E.3 | Principal inclusion criteria |
1. Histologically verified adenocarcinoma of the oesophagus or oesophago-gastric junction based on endoscopy (OGD). 2. CT-18FDG-PET in all patients for disease staging 3. EUS, if feasible. This should be performed in all patients unless luminal obstruction precludes sensitivity of the test. 4. Staging laparoscopy will be performed at the investigators discretion for locally advanced AEG II and AEG III tumours 5. Pre-treatment stage cT2-3, N0-3, M0 6. Maximum tumour length 8cm 7. No prior abdominal or thoracic radiotherapy 8. Male/female patients aged ≥18 years. 9. ECOG Performance Status 0, 1 or 2 10. ASA I-II 11. Adequate cardiac function. For all patients an ejection fraction of > 50% is required. If patients have a known cardiac history (e.g. known ischemic disease, cardiomyopathy) an ejection fraction > 50% and cardiac clearance by a consultant cardiologist for major surgery and cancer therapies is required 12. Adequate respiratory function. Patients should have pulmonary function tests completed with a minimum FEV1 ≥ 1.5L 13. Adequate bone marrow function: absolute neutrophil count (ANC) >1.5x109/l; white blood cell count >3x109/l; platelets >100x109/l; haemoglobin (Hb) >9g/dl (can be post-transfusion). 14. Adequate renal function: glomerular filtration rate >60ml/minute calculated using the Cockcroft-Gault Formula (Appendix O). 15. Adequate liver function: serum bilirubin <1.5x ULN; AST <2.5x ULN and ALP <3x ULN (ULN as per institutional standard) 16. A written informed consent must be obtained from the patient before any study-trial specific procedures are performed. 17. Women of child-bearing potential and male subjects must agree to use an effective barrier method of contraception for up to 6 months following discontinuation of therapy. Effective contraception is defined as any medically recommended (or combination of methods) as per standard of care. 18. Women of childbearing potential must have pregnancy excluded by urine or serum beta-HCG testing within 7 days prior to treatment. 19. Patients covered by a Health Insurance System. |
1. Adénocarcinome de œsophage ou de la jonction oesogastrique histologiquement confirmé sur la base de la fibroscopie. 2. Scanner et TEP-scan au 18FDG chez tous les patients pour l’évaluation du stade de la maladie. 3. Echo-endoscopie si faisable. Cet examen devra être réalisé pour tous les patients sauf en cas de lésion non franchissable en endoscopie. 4. Une laparoscopie exploratrice sera réalisée à la discrétion de l’investigateur pour les tumeurs localement avancées de la jonction oesogastrique de type II ou III. 5. Stade tumoral pré-thérapeutique cT2-3, N0-3, M0. 6. Taille maximale de la tumeur = 8cm. 7. Absence d’antécédent de radiothérapie abdominale ou thoracique. 8. Homme ou Femme ≥18 ans. 9. Indice de performance ECOG 0, 1 ou 2. 10. ASA I-II. 11. Fonction cardiaque appropriée. Une fraction d’éjection > 50% est requise pour tous les patients. Pour les patients avec des antécédents cardiaques connus (coronaropathie, cardiomyopathie), une fraction d’éjection > 50% et une évaluation de la fonction cardiaque pour intervention chirurgicale majeure et thérapies anti-cancéreuses par un cardiologue est nécessaire. 12. Fonction pulmonaire appropriée. Les patients doivent avoir aux Epreuves Fonctionnelles Respiratoires un VEMS > 1.5l (Volume Expiratoire Maximal en une Seconde). 13. Fonction hématopoïétique appropriée : taux de PNN (Polynucléaires neutrophiles) > 1.5x109/l; globules blancs > 3x109/l; plaquettes > 100x109/l; hémoglobine > 9g/dl (une transfusion peut être nécessaire avant). 14. Fonction rénale appropriée : débit de filtration glomérulaire > 60ml/minute, calculé selon la formule de Cockcroft-Gault (Annexe O). 15. Fonction hépatique appropriée : bilirubinémie < 1.5x la LSN (Limite Supérieure Normale); ASAT < 2.5x la LSN et ALAT < 3x la LSN. 16. Consentement éclairé et signé devant être obtenu avant la réalisation de toute procédure spécifique à l’étude. 17. Femmes en âge de procréer et hommes : accord d’utilisation d’une méthode de contraception efficace jusqu’à 6 mois après la fin du traitement. Une méthode de contraception efficace est définie par la méthode médicale (ou combinaison de méthodes) recommandée selon les normes de soins. 18. Femmes en âge de procréer : test de grossesse urinaire ou sanguin (beta-HCG) dans les 7 jours précédant le début du traitement. 19. Affiliation à un régime de sécurité sociale. |

1. Tumours of squamous histology. 2. Patients with advanced inoperable or metastatic oesophageal, junctional or gastric adenocarcinoma. 3. Any prior chemotherapy for gastrointestinal cancer. 4. Patients who are unfit for surgery or cancer treatments based on cardiac disease. 5. Patients with acute systemic infections. 6. Patients who are receiving treatment with sorivudine or its chemical related analogues, such as brivudine which is contraindicated with capecitabine and 5-Fluorouracil administration. 7. Clinical COPD with significant obstructive airways disease classified by FEV1 < 1.5 L or PaO2 less than 9kPa on room air 8. Known peripheral neuropathy >Grade 1 (absence of deep tendon reflexes as the sole neurological abnormality does not render the patient ineligible). 9. Known positive tests for human immunodeficiency virus (HIV) infection, acute or chronic active hepatitis B infection. 10. Any other malignancies within the last 5 years (other than curatively treated basal cell carcinoma of the skin and/or in situ carcinoma of the cervix) 11. Participation in other clinical trials of investigational or marketed agents for the treatment of oesophagial cancer or other diseases within 30 days from registration 12. Women who are pregnant or who are breastfeeding. 13. Patients in emergency situations. 14. Patients kept in detention.

1. Tumeurs d’histologie épidermoïde. 2. Patients avec adénocarcinome avancé inopérable ou métastatique de œsophage, de la jonction oesogastrique ou de l’estomac. 3. Antécédent de chimiothérapie pour un cancer gastro-intestinal. 4. Patients non opérable ou ne pouvant bénéficier de la chimio/radiothérapie du fait de l’état cardiaque. 5. Patients présentant des infections systémiques aiguës. 6. Patients recevant un traitement de Sorivudine ou analogues chimiques liés, tels que la Brivudine qui est contre-indiquée lors de l’administration de la Capecitabine et du 5-Fluorouracile. 7. BPCO avec un syndrome obstructif significatif se traduisant par un VEMS < 1.5 L ou une PaO2 < 9kPa en air ambiant. 8. Neuropathie périphérique connue de grade > 1 (l’absence de réflexes tendineux comme seule anomalie n’est pas un critère d’exclusion). 9. Tests biologiques connus comme étant positifs pour HIV, hépatite B aigüe ou chronique. 10. Tout autre cancer de moins de 5 ans (à l’exclusion d’un carcinome basocellulaire de la peau traité et/ou d’un cancer in situ du col utérin). 11. Participation à d’autres études cliniques impliquant des traitements (expérimentaux ou déjà commercialisés) du cancer de œsophage ou d’autres maladies jusqu’à 30 jours après l’inclusion. 12.Femmes enceintes ou allaitantes. 13. The study is expected to take approximately 6-8 years with an accrual time of 60 months, with up to 3 years follow up. It is anticipated that up to 594 patients will be accrued to this study.
