1. Age = 18 years (at signature of ICF), mentally and physically able and willing to provide informed consent for study participation. 2. Histologically confirmed Merkel Cell Carcinoma (MCC). 3. ECOG performance status = 1. 4. MCC in an advanced, unresectable stage III or metastatic stage IV (includes patients who refused surgical resection or are not eligible for such surgical resection) [Note: patients with PD post-R0 surgical resection and adjuvant anti-PD-(L)1 antibody monotherapy of at least 12 weeks will be eligible as long as Inclusion Criterion #6 is fulfilled] 5. RECIST v1.1 evaluable disease. 6. Progressing on previous anti-PD-(L)1 antibody monotherapy within the last 12 weeks before planned first administration of study medication fulfilling at least one of the following criteria: • Radiology Criteria: - Detection of new lesion(s) or - At least a 20% increase in the sum of diameters; in addition, the sum must also demonstrate an absolute increase of at least 5 mm. • In case of unresectable locoregional tumor not measurable by scan, assessment with a caliper will be allowed: a single, unirradiated/ untreated lesion must have a diameter of > 10 mm, at least a 20% increase in the diameter and an absolute increase of at least 5 mm. • Biopsy of new lesion(s) and histological confirmation of PD in case of progression during adjuvant anti-PD-(L)1 treatment. 7. Confirmation of PD not earlier than 4 weeks after initial assessment of PD on previous anti-PD-(L)1 monotherapy. [Note: Confirmatory scan can be the baseline scan for this study, if evaluable for RECIST v1.1 and can be performed during screening phase] 8. Pretreatment with avelumab monotherapy (cohort 1) or any anti PD-1 antibody monotherapy (cohort 2) fulfilling the following minimum exposure criteria: • Anti-PD-(L)1 antibody given every 2 weeks Q2W: at least 6 administrations within the last 6 months, last dose within 3 months before planned first administration of study medication. • Anti-PD-(L)1 antibody given every 3 weeks Q3W: at least 4 administrations within the last 6 months, last dose within 3 months before planned first administration of study medication. • Anti-PD-(L)1 antibody given every 4 weeks Q4W: at least 3 administrations within the last 6 months, last dose within 3 months before planned first administration of study medication. 9. Patients must have been treated with anti-PD-(L)1 antibody therapy as the most recent systemic anti-neoplastic therapy 10. Patients must have been treated with approved doses and schedules of avelumab or anti-PD-1 antibodies. For investigational anti-PD-1 antibodies, patients must have been treated with the recommended phase 2 dose and schedule. 11. Patients with brain or central nervous system metastases will be eligible, if asymptomatic, treated with surgery, whole brain or stereotactic radiotherapy, clinically stable (at least for a period of 2 months prior to signing ICF) and do not require continued steroid therapy. [Note: patients with known leptomeningeal carcinomatosis must be excluded] 12. Locally advanced/unresectable MCC must not be eligible for radiation therapy due to prior cumulative radiation treatment, judgment of radiation oncologist that the tumor is unlikely to respond to therapy or because radiation treatment is contraindicated for other reasons (e.g. tumor location). 13. Female patients of childbearing potential must have a negative urine or serum pregnancy test before receiving the first dose of study medication and they must comply with contraception methods as requested by the study protocol. |
1. Pazienti di età = 18 anni, mentalmente e fisicamente capaci e disposti a fornire il pro-prio consenso informato per la partecipazione allo studio. 2. Carcinoma a cellule di Merkel (MCC) confermato dall’esame istologico. 3. Performance status ECOG = 1. 4. MCC di stadio III avanzato, non operabile o di fase IV metastatico (compresi i pazienti che hanno rifiutato l’asportazione chirurgica o non sono idonei a tale asportazione) 5. Malattia valutabile secondo RECIST v1.1. 6. Pazienti precedentemente trattati con monoterapia di anticorpi anti-PD-(L)1 nelle ultime 12 settimane precedenti la prima somministra-zione prevista del farmaco in studio, che soddisfino almeno uno dei seguenti criteri: • Criteri radiologici: - Individuazione di nuova/e lesione/i o - Un aumento di almeno 20% della somma dei diametri; inoltre, la somma deve anche dimostrare un aumento assoluto di almeno 5 mm. • In caso di tumore locoregionale non operabile e non misurabile tramite TAC, sarà consentita la valutazione con un calibro: una lesione singola, non irradiata / non trattata, deve avere un diametro > 10 mm, un aumento di almeno 20% del diametro e un aumento assoluto di almeno 5 mm. • Biopsia di nuova/e lesione/i e conferma istologica di PD in caso di progressione durante il trattamento adiuvante con anti-PD-(L)1. 7. Conferma di PD non prima di 4 settimane dopo la valutazione ini-ziale con precedente monoterapia con anti-PD-(L)1. [Nota: La TAC di conferma può essere quella di riferimento per questo studio, se valutabile secondo RECIST v1.1, e può essere eseguita durante la fase di scree-ning] 8. Monoterapia precedente con avelumab (coorte 1) o con un qual-siasi altro anticorpo anti-PD-1 (coorte 2), che soddisfi i seguenti cri-teri di esposizione minima: • Anticorpi anti-PD-(L)1 somministrati ogni 2 settimane Q2W: al-meno 6 somministrazioni negli ultimi 6 mesi, ultima dose nei 3 mesi che precedono la prima somministrazione prevista del far-maco in studio. • Anticorpi anti-PD-(L)1 somministrati ogni 3 settimane Q3W: al-meno 4 somministrazioni negli ultimi 6 mesi, ultima dose nei 3 mesi che precedono la prima somministrazione prevista del far-maco in studio. • Anticorpi anti-PD-(L)1 somministrati ogni 4 settimane Q4W: al-meno 3 somministrazioni negli ultimi 6 mesi, ultima dose nei 3 mesi che precedono la prima somministrazione prevista del far-maco in studio. 9. I pazienti devono essere stati trattati con terapia con anticorpi anti-PD-(L)1 come la più recente terapia antineoplastica sistemica 10. I pazienti devono essere stati trattati con dosi e regimi di tratta-mento approvati di avelumab o anticorpi anti-PD-1. In caso di anti-corpi anti-PD-1 sperimentali, i pazienti devono essere stati trattati con la dose e il regime di trattamento raccomandati per la Fase 2. 11. I pazienti con metastasi cerebrali o a carico del sistema nervoso centrale, saranno considerati idonei se asintomatici, trattati con in-tervento chirurgico, con radioterapia del cervello intero o radiotera-pia stereotassica, clinicamente stabili (da almeno 2 mesi prima della firma del modulo di consenso informato), e se non necessi-tano di terapia steroidea continuativa. [Nota: i pazienti con carcinomatosi leptomeningea nota devono essere esclusi] 12. I MCC localmente avanzati/non operabili non devono essere stati considerati idonei alla radioterapia sulla base della precedente esposizione radiologica cumulativa; se a giudizio del radioterapista, è improbabile che il tumore risponda alla terapia, o se la radiotera-pia è controindicata per altri motivi (ad es. la posizione del tumore). 13. Le pazienti in età fertile devono risultare negative ad un test di gra-vidanza su siero o urine prima di ricevere la prima dose del tratta-mento in studio e devono utilizzare i metodi di contraccezione come richiesto dal protocollo di studio. |

E.4 | Principal exclusion criteria |
1. History of serious anti-PD-(L)1 therapy-related adverse reactions prohibiting further avelumab treatment: a) Pneumonitis: Grade 3 or 4 or recurrent Grade 2; b) Hepatitis: AST or ALT > 5 ULN or total bilirubin > 3 ULN; c) Colitis/diarrhea: Grade 4 or recurrent Grade 3; d) Nephritis and renal dysfunction: serum creatinine > 6 ULN; e) Any other immune-mediated adverse reactions which resulted in a lifethreatening situation for the patient (excluding endocrinopathies) or infusion-related reactions Grade 3 or 4. 2. More than one line of previous systemic anti-neoplastic therapy other than anti-PD-(L)1 antibody monotherapy. 3. Palliative radiation therapy of single lesions within 2 weeks before planned administration of study medication. 4. Patients currently participating or having participated in a clinical study in which the last administration of the investigational medicinal product was within 2 weeks before consenting to study participation (i.e. signing ICF). 5. Not recovered adequately (= Grade 1) from toxicities and/or complications from surgical intervention or from previous anticancer therapies (excluding alopecia, fatigue or endocrine dysfunction on replacement therapy) as judged by the investigator. 6. History or current evidence of clinically relevant allergies or hypersensitivity, which includes known or suspected intolerabilities attributed to domatinostat or avelumab or to constituents of the domatinostat tablets or avelumab infusion including known severe hypersensitivity reactions (Grade = 3) to monoclonal antibodies. 7. Inadequate organ function defined by the following laboratory parameters: • ANC < 1500/µl. • Hb < 9 g/dl (< Hb 5.6 mmol/L), may have been transfused. • Platelet count < 100.000/µl. • Serum creatinine > 1.5 x ULN or eGFR < 60 mL/min (as per Cockroft-Gault formula). • ALT or AST > 1.5 x ULN. • Serum total bilirubin > 1.5 x ULN. 8. Any medical condition requiring continuous systemic treatment with either corticosteroids (> 10 mg daily prednisone equivalents) or other systemic immunosuppressive medications within 2 weeks before consenting to study participation except for the following: intranasal, inhaled, topical, local steroid applications/injection or single doses of systemic corticosteroids as premedication/prevention for hypersensitivity reactions. 9. Any active gastrointestinal disorder that could interfere with the absorption of domatinostat. 10. Any known or suspected, current or chronic infection, immunodeficiency disorder or autoimmune disease requiring systemic treatment and/or that might deteriorate when receiving an immunostimulatory agent (e.g. CLL or allogeneic stem-cell transplantation). 11. History of other hematologic or primary solid malignancies which received or require any form of active systemic anti-cancer treatment during the last 12 months before consenting to study participation. 12. Received a live vaccine within 30 days before consenting to study participation. 13. Pregnant or breastfeeding. 14. Conditions requiring systemic anti-arrhythmic therapy known to prolong QT/QTc interval, patients with QTcF interval >480 msec on at least 2 separate ECGs or a medical history of long-QT-Syndrome. 15. Clinically significant (i.e. active) cardiovascular and/or thromboembolic diseases: • Cerebral vascular accident or stroke. • Uncontrolled hypertension. • Congestive heart failure (NYHA Class III or IV). • Serious cardiac arrhythmia requiring medication (patients with status post pace maker and/or defibrillator implantation can be included). • Symptomatic ischemic or severe valvular heart disease. • Unstable angina pectoris or a myocardial infarction within 6 months prior to signing ICF. 1 16. Patients with known HIV, acute or chronic active hepatitis B or Hepatitis C. 17. Significant (current or chronic) diseases or other intercurrent illness, psychiatric illness or social situation. |

Pazienti che non abbiano adeguatamente recuperato (= grado 1) da tossicità e/o complicazioni di intervento chirurgico, o da precedenti terapie antitumorali (esclusi alopecia, stanchezza o disfunzione en-docrina dopo terapia sostitutiva). 6. Precedenti o attuali evidenze di allergie clinicamente rilevanti o ipersensibilità, incluse intollerabilità note o sospette attribuite a domatinostat o avelumab o ai componenti delle compresse di domatinostat o dell’infusione di avelumab, comprese reazioni di ipersensibilità grave (grado = 3) note agli anticorpi monoclonali. 7. Inadeguata funzionalità di organo, secondo i seguenti parametri di laboratorio: • ANC < 1500/µl. • Hb < 9 g/dl (< Hb 5.6 mmol/L), anche in presenza di trasfuzioni. • Piastrine < 100.000/µl. • Creatinina > 1.5 x ULN o eGFR < 60 mL/min (secondo la formula di Cockroft e Gault). • ALT o AST > 1.5 x ULN. • Bilirubinemia totale > 1.5 x ULN. 8. Qualsiasi disturbo gastrointestinale attivo che possa interferire con l’assorbimento di domatinostat. 10. Qualsiasi infezione, disturbo da immunodeficienza o malattia autoimmune cronica, che necessiti di trattamento sistemico e/o possa peggiorare a seguito di agente immunostimolante (ad es. CLL o tra-pianto di cellule staminali allogeniche). 11. Precedenti di altri tumori solidi primari o ematologici trattati con, o che necessitino di, un trattamento antitumorale sistemico attivo negli ultimi 12 mesi prima del consenso per la partecipazione allo studio. 12. Somministrazione di un vaccino vivo fino a 30 giorni prima del consenso. 13. In gravidanza o allattamento. 14. Condizioni che necessitano di una terapia antiaritmica sistemica nota per prolungare l’intervallo QT/QTc, pazienti con intervallo QTcF >480 msec in almeno 2 ECG, o anamnesi di Sindrome del QT lungo. 15. Malattie cardiovascolari e/o tromboemboliche clinicamente signifi-cative (ovvero attive): • Incidente cerebrovascolare o ictus. • Ipertensione non controllata. • Insufficienza cardiaca congestizia (Classe III o IV NYHA)). • Aritmia cardiaca grave che necessita di terapia farmacologica. • Malattia ischemica sintomatica o patologia valvolare cardiaca grave. • Angina pectoris instabile o un infarto del miocardio nei 6 mesi precedenti la sottoscrizione del modulo per il consenso infor-mato. 16. Pazienti con HIV, epatite B attiva cronica o acuta o epatite C. 17. Patologie significative (in atto o croniche) o altre malattie intercorrenti, disturbi psichiatrici o situazioni sociali. |

Efficacy: for the first approximately 6 months imaging will be done every 8 weeks; thereafter the imaging interval will increase to every 12 weeks. Safety: During the regular study visits safety and tolerability assessments will be done. A full physical examination will be performed at screening, every 8 weeks until week 24 (i.e. weeks 9, 17, 25), every 12 weeks thereafter (i.e. weeks 37, 49, 61, 73, 85, 97) and at EOS Visit; brief physical examinations will be performed at all other visits. PK: Blood sampling for PK analysis will be performed on Day 1 and weeks 3, 5, 9, 17, 25, 37 and 49. |
