E.3 | Principal inclusion criteria |
1. Institutional Review Board (IRB)/Independent Ethics Committee (IEC) approved written informed consent and privacy language as per national regulations must be obtained from the subject prior to any study-related procedures (including withdrawal of prohibited medication, if applicable). 2. Subject is legally an adult according to local regulation at the time of signing informed consent. 3. Subject has histologically or cytologically confirmed urothelial carcinoma. Subjects with urothelial carcinoma (transitional cell) with squamous differentiation or mixed cell types are eligible. 4. Subject must have experienced radiographic progression or relapse during or after a CPI (anti-PD1 or anti-PD-L1) for locally advanced or metastatic disease. Subjects who discontinued CPI treatment due to toxicity are eligible provided that they have evidence of disease progression following discontinuation. The CPI need not be the most recent therapy. Subjects for whom the most recent therapy has been a non-CPI based regimen are eligible if they have progressed/relapsed during or after their most recent therapy. Locally advanced disease must not be amenable to resection with curative intent per the treating physician. 5. Subject must have received a platinum containing regimen (cisplatin or carboplatin) in the metastatic/locally advanced, neoadjuvant or adjuvant setting. If platinum was administered in the adjuvant/neoadjuvant setting subject must have progressed within 12 months of completion. 6. Subject has radiologically documented metastatic or locally advanced disease at baseline. 7. An archival tumor tissue sample should be available for submission to central laboratory prior to study treatment. If an archival tumor tissue sample is not available, a fresh tissue sample should be provided. If a fresh tissue sample cannot be provided due to safety concerns, enrollment into the study must be discussed with the medical monitor. 8. Subject has ECOG PS of 0 or 1 9. The subject has the following baseline laboratory data: - absolute neutrophil count (ANC) = 1500/mm3 - platelet count = 100 × 10^9/L -hemoglobin = 9 g/dL - serum bilirubin = 1.5 × upper limit of normal (ULN) or = 3 × ULN for subjects with Gilbert’s disease - creatinine clearance (CrCl) = 30 mL/min as estimated per institutional standards or as measured by 24 hour urine collection (glomerular filtration rate [GFR] can also be used instead of CrCl) - alanine aminotransferase (ALT) and aspartate aminotransferase (AST) = 2.5 × ULN or = 3 x ULN for subjects with liver metastases 10. Female subject must either: - Be of nonchildbearing potential: - Postmenopausal (defined as at least 1 year without any menses for which there is no other obvious pathological or physiological cause) prior to screening, or - Documented surgically sterile (e.g., hysterectomy, bilateral salpingectomy, bilateral oophorectomy).

1. Il soggetto deve avere manifestato progressione radiografica o recidiva durante o dopo un CPI (anti-PD1 o anti-PD-L1) per malattia localmente avanzata o metastatica. I soggetti che hanno interrotto il trattamento con CPI a causa di tossicità sono eleggibili a condizione che mostrino evidenza di progressione della malattia dopo l’interruzione. Il CPI non deve necessariamente essere la terapia più recente. I soggetti per i quali la terapia più recente non prevedeva un regime basato su CPI sono eleggibili se hanno mostrato progressione/recidiva durante o dopo la terapia più recente. La malattia localmente avanzata non deve essere resecabile con intento curativo da parte del medico curante. 5. Il soggetto deve avere ricevuto un regime contenente platino (cisplatino o carboplatino) in contesto neoadiuvante o adiuvante metastatico/localmente avanzato. Se il platino è stato somministrato in contesto adiuvante/neoadiuvante, il soggetto deve avere mostrato progressione entro 12 mesi dal completamento. 6. Al basale, il soggetto presenta malattia metastatica o localmente avanzata radiologicamente documentata. 7. Disponibilità di un campione di tessuto tumorale archiviato da presentare al laboratorio centrale prima del trattamento dello studio. Qualora non fosse disponibile un campione di tessuto tumorale archiviato, dovrà essere prelevato un campione tissutale fresco. Qualora non fosse possibile prelevare un campione tissutale fresco per motivi di sicurezza, l’arruolamento nello studio dovrà essere discusso con il responsabile del monitoraggio medico. 8. Il soggetto presenta ECOG PS pari a 0 o 1. 9. Il soggetto presenta i seguenti dati di laboratorio al basale: -conta assoluta dei neutrofili (ANC) =1.500/mm3 -conta piastrinica =100 × 109 /l -emoglobina =9 g/dl -bilirubina sierica =1,5 volte il limite superiore della normalità (ULN) o =3 volte l’ULN per i soggetti con malattia di Gilbert -clearance della creatinina (CrCl) =30 ml/min, stimata in base agli standard istituzionali o misurata mediante raccolta delle urine nell’arco di 24 ore (è anche possibile utilizzare la velocità di filtrazione glomerulare [GFR] al posto della CrCl) -alanina aminotransferasi (ALT) e/o aspartato aminotransferasi (AST) =2,5 volte l’ULN o =3 volte l’ULN per i soggetti con metastasi al fegato* * Docetaxel non deve essere scelto come comparatore se il soggetto presenta bilirubina > all’ULN o AST e/o ALT > 1,5 volte l’ULN in concomitanza con fosfatasi alcalina > 2,5 volte l’ULN. 10. Il soggetto di sesso femminile deve: -Non essere in età fertile: -Essere in post-menopausa (definita come assenza delle mestruazioni da almeno 1 anno senza altre evidenti cause patologiche o fisiologiche) prima dello screening, oppure -Presentare sterilità chirurgica documentata (per es., isterectomia, salpingectomia bilaterale, ooforectomia bilaterale).

E.4 | Principal exclusion criteria |
1. Subject has preexisting sensory or motor neuropathy Grade = 2. 2. Subject has active central nervous system (CNS) metastases. Subjects with treated CNS metastases are permitted on study if all the following are true: - CNS metastases have been clinically stable for at least 6 weeks prior to screening - If requiring steroid treatment for CNS metastases, the subject is on a stable dose = 20 mg/day of prednisone or equivalent for at least 2 weeks - Baseline scans show no evidence of new or enlarged brain metastasis - Subject does not have leptomeningeal disease 3. Subject has ongoing clinically significant toxicity (Grade 2 or higher with the exception of alopecia) associated with prior treatment (including systemic therapy, radiotherapy or surgery). Subject with hypothyroidism or panhypopituitarism related to treatment with PD-1 and PD-L1 inhibitors may be enrolled. Subject on hormone replacement therapy may be enrolled if on a stable dose. Subjects with ongoing immunotherapy related colitis, uveitis, or pneumonitis or subjects with other immunotherapy related AEs requiring high doses of steroids (> 20 mg/day of prednisone or equivalent) are excluded. 4. Subject has prior treatment with EV or other monomethyl auristatin E (MMAE)-based ADCs. 5. Subject has received prior chemotherapy for urothelial cancer with all available study therapies in the control arm (i.e., both prior paclitaxel and docetaxel in regions where vinflunine is not an approved therapy, or prior paclitaxel, docetaxel and vinflunine in regions where vinflunine is an approved therapy). Note: after vinflunine cap is reached, subjects who have received both docetaxel and paclitaxel will be excluded. 6. Subject has received more than 1 prior chemotherapy regimen for locally advanced or metastatic urothelial cancer, including chemotherapy for adjuvant or neo-adjuvant disease if recurrence occurred within 12 months of completing therapy. The substitution of carboplatin for cisplatin does not constitute a new regimen provided no new chemotherapeutic agents were added to the regimen. 7. Subject has history of another malignancy within 3 years before the first dose of study drug, or any evidence of residual disease from a previously diagnosed malignancy. Subjects with nonmelanoma skin cancer, localized prostate cancer treated with curative intent with no evidence of progression, low-risk or very low-risk (per standard guidelines) localized prostate cancer under active surveillance/watchful waiting without intent to treat, or carcinoma in situ of any type (if complete resection was performed) are allowed. 8. Subject is currently receiving systemic antimicrobial treatment for viral, bacterial, or fungal infection at the time of first dose of EV. Routine antimicrobial prophylaxis is permitted. 9. Subject has known active Hepatitis B (e.g., HBsAg reactive) or active hepatitis C (e.g., HCV RNA [qualitative] is detected). 10. Subject has known history of human immunodeficiency virus (HIV) infection (HIV 1 or 2).

I soggetti saranno esclusi dalla partecipazione se soddisfaranno una delle seguenti condizioni: 1. Il soggetto manifesta pre-esistente neuropatia sensoriale o motoria di grado =2. 2. Il soggetto presenta metastasi attive nel sistema nervoso centrale (SNC). I soggetti con metastasi del SNC trattate possono partecipare allo studio se tutte le seguenti condizioni sono vere: -Le metastasi del SNC sono clinicamente stabili da almeno 6 settimane prima dello screening -Se le metastasi del SNC necessitano di trattamento a base di steroidi, il soggetto riceve una dose stabile =20 mg/giorno di prednisone o equivalente da almeno 2 settimane -Le scansioni al basale non mostrano evidenze di metastasi cerebrali nuove o ingrossate -Il soggetto non presenta malattia leptomeningea 3. Il soggetto manifesta una tossicità in corso clinicamente significativa (grado 2 o superiore, ad eccezione dell’alopecia) associata ad un precedente trattamento (tra cui terapia sistemica, radioterapia o intervento chirurgico). Il soggetto con ipotiroidismo o panipopituitarismo correlato al trattamento con inibitori di PD-1 e PD-L1 può essere arruolato. Il soggetto che assume una terapia ormonale sostitutiva può essere arruolato se riceve una dose stabile. Sono esclusi i soggetti con in corso colite, uveite o polmonite correlata a immunoterapia oppure i soggetti che manifestano altri EA correlati a immunoterapia che richiedono elevate dosi di steroidi (>20 mg/giorno di prednisone o equivalente). 4. Il soggetto ha ricevuto un precedente trattamento con EV o altri ADC a base di monometil auristatina E (MMAE). 5. Il soggetto ha ricevuto una precedente chemioterapia per il tumore uroteliale con tutte le terapie dello studio disponibili nel braccio di controllo (ovvero, precedente terapia sia con paclitaxel che docetaxel nelle regioni in cui la vinflunina non è una terapia approvata, o precedente terapia con paclitaxel, docetaxel e vinflunina nelle regioni in cui la vinflunina è una terapia approvata). Nota: dopo aver raggiunto il limite di vinflunina, i soggetti che hanno ricevuto sia docetaxel che paclitaxel saranno esclusi. 6. Il soggetto ha ricevuto più di 1 precedente regime chemioterapico per il tumore uroteliale localmente avanzato o metastatico, inclusa chemioterapia adiuvante o neoadiuvante se la recidiva si è verificata entro 12 mesi dal completamento della terapia. La sostituzione del carboplatino con il cisplatino non costituisce un nuovo regime a condizione che non siano stati aggiunti nuovi agenti chemioterapici al regime. 7. Il soggetto presenta anamnesi di un’altra malignità nei 3 anni precedenti la prima dose del farmaco in studio o qualsiasi evidenza di malattia residua da una malignità precedentemente diagnosticata. Sono consentiti i soggetti con tumore cutaneo non melanoma, tumore prostatico localizzato trattato con intento curativo senza evidenza di progressione, tumore prostatico localizzato a rischio basso o molto basso (secondo le linee guida standard) sotto sorveglianza attiva/vigile attesa che non si intende trattare o carcinoma in situ di qualsiasi tipo (se è stata eseguita una resezione completa). 8. Il soggetto sta attualmente ricevendo un trattamento antimicrobico sistemico per infezione virale, batterica o fungina al momento della prima dose di EV. È consentita la profilassi antimicrobica di routine. 9. Il soggetto presenta nota epatite B attiva (per es. HBsAg reattivo) o epatite C attiva (per es. rilevamento di HCV RNA [qualitativo]). 10. Il soggetto presenta anamnesi nota di infezione da virus dell’immunodeficienza umana (HIV) (HIV 1 o 2).
