Phase IIb part: Primary Objective: to prospectively validate the level of CD16+CD56+CD69+ lymphocytes, a phenotype of activated Natural Killer (aNK) cells as a predictive biomarker of TG4010’s activity by comparing Progression-Free Survival (PFS) between the TG4010 arm (TG4010 + first-line therapy) and the placebo arm (placebo + first-line therapy) in the two subgroups of patients according to their level of aNK cells before randomization (normal and high level of aNK cells). Phase III part: Primary Objective: to compare OS between the TG4010 arm and the placebo arm in the identified patient population based on the results of the phase IIb part of the study. |

Inclusion criteria: Patients must satisfy all of the following criteria for entry into the protocol: - Male or female patients, age > 18 years old - Histologically confirmed NSCLC (adenocarcinoma, squamous cell carcinoma, large cell carcinoma, undifferentiated carcinoma or other) - Stage IV cancer according to TNM classification (7th edition – UICC, December 2009; includes tumor with malignant pleural or pericardial effusion - Tumor biopsy specimen with ≥ 50% of MUC1 expressing tumor cells determined by Immunohistochemistry (IHC) staining on fixed pathological material. Biopsy may come either from the primary tumor or from a metastasis. Cytological material is not accepted for this analysis - Patient's naïve to first-line therapy for the advanced stage of the disease. Previous neoadjuvant or adjuvant therapy is allowed for patients who successfully underwent complete radical surgery and if last treatment was administered more than 12 months prior to the start of the study treatment, i.e., D1 of Cycle 1. - At least one measurable lesion by CT scan or MRI based on RECIST version 1.1 (CT is preferred for the chest exam) - PS 0 or 1 on the ECOG scale (see APPENDIX 1) - Adequate hematological, hepatic, and renal function: • Hemoglobin >or= 10.0 g/dL • White Blood Cells (WBC) >or= 3.0x109/L including - Neutrophils >or= 1.5x109/L - Total lymphocytes count >or= 0.5x109/L • Platelets count >or= 100x109/L • Serum alkaline phosphatase <or= 5 x ULN (except in patients with documented bone or liver metastases) • Serum transaminases (alanine aminotransferase [ALT] and aspartate aminotransferase [AST]) <or= 2.5 x ULN in the absence of liver metastases and <or= 5 x ULN in case of liver metastases) • Glomerular Filtration Rate ≥60 mL/min (according to Modification of the Diet in Renal Disease (MDRD) formula or Cockroft & Gault formula) • Serum albumin ≥ 30 g/L - Effective contraception during the study period and for 3 months after the last study treatment administration (male and female patient) - Written informed consent - For Phase III part: additional inclusion criterion for the level of aNK cells may be added according to the results observed in Phase IIb part. |

La biopsia può essere eseguita sia sul tumore primario che su metastasi. Il materiale citologico non è accettato per questa analisi - Pazienti che non sono stati precedentemente sottoposti a terapia di prima linea per lo stadio avanzato della malattia. La precedente terapia neoadiuvante o adiuvante è consentita per i pazienti che hanno superato con successo un intervento chirurgico radicale completo e se l'ultimo trattamento è stato somministrato oltre 12 mesi prima dell'inizio del trattamento dello studio, cioè G1 del Ciclo 1. - Almeno una lesione misurabile tramite TAC o RM in base a RECIST versione 1.1 (per l'esame del torace è preferibile la TAC) - PS 0 o 1 sulla scala ECOG (vedere Errore. Exclusion criteria: Patients will be excluded from the study for any of the following reasons: - Patients having Central Nervous System (CNS) metastases. Patients who have had brain metastases surgically removed or irradiated with no residual disease confirmed by imaging are allowed - Documented EGFR activating mutations (if already tested) - Prior history of other malignancy except: • Basal cell carcinoma of the skin • Cervical intra epithelial neoplasia • Other cancer curatively treated with no evidence of disease for at least 5 years - Patients under chronic treatment with systemic corticoids or other immunosuppressive drugs (e.g., cyclosporine) for a period of at least 4 weeks and whose treatment was not stopped 1 week prior to the start of the study treatment (i.e., D1 of Cycle 1) - Positive serology for Human Immunodeficiency Virus (HIV) or Hepatitis C Virus (HCV); presence in the serum of the antigens HBs - Patient with any underlying medical condition that the treating physician considers might be aggravated by treatment or which is not controlled (e.g., elevated troponin or creatinine, uncontrolled diabetes) - Patient with major surgery or radiotherapy within 4 weeks prior to the start of the study treatment (i.e., D1 of Cycle 1). Prior surgery or radiation therapy aimed at local palliation or attempted local disease control is permitted - Pregnant or nursing (lactating) women, where pregnancy is defined as the state of a female after conception and until the termination of gestation, confirmed by a positive hCG laboratory test (> 5 mIU/mL) - Women of child-bearing potential, defined as all women physiologically capable of becoming pregnant, UNLESS they are: • women whose sexual orientation precludes intercourse with a male partner • women whose partners have been sterilized by vasectomy or other means • using a highly effective method of birth control (i.e. one that results in a less than 1% per year failure rate when used consistently and correctly, such as implants, injectables, combined oral contraceptives, and some intrauterine devices [IUDs]; periodic abstinence (e.g. calendar, ovulation, symptothermal, post-ovulation methods) is not acceptable) - Patient with an organ allograft - Known allergy to eggs or gentamicin - Participation in a clinical study with an investigational product within 4 weeks prior to the start of the study treatment (i.e., D1 of Cycle 1) - Patient unable or unwilling to comply with the protocol requirements - Bevacizumab will be allowed for patients with non-squamous carcinoma. Prescribing information must be followed and precautions have to be taken into consideration (e.g., patients having presented a serious hemorrhage or recent hemoptysis should not receive bevacizumab). |
Criteri di esclusione: I pazienti saranno esclusi dallo studio per uno qualsiasi dei seguenti motivi: - Pazienti con metastasi nel sistema nervoso centrale (SNC). Sono ammessi i pazienti con metastasi cerebrali asportate chirurgicamente o irradiate senza malattia residua confermata per immagini - Mutazioni attivanti dell'EGFR documentate (se già testate) - Precedente anamnesi di altra neoplasia maligna ad eccezione di: • Carcinoma basocellulare della cute • Neoplasia intraepiteliale cervicale • Altro tipo di cancro adeguatamente trattato senza evidenza di malattia per almeno 5 anni - Pazienti soggetti a trattamento cronico con corticosteroidi sistemici o altri farmaci immunosoppressivi (ad es. ciclosporina) per un periodo di almeno 4 settimane e il cui trattamento non sia stato interrotto 1 settimana prima dell'inizio del trattamento dello studio (cioè G1 del Ciclo 1) - Risultato sierologico positivo al virus dell'immunodeficienza umana (HIV) o dell'epatite C; presenza di antigeni HB nel siero - Pazienti affetti da qualunque condizione medica di base che il medico curante ritenga possa essere aggravata dal trattamento o che non sia controllata (ad es. troponina o creatinina elevata, diabete non controllato) - Pazienti sottoposti a intervento chirurgico importante o radioterapia nelle 4 settimane precedenti l'inizio del trattamento dello studio (cioè G1 del Ciclo 1). Sono ammessi pazienti sottoposti a precedente intervento chirurgico o radioterapia come palliativo locale o tentativo di controllo locale della malattia - Donne in gravidanza o che allattano al seno; per gravidanza si intende lo stato in cui si trova una donna dal concepimento fino al termine della gestazione, confermato da test di laboratorio hCG positivo (> 5 mIU/mL) - Donne in età fertile, cioè tutte le donne fisiologicamente in grado di iniziare una gravidanza, A MENO CHE non siano: • donne il cui orientamento sessuale precluda rapporti con partner di sesso maschile • donne i cui partner siano stati sterilizzati tramite vasectomia o altri mezzi • donne che utilizzano un metodo anticoncezionale altamente efficace (cioè un metodo che presenti un tasso di errore inferiore all'1% all'anno se utilizzato in modo continuo e corretto, come ad esempio contraccettivi impiantabili, iniettabili, orali combinati e alcuni dispositivi intrauterini [IUD]; l'astinenza periodica (ad es. calendario, ovulazione, sintotermico, post-ovulazione) non è accettabile) - Pazienti con allotrapianto di organo - Allergia nota a uova o gentamicina - Partecipazione ad uno studio clinico con un prodotto sperimentale nelle 4 settimane precedenti l'inizio del trattamento dello studio (cioè G1 del Ciclo 1) - Paziente non disponibile o incapace a conformarsi ai requisiti del protocollo - La somministrazione di Bevacizumab è consentita ai pazienti con carcinoma non squamocellulare È necessario seguire le indicazioni di prescrizione e prendere in considerazione le precauzioni (ad es. pazienti che hanno mostrato grave emorragia o recente emottisi non devono ricevere il bevacizumab). |
