E.3 | Principal inclusion criteria |
1. Male or female and aged =18 years at the time of screening; 2. Clinical diagnosis of PN for at least 6 months with: • Pruriginous nodular lesions on upper limbs, trunk, and/or lower limbs; • At least 20 nodules on the entire body with a bilateral distribution 3. IGA score = 3 (based on the IGA scale ranging from 0 to 4, in which 3 is moderate and 4 is severe) at both the screening and baseline visits; 4. Severe pruritus defined as follows on the PP NRS: • At the screening visit (Visit 1): PP NRS score is = 7.0 for the 24-hour period immediately preceding the screening visit; • At the baseline visit (Visit 2): Mean of the daily intensity of the PP NRS score is = 7.0 over the previous week; 5. Female subjects of childbearing potential must agree to use at least 1 adequate and approved method of contraception throughout the study and for 12 weeks after the last study drug injection. Adequate and approved methods of contraception applicable for the subject and/or her partner are defined below: • True abstinence, when in line with the preferred and usual lifestyle of the subject. Periodic abstinence (eg, calendar, ovulation, symptothermal, post-ovulation methods) and withdrawal are not acceptable methods of contraception; • Progestogen-only oral hormonal contraception; • Combination of male condom with cap, diaphragm, or sponge with spermicide (double barrier methods); • Combined (estrogen- and progestogen-containing) oral, intravaginal, or transdermal hormonal contraception; • Injectable or implanted hormonal contraception; • Intrauterine devices; • Bilateral tubal ligation or tube insert (such as the Essure system) at least 3 months before the study; • Vasectomy of male partner at least 3 months before the study 6. Female subjects of non-childbearing potential must meet 1 of the following criteria: • Absence of menstrual bleeding for 1 year prior to screening without any other medical reason; • Documented hysterectomy or bilateral oophorectomy at least 3 months before the study; 7. Subject is willing and able to comply with all of the time commitments and procedural requirements of the clinical study protocol, including daily diary recordings by the subject using an electronic handheld device provided for this study. 8. Read, understood and signed an informed consent form (ICF) before any investigational procedure(s) are performed. |

Punteggio IGA =3 (in base alla scala IGA compresa tra 0 e 4, dove 3 indica moderata e 4 grave) sia alla visita di screening sia alla visita basale 4. Prurito grave, definito come segue sulla scala PP NRS: • Alla visita di screening (Visita 1): punteggio PP NRS =7,0 per il periodo di 24 ore immediatamente precedente alla visita di screening • Alla visita basale (Visita 2): media dell’intensità giornaliera del punteggio PP NRS =7,0 nell’ultima settimana; 5. I soggetti di sesso femminile in età fertile devono acconsentire a usare almeno 1 metodo contraccettivo adeguato e approvato per tutta la durata dello studio e per almeno 12 settimane dopo l’ultima iniezione di farmaco dello studio. I metodi contraccettivi adeguati e approvati applicabili per il soggetto e/o il suo partner sono definiti seguito: • astinenza reale, se in linea con lo stile di vita preferito e abituale del soggetto. L’astinenza periodica (per es. metodi del calendario, dell’ovulazione, sintotermico o post-ovulatorio) e il coito interrotto non sono metodi di contraccezione accettabili; • contraccezione ormonale orale contenente solo progestinici; • combinazione di preservativo maschile con cappuccio, diaframma o spugna con spermicida (metodi a doppia barriera); • contraccezione ormonale combinata (contenente estrogeno e progestinico) per via orale, intravaginale o transdermica; • contraccezione ormonale iniettabile o impiantabile; • dispositivi intrauterini; • legatura delle tube bilaterale o inserto tubarico (come il sistema Essure) almeno 3 mesi prima dello studio; • vasectomia del partner maschile eseguita almeno 3 mesi prima dello studio. 6. I soggetti di sesso femminile non potenzialmente fertili devono soddisfare 1 dei seguenti criteri: • assenza di sanguinamento mestruale per almeno 1 anno prima dello screening senza alcun’altra causa medica; • isterectomia od ooforectomia bilaterale documentata, eseguita almeno 3 mesi prima dello studio. 7. Il soggetto è disposto e in grado di attenersi a tutti gli impegni temporali e ai requisiti procedurali del protocollo di studio clinico, compresa la compilazione del diario giornaliero da parte del soggetto utilizzando un dispositivo palmare elettronico fornito per questo studio 8. E.4 | Principal exclusion criteria |
1. Body weight < 30 kg; 2. Chronic pruritus resulting from another active condition other than PN, such as to scabies, lichen simplex chronicus, psoriasis, atopic dermatitis, contact dermatitis, acne, folliculitis, lichen planus, habitual picking/excoriation disorder, sporotrichosis, bullous autoimmune disease, end-stage renal disease, or cholestatic liver disease ; 3. Unilateral lesions of PN; 4. History of or current confounding skin condition 5. Subjects meeting 1 or more of the following criteria at screening or baseline: - Had an exacerbation of asthma requiring hospitalization in the preceding 12 months; - Reporting asthma that has not been well-controlled during the preceding 3 months; - Asthma Control Test = 19 (only for subjects with a history of asthma) - Peak expiratory flow < 80% of the predicted value. 6. Subjects with a current medical history of chronic obstructive pulmonary disease and/or chronic bronchitis; 7. Cutaneous infection within 1 week before the screening visit or any infection requiring treatment with oral or parenteral antibiotics, antivirals, antiparasitics, or antifungals within 1 week before the screening visit. Subjects may be rescreened once the infection has resolved; 8. Positive serology results at the screening visit; 9. Requiring rescue therapy for PN during the screening period or expected to require rescue therapy within 4 weeks following the baseline visit; 10. Subjects with active atopic dermatitis in the last 3 months; 11. Neuropathic and psychogenic pruritus such as but not limited to notalgia paresthetica, brachioradial pruritus, small fiber neuropathy, skin picking syndrome, or delusional parasitosis; 12. Having received any of the treatments in the table reported in the protocol within the specified timeframe before the baseline visit; 13. Previous participation in a clinical study with nemolizumab; 14. Pregnant women, breastfeeding women, or women planning a pregnancy during the clinical study; 15. History of lymphoproliferative disease or history of malignancy of any organ system within the last 5 years, except for: - Basal cell carcinoma, squamous cell carcinoma in situ, or carcinomas in situ of the cervix that have been treated and have no evidence of recurrence in the last 12 weeks before the screening visit, or; - Actinic keratoses that have been treated 16. History of hypersensitivity (including anaphylaxis) to an immunoglobulin product (plasma-derived or recombinant, eg, monoclonal antibody) or to any of the study drug excipients; 17. Known active or latent tuberculosis infection; 18. Known or suspected immunosuppression or unusually frequent, recurrent, severe, or prolonged infections as per PI judgment; 19. Any medical or psychological condition or any clinically relevant laboratory abnormalities, such as but not limited to elevated ALT or AST (> 3 × upper limit of normal [ULN]) in combination with elevated bilirubin (> 2 × ULN), during the screening period that may put the subject at significant risk according to the PI's judgment, if he/she participates in the clinical study, or may interfere with study assessments (eg, poor venous access or needle-phobia); 20. History of alcohol or substance abuse within 6 months of the screening visit; 21. Planned or expected major surgical procedure during the clinical study; 22. Subject is unwilling to refrain from using prohibited medications during the clinical study; 23. Currently participating or participated in any other study of a drug or device, within the past 8 weeks before the screening visit, or is in an exclusion period from a previous study. |

Soggetti che soddisfano 1 o più dei seguenti criteri allo screening o al basale: 5a. precedente riacutizzazione dell’asma che ha richiesto il ricovero ospedaliero negli ultimi 12 mesi; 5b. episodio di asma che non è stato adeguatamente controllato (ovvero, sintomi verificatisi per >2 giorni a sett, =2 risvegli notturni a sett o qualche interferenza con le attività abituali) durante gli ultimi 3 mesi; 5c. test per il controllo dell’asma =19 5d. picco di flusso espiratorio <80% del valore predetto 6. Soggetti con un’anamnesi medica di malattia polmonare ostruttiva cronica e/o bronchite cronica 7. Infezione cutanea entro 1 sett prima della visita di screening o qualsiasi infezione che richieda un trattamento con antibiotici, antivirali, antiparassitari o antifungini per via orale o parenterale entro 1 sett prima della visita di screening. I soggetti possono essere sottoposti nuovamente a screening una volta che l’infezione si è risolta 8. Risultati sierologici positivi alla visita di screening 9. Necessità di una terapia di soccorso per la PN durante il periodo di screening o necessità prevista di una terapia di soccorso nelle 4 sett successive alla visita basale 10. Soggetti con dermatite atopica attiva (segni e sintomi diversi da secchezza cutanea) negli ultimi 3 mesi 11. Prurito neuropatico e psicogeno, come per esempio, ma senza limitazione, notalgia parestesica, prurito brachioradiale, neuropatia delle piccole fibre ecc 12. Precedente somministrazione di uno qualsiasi dei seguenti trattamenti indicati nel protocollo entro l’intervallo specificato prima della visita basale: 13. Precedente partecipazione a uno studio con nemolizumab 14. Donne in gravidanza, donne in fase di allattamento al seno o donne che hanno in programma di avviare una gravidanza durante lo studio 15. Anamnesi di malattia linfoproliferativa o anamnesi di tumore maligno a carico di qualsiasi sistema di organi negli ultimi 5 anni, fatta eccezione per: • carcinoma a cellule basali, carcinoma a cellule squamose in situ o carcinomi in situ del collo dell’utero che sono stati trattati e non presentano alcuna evidenza di recidiva nelle ultime 12 sett prima della visita di screening, oppure; • cheratosi attiniche che sono state trattate 16. Anamnesi di ipersensibilità a un prodotto a base di immunoglobuline o ad uno qualsiasi degli eccipienti del farmaco dello studio 17. Nota infezione da tubercolosi attiva o latente 18. Immunosoppressione nota o sospetta, oppure infezioni insolitamente frequenti, ricorrenti, gravi o prolungate secondo il giudizio del PI 19. Qualsiasi condizione medica o psicologica o qualsiasi anomalia di laboratorio clinicamente rilevante, come per esempio, aumento dei livelli di ALT o AST (>3 volte il limite superiore alla norma [ULN]) in combinazione con > livelli di bilirubina (>2 volte l’ULN durante il periodo di screening) tale da poter comportare un rischio per il soggetto secondo il giudizio del PI qualora dovesse partecipare allo studio, o che possa interferire con le valutazioni dello studio 20.
