) di almeno 1500/mm3 o di almeno 1,5 x 109/l.
b) Conta delle piastrine di almeno 100.000/mm3 o di almeno 100 x 109/l.
c) Emoglobina (Hb) di almeno 10 g/dl al basale (sono ammessi trasfusioni di sangue, fattori di crescita ematopoietica ed ematinici durante la fase di pre-randomizzazione per correggere i valori di Hb inferiori a 10 g/dl).
10. Funzionalità epatica adeguata, definita come:
a) Bilirubina inferiore o uguale a 1,5 volte il limite superiore della norma (ULN, Upper limit of normal), fatta eccezione per la iperbilirubinemia non coniugata della sindrome di Gilbert.
b) Fosfatasi alcalina (ALP, Alkaline phosphatase), alanina aminotransferasi e aspartato aminotransferasi inferiori o pari a 3 volte l'ULN. Per valori totali di ALP maggiori di 3 volte l'ULN, l'isoenzima epatico ALP deve essere inferiore o pari a 3 volte l'ULN.
11.Tutte le donne fertili devono accettare di utilizzare due metodi di contraccezione altamente efficaci a partire dall'ultimo ciclo mestruale prima della randomizzazione, durante i cicli di trattamento e per i 3 mesi successivi alla dose finale di trattamento dello studio.Sono esenti da questo requisito le donne che praticano l'astinenza totale.
12. I pazienti di sesso maschile e le loro compagne fertili, che non praticano l'astinenza totale devono accettare di utilizzare due metodi di contraccezione altamente efficaci a partire dall'ultimo ciclo mestruale della compagna prima della randomizzazione, durante i cicli di trattamento e per i 3 mesi successivi alla dose finale di trattamento dello studio.
13. Accettazione volontaria e firma del consenso informato scritto, nonché disponibilità e capacità di conformarsi a tutti i requisiti previsti dal protocollo. |

E.4 | Principal exclusion criteria |
1. Subjects who have received any anti-cancer therapy, including surgery or tumor embolization, chemotherapy, targeted, biological (including humanized antibodies), investigational, immunotherapy, hormonal, or radiotherapy, within 14 days, or five half-lives of the drug (whichever is longer), prior to randomization.2. Subjects who have not recovered from toxicities as a result of prior anti cancer therapy to less than Grade 2, according to Common Terminology Criteria for Adverse Events (CTCAE), except for peripheral neuropathy (see Exclusion 4) and alopecia.
3. Subjects who have previously been treated, or participated in a study with eribulin, whether treated with eribulin or not. The TPC option must not include the same agent which the subject received in a prior regimen.
4. Peripheral neuropathy more than CTCAE Grade 2.
5. Significant cardiovascular impairment, defined as:
a) Congestive heart failure greater than Class II according to the New York Heart Association Cardiac Disease Classification,
b) Unstable angina or myocardial infarction within 6 months of enrolment, or
c) Serious cardiac arrhythmia or cardiac arrhythmia requiring treatment.
6. Subjects with a high probability of Long QT Syndrome, or QT corrected (QTc) interval >500 ms on at least two separate ECGs.
7. Subjects with brain or subdural metastases are not eligible, unless the metastases are asymptomatic and do not require treatment or have been adequately treated by local therapy to the affected area of the brain (e.g. stereotactic radiosurgery), and have discontinued the use of corticosteroids for this indication for at least 4 weeks prior to study entry and have confirmed radiographic stability. Confirmation of radiographic stability will be done by comparing the brain scan (CT or MRI) performed during the Screening Period to a brain scan performed at least 4 weeks earlier (and following local therapy where applicable) using the same imaging modality as during the Screening Period. It is not the intention of this protocol to treat subjects with active brain metastasis.
8. Any serious concomitant illness.
9. Known HIV positive, or have an infection requiring treatment.
10. Any malignancy that required treatment, or has shown evidence of recurrence (except for NSCLC, non-melanoma skin cancer, or histologically confirmed complete excision of carcinoma in-situ during the 5 years prior to study entry.
11. Female subjects must not be pregnant, as documented by a negative beta-human chorionic gonadotropin (β-hCG) test with a minimum sensitivity 25 IU/L or equivalent unit of β hCG, and must not be breastfeeding.
12. Hypersensitivity to either halichondrin B or halichondrin B chemical derivatives or both, or to any of the excipients of the eribulin formulation. Selection of the TPC option prior to randomization will be made exclusive of an option to which the subject has known allergies or contra indications (please refer to the Prescribing Information of the TPC option).
13. Any medical or other condition which, in the opinion of the investigator, will preclude participation in a clinical trial. |
1. Pazienti sottoposti a qualsiasi terapia antitumorale, compresi intervento chirurgico o embolizzazione del tumore, chemioterapia, terapia mirata, terapia biologica (compresi anticorpi umanizzati), terapia sperimentale, immunoterapia, terapia ormonale o radioterapia nei 14 giorni, o nelle cinque emivite del farmaco (a seconda di quale periodo sia più lungo), precedenti alla randomizzazione.2.Pazienti che non si siano ripresi da tossicità derivate dalla terapia antitumorale precedente fino a un grado inferiore a 2, secondo i Criteri comuni di terminologia per gli eventi avversi (CTCAE,Common Terminology Criteria for Adverse Events), fatta eccezione per la neuropatia periferica (vedere il criterio di esclusione n. 4) e l'alopecia.
3. Pazienti che siano stati trattati in precedenza con eribulin o abbiano partecipato a uno studio con eribulin, sia che siano stati trattati con eribulin o meno. Il trattamento di scelta del medico non deve includere lo stesso principio attivo somministrato al paziente in un regime precedente.
4. Neuropatia periferica di grado superiore a 2 secondo CTCAE.
5. Compromissione cardiovascolare significativa, definita come:
a) Insufficienza cardiaca congestizia di classe superiore a II secondo la classificazione della malattia cardiaca della New York Heart Association,
b) Angina instabile o infarto miocardico nei 6 mesi precedenti all'arruolamento o
c) Aritmia cardiaca seria o che necessiti di trattamento.
6. Pazienti con elevata probabilità di sindrome del QT lungo o intervallo QT corretto (QTc) >500 ms in almeno due elettrocardiogrammi separati.
7. I pazienti con metastasi cerebrali o subdurali non sono idonei, a meno che le metastasi siano asintomatiche e non necessitino di trattamento o siano state adeguatamente trattate con terapia localizzata nell'area colpita del cervello (ad es. radiochirurgica stereotattica), e i pazienti abbiano interrotto l'assunzione di corticosteroidi per questa indicazione almeno 4 settimane prima dell'arruolamento dello studio e presentino stabilità radiografica confermata. La stabilità radiografica sarà confermata confrontando la scansione cerebrale (TAC o RM) eseguita durante il periodo di screening con una scansione cerebrale eseguita almeno 4 settimane prima (e dopo la terapia localizzata, dove applicabile) usando la stessa modalità di imaging impiegata nel periodo di screening. Il protocollo non prevede il trattamento di pazienti con metastasi cerebrali attive.
8. Qualsiasi malattia concomitante seria.
9. Positività accertata all'HIV o infezione che necessiti di trattamento.
10. Qualsiasi tumore maligno che sia stato clinicamente trattato o abbia mostrato evidenza di recidiva, fatta eccezione per NSCLC, cancro della cute non melanomatoso o escissione completa, confermata istologicamente, di carcinoma in situ nei 5 anni precedenti all'arruolamento nello studio.
11. Le pazienti non devono essere incinte, come documentato da un test negativo della gonadotropina corionica umana beta (β-hCG), con una sensibilità minima di 25 UI/l o unità equivalenti di β hCG, e non devono allattare al seno.
12. Ipersensibilità all'alicondrina B o ai derivati chimici dell'alicondrina B o a entrambi oppure a uno qualsiasi degli eccipienti della formulazione di eribulin. Il trattamento di scelta del medico, deciso prima della randomizzazione, escluderà i trattamenti per i quali il paziente presenti allergie accertate o controindicazioni (fare riferimento alle informazioni per la prescrizione del trattamento di scelta del medico).
13. Qualsiasi condizione medica o di altro tipo che, secondo il parere dello sperimentatore, precluda la partecipazione a una sperimentazione clinica. |
