    E.1.1Medical condition(s) being investigated
    The study will include healthy children who have already been vaccinated with 2 doses of Prevenar 13 according to the UK routine immunisation schedule at 2 and 4 months of age. Both vaccines used in the study (Prevenar 13 and Synflorix) are licensed for protection against pneumococcal disease. We do not intend to extend the indication of these vaccines further but intend to confirm non-inferiority of a combinatory vaccine schedule compared to the standard schedule with respect to immunogenicity.

    E.2 Objective of the trial
    E.2.1Main objective of the trial
    The primary objective of this study is to assess whether the study vaccine (PHiD-CV) is as good as the standard vaccine (PCV-13) in terms of percentage of participants who have antibody concentrations above the protective threshold (≥0.35mcg/ml) for the 10 serotypes included in PHiD-CV (serotypes 1, 4, 5, 6B, 7F, 9V, 14, 18C, 19F, 23F) one month following booster vaccination at 12 months of age with PHiD-CV or PCV-13.
    E.2.2Secondary objectives of the trial
    To assess the proportion of participants with serotype-specific IgG concentrations ≥0.35mcg/ml to the serotypes 3, 6A, 6C and 19A at 12 months of age one month following a booster with either PHiD-CV or PCV-13. To assess serotype-specific IgG antibody concentrations to 14 serotypes at baseline (before the booster is given) as well as 1 and 12 months following a booster with either PCV-10 or PCV-13. To assess serotype-specific opsonophagocytic activity (OPA) titres (OPA = an assay that determines how well participants' serum is able to kill live bacteria) and the proportion of participants with PCV-10 serotype-specific OPA titres ≥1:8 at baseline as well as 1 and 12 months following a booster dose of either PCV-10 or PCV-13. To assess the proportion of participants at baseline, who have IgG concentrations ≥0.35mcg/ml for 14 serotypes.     E.3Principal inclusion criteria
    • Aged 12 months (-2 weeks to +6 weeks) at time of enrolment. • Have received two doses of PCV-13 at less than 6 months of age with a gap of at least 6 weeks between the two vaccinations. • Have received all primary vaccines according to the UK routine immunisation schedule. • Available for entire study period and whose parent/legal guardian can be reached by telephone. • Healthy children as determined by medical history and physical examination, done by a study nurse (and/or study doctor if required, depending on the medical history of the participant and physical assessment), and judgment of the investigator. • Parent/legal guardian must be able to complete all relevant study procedures during study participation.
    E.4Principal exclusion criteria
    • Previous receipt of pneumococcal vaccine other than the 13-valent pneumococcal conjugate vaccine (Prevenar 13®, Pfizer). • Receipt of the routine 12-month immunisations (PCV13 (3rd dose), combined Haemophilus influenzae type b and serogroup C meningococcal glyco-conjugate vaccine (Hib-MenC) or measles, mumps and rubella vaccine (MMR)). • A previous anaphylactic reaction to any vaccine or vaccine-related component. • Contraindication to vaccination with pneumococcal conjugate vaccine. • Bleeding diathesis or condition associated with prolonged bleeding time that would contraindicate intramuscular injection. • Known or suspected immune deficiency or suppression. • History of culture-proven invasive disease caused by S. pneumoniae. • Major known congenital malformation or serious chronic disorder. • Significant neurologic disorder or history of seizures including febrile seizure, or significant stable or evolving disorders such as cerebral palsy, encephalopathy, hydrocephalus, or other significant disorder. • Receipt of blood products or gamma-globulin (including hepatitis B immunoglobulin and monoclonal antibodies; e.g., Synagis B). • Parents who plan to move out of the geographical area where the study would be conducted.

    E.5.1Primary end point(s)
    A clinically significant difference would be shown if the difference in the proportion of participants with serotype-specific IgG concentrations ≥0.35μg/ml for the 10 serotypes included in PHiD-CV between the participants in the study vaccine group and the standard vaccine group one month following the booster dose at 12 months, is ≥10%.

    To assess serotype-specific IgG geometric mean concentrations (GMC) to 14 serotypes at baseline (before the booster is given) as well as 1 and 12 months following a booster with either PHiD-CV or PCV-13. To assess serotype-specific opsonophagocytic activity (OPA) geometric mean titres (GMT) and the proportion of participants with serotype-specific OPA titres ≥1:8 at baseline as well as 1 and 12 months following a booster dose of either PHiD-CV or PCV-13. To assess the proportion of participants at baseline, who have IgG concentrations ≥0.35mcg/ml for 14 serotypes. To assess reactogenicity of a booster at 12 months of age with either PHiD-CV or PCV-13 in terms of rates of local and systemic reactions following vaccination. To assess immediate pain at time of injection of a booster at 12 months of age with either PHiD-CV or PCV-13 determined by a validated pain assessment tool and crying time.
