The primary objective is to determine the efficacy of 2 treatment regimens of daratumumab, as measured by the overall response rate (ORR) (complete response [CR] + partial response [PR]),
in subjects with multiple myeloma who have received at least 3 prior lines of therapy including a PI and an IMiD or whose disease is double refractory to both a PI and an IMiD. |

1.El sujeto deberá tener al menos 18 años de edad.
2. El sujeto deberá tener mieloma múltiple comprobado, definido por los siguientes criterios, y signos de progresión de la enfermedad según los criterios del IMWG:

Mieloma múltiple de cadenas ligeras: Cadenas ligeras libres de inmunoglobulina 10 mg/dl y relación anormal entre las cadenas ligeras kappa y lambda de inmunoglobulinas en suero.
3.El sujeto deberá:
•tener signos de respuesta (es decir, haber logrado una reducción ≥25% de la proteína M durante ≥6 semanas [RM]) al menos a 1 de las pautas de tratamiento previas.
•Haber recibido un agente alquilante (≥2 ciclos o 2 meses), solo o combinado con otros tratamientos del mieloma. Un ciclo de un alquilante para TACM solo o combinado es aceptable. En las directrices de la Comprehensive National Cancer Network (NCCN) se facilita una lista de fármacos alquilantes (2013 NCCN18).
4.Sujetos que hayan recibido al menos 3 líneas de tratamiento previas (ver la definición al final de los criterios de inclusión) que incluyan un inhibidor del proteosoma (≥2 ciclos o 2 meses de tratamiento) y un IMID (≥2 ciclos o 2 meses de tratamiento) en cualquier orden durante el tratamiento (excepto los sujetos que interrumpieron cualquiera de estos tratamientos debido a una reacción alérgica grave en los primeros 2 ciclos/meses).

Sujetos cuya enfermedad sea doble refractaria a IPs e IMIDs. Para pacientes que hayan recibido más de un tipo de IP, su enfermedad debe ser refractaria al más reciente. Del mismo modo, para aquellos que hayan recibido más de un IMID su enfermedad debe ser refractaria al más reciente.
5.El sujeto deberá tener una puntuación del estado funcional del ECOG de 0, 1 ó 2 (véase el apéndice 2).
6.Las mujeres en edad fértil deberán estar utilizado un método anticonceptivo muy eficaz compatible con la normativa local sobre el uso de métodos anticonceptivos para los sujetos que participan en estudios clínicos: por ejemplo, uso establecido de anticonceptivos orales, inyectables o implantados; colocación de un dispositivo o sistema intrauterino; métodos de barrera: preservativo con espermicida en espuma/gel/película/crema/supositorio o capuchón oclusivo (diafragma o capuchón cervical/vaginal) con espermicida en espuma/gel/película/crema/supositorio; esterilización de la pareja masculina (la pareja vasectomizada debe ser el único compañero sexual de esa mujer); abstinencia real (cuando esté en consonancia con el modo de vida preferido y habitual del sujeto) durante y después del estudio (6 meses después de la última dosis de daratumumab en las mujeres).
7.Un varón sexualmente activo con una mujer en edad fértil que no se haya sometido a una vasectomía deberá comprometerse a utilizar un método anticonceptivo de barrera, como preservativo con espermicida en espuma, gel, película, crema o supositorio o pareja con capuchón oclusivo (diafragma o capuchón cervical/vaginal) con espermicida en espuma, gel, película, crema o supositorio, y ningún varón deberá donar semen durante el estudio ni en los 6 meses siguientes a la administración de la última dosis del fármaco del estudio.
8.Una mujer en edad fértil deberá dar un resultado negativo en la prueba de embarazo en suero u orina realizada en la selección.
