- Histologically and/or cytologically documented adenocarcinoma of the colon or rectum, which is metastatic and/or unresectable - Unless contraindicated, subjects must have received and failed regimens containing the following agents: fluoropyrimidine (e.g., 5-fluorouracil or capecitabine), oxaliplatin, irinotecan, an anti-VEGF monoclonal antibody (bevacizumab, ramucirumab, or ziv-aflibercept), and an anti-PD-(L)1 therapy (nivolumab or pembrolizumab) if tumor has deficient mismatch repair proteins or is MSI-High. - Have progression of unresectable or mCRC after last systemic therapy, or be intolerant of last systemic therapy - Have RAS wild-type in primary or metastatic tumor tissue, based on expanded RAS testing - Subjects must be willing and able to provide the most recent tissue blocks obtained prior to treatment initiation. If archival tissue is not available, then a newly-obtained baseline biopsy of an accessible tumor lesion is required - Have confirmed HER2-positive mCRC, as defined by having tumor tissue tested at a Clinical Laboratory Improvement Act (CLIA)-certified laboratory, meeting at least one of the following criteria: a. HER2+ overexpression (3+ immunohistochemistry [IHC]) by an FDA-approved HER2 IHC test b. HER2 2+ IHC is eligible if the tumor is amplified by an FDA-approved HER2 in situ hybridization assay c. HER2 (ERBB2) amplification by CLIA-certified next generation sequencing (NGS) sequencing assay - Have radiographically measurable disease assessable by RECIST 1.1, with at least one site of disease that is measurable and that has not been previously irradiated; or, if the subject has had previous radiation to the target lesion(s), there must be evidence of progression since the radiation - Have an Eastern Cooperative Oncology Group (ECOG) Performance Status (PS) of 0, 1, or 2 - Life expectancy greater than 3 months - Have adequate hematological, hepatic, renal, coagulation, and cardiac function obtained ≤7 days prior to the first study treatment Other protocol-defined inclusion criteria may apply. |

Si le tissu archivé n’est pas disponible, une biopsie nouvellement obtenue d’une lésion tumorale accessible est alors nécessaire à la référence -Présenter un CCRm HER2-positif confirmé, comme défini en ayant du tissu tumoral analysé dans un laboratoire certifié par les amendements d’amélioration de laboratoire clinique (Clinical Laboratory Improvement Amendments, CLIA) ou accrédité par l’Organisation internationale de normalisation (ISO), répondant à au moins un des critères suivants : a.Surexpression de HER2+ (immunohistochimie [IHC] 3+) par un test d’IHC HER2 approuvé par l’Agence américaine des produits alimentaires et médicamenteux (Food and Drug Administration, FDA) ou portant le marquage Conformité Européenne (CE), conformément au manuel d’interprétation de la notice d’emballage pour le cancer du sein ; b.L’IHC HER2 2+ est éligible si la tumeur est amplifiée par un test d’hybridation in situ HER2 approuvé par la FDA ou portant le marquage CE (hybridation in situ en fluorescence [FISH] ou hybridation in situ chromogénique [CISH]) conformément au manuel d’interprétation de la notice d’emballage pour le cancer du sein ; c.Amplification HER2 (ERBB2) par un test de séquençage de nouvelle génération (SNG) certifié CLIA ou accrédité par l’ISO. -Âge ≥ 18 ans au moment du consentement -Présenter une maladie mesurable par radiographie évaluable selon les critères RECIST 1.1, avec au moins un site de la maladie qui soit mesurable et qui n’a pas été irradié auparavant ; ou, si le patient a subi précédemment des radiations au niveau de la ou des lésions cibles, il doit y avoir des preuves de progression depuis la radiothérapie -Présenter un indice de performance (IP) ECOG de 0, 1 ou 2 -Espérance de vie supérieure à 3 mois, de l’avis de l’investigateur -Présenter une fonction hématologique, hépatique, rénale, de coagulation et cardiaque adéquate comme défini ci-dessous, obtenue 7 jours avant le premier traitement à l’étude : a.Numération absolue des neutrophiles (NAN) ≥ 1,0 x 103/µl ; b.Numération plaquettaire ≥ 100 × 103/µl ; les patients ayant une numération plaquettaire stable de 75 à 100 × 103/µl peuvent être inclus avec l’approbation du moniteur médical ; c.Hémoglobine 9,0 g/dl ; d.Bilirubine totale ≤ 1,5 × limite supérieure de la normale (LSN). Les patients ayant des antécédents connus de syndrome de Gilbert et bilirubine directe normale, aspartate aminotransférase (ASAT) et alanine aminotransférase (ALAT) sont éligibles ; e.ASAT et ALAT ≤ 2,5 × LSN (≤ 5 × LSN si des métastases hépatiques sont présentes) ; f.Clairance de la créatinine calculée ≥ 50 ml/min en utilisant la formule de Cockcroft-Gault ; g.Rapport normalisé international (RNI) et temps de céphaline activée (TCA) ≤ 1,5 × LSN sauf en cas de médicaments connus pour altérer le RNI et/ou le TCA ; h.Fraction d’éjection ventriculaire gauche (FEVG) ≥ 50 % telle qu’évaluée par échocardiographie (ECHO) ou angiographie isotopique (multiple-gated acquisition, MUGA) documentée ≤ 28 jours avant le traitement à l’étude.

- Previous treatment with anti-HER2 targeting therapy - Previous treatment with any systemic anticancer therapy, non-central nervous system radiation, or experimental agent ≤3 weeks of first dose of study treatment or are currently participating in another interventional clinical trial - Have any toxicity related to prior cancer therapies that has not resolved to ≤ Grade 1, with the following exceptions: • Alopecia and neuropathy, which must have resolved to ≤ Grade 2 • Congestive heart failure (CHF), which must have been ≤ Grade 1 in severity at the time of occurrence, and must have resolved completely • Anemia, which must have resolved to ≤ Grade 2 • Decreased ANC, which must have resolved to ≤ Grade 2 - Have clinically significant cardiopulmonary disease - Have known myocardial infarction, unstable angina, cardiac or other vascular stenting, angioplasty, or cardiac surgery within 6 months prior to first dose of study treatment - Major surgical procedure, open biopsy, or significant traumatic injury ≤28 days prior to enrollment (≤56 days for hepatectomy, open thoracotomy, or major neurosurgery) or anticipation of need for major surgical procedure during the course of the study - Serious, non-healing wound, ulcer, or bone fracture - Known to be positive for hepatitis B by surface antigen expression, HIV or to have active hepatitis C infection - Subjects who are pregnant, breastfeeding, or planning a pregnancy - Inability to swallow pills or any significant gastrointestinal disease which would preclude the adequate oral absorption of medications - Have used a strong CYP2C8 inhibitor within 5 half-lives of the inhibitor, or have used a CYP2C8 or CYP3A4 inducer within 5 days prior to first dose of study treatment - Have any other medical, social, or psychosocial factors that, in the opinion of the investigator, could impact safety or compliance with study procedures - History of another malignancy within 3 years before the first dose of study drug, or any evidence of residual disease from a previously diagnosed malignancy - Subjects with known active CNS metastasis Other protocol-defined exclusion criteria may apply. |
Les patients seront exclus de l’étude pour l’une quelconque des raisons suivantes : -Avoir été précédemment traité(e) par une thérapie ciblée anti-HER2 -Avoir reçu un traitement par toute thérapie anticancéreuse systémique (y compris thérapie hormonale et biologique), radiation hors système nerveux central (SNC) ou agent expérimental ≤ 3 semaines précédant la première dose du traitement de l’étude, ou participent actuellement à un autre essai clinique interventionnel -Présenter toute toxicité liée à des thérapies anticancéreuses antérieures qui ne s’est pas résolue à un grade ≤ 1, avec les exceptions suivantes : •Alopécie et neuropathie, qui doit être résolue à un grade ≤ 2 ; •Insuffisance cardiaque congestive (ICC), qui doit avoir été d’une gravité de ≤ grade 1 au moment de la survenue, et doit être complètement résolue ; •Anémie, qui doit être résolue à un grade ≤ 2 ; •Diminution de la NAN, qui doit être résolue à un grade ≤ 2. -Avoir une maladie cardiopulmonaire cliniquement significative telle que : •Arythmie ventriculaire nécessitant un traitement ; •Hypertension symptomatique ou hypertension asymptomatique non contrôlée, comme déterminé par l’investigateur ; •Tout antécédent d’ICC symptomatique, une dysfonction systolique ventriculaire gauche ou une diminution de la fraction d’éjection ; •Grave dyspnée au repos (CTCAE de grade 3 ou plus) en raison de complications de la tumeur maligne avancée ou d’une hypoxie nécessitant une oxygénothérapie supplémentaire ; •Présence de prolongation de l’intervalle QT corrigé (QTc) de grade ≥ 2 sur l’électrocardiogramme (ECG) de sélection. -Présenter des antécédents d’infarctus du myocarde, angor instable, pose d’endoprothèse cardiaque ou vasculaire, angioplastie ou chirurgie cardiaque dans les 6 mois précédant la première dose du traitement à l’étude -Intervention chirurgicale majeure, biopsie ouverte ou dommage traumatique significatif ≤ 28 jours précédant l’inclusion (≤ 56 jours pour une hépatectomie, thoracotomie ouverte ou neurochirurgie majeure) ou prévision de nécessité d’une intervention chirurgicale majeure au cours de l’étude -Blessure, ulcère ou fracture osseuse grave, qui ne se cicatrise ou ne guérit pas -Résultat positif connu pour l’hépatite B par expression de l’antigène de surface -Infection active connue par le virus de l’hépatite C (positive par réaction en chaîne par polymérase ou par thérapie antivirale pour l’hépatite C au cours des 6 derniers mois). Les patients ayant été traités pour une infection par l’hépatite C sont autorisés à participer s’ils présentent une réponse virologique soutenue documentée sur 12 semaines -Séropositivité connue pour le virus de l’immunodéficience humaine (VIH) -Les patientes qui sont enceintes, qui allaitent ou qui prévoient une grossesse -Incapacité à avaler des comprimés ou toute maladie gastro-intestinale significative pouvant empêcher l’absorption orale adéquate de médicaments -Avoir utilisé un puissant inhibiteur du CYP2C8 dans les 5 demi-vies de l’inhibiteur, ou avoir utilisé un inducteur du CYP2C8 ou du CYP3A4 dans les 5 jours précédant la première dose du traitement à l’étude. Les inhibiteurs puissants du CYP2C8 et les inducteurs du CYP2C8 ou du CYP3A4 sont également interdits dans les 2 semaines suivant l’interruption du traitement par tucatinib. L’utilisation des substrats sensibles du CYP3A doit être évitée 2 semaines avant la première dose du traitement à l’étude et pendant le traitement à l’étude -Présenter tout autre facteur médical, social ou psychosocial, qui, de l’avis de l’investigateur, pourrait avoir un impact sur la sécurité d’emploi ou le respect des procédures de l’étude -Antécédents d’autre tumeur maligne dans les 3 ans précédant la première dose du médicament de l’étude ou toute preuve de maladie résiduelle provenant d’une tumeur maligne précédemment diagnostiquée. Exception sera faite des tumeurs malignes ayant un risque négligeable de métastase ou de décès (par ex., SG à 5 ans ≥ 90 %), comme un carcinome in situ du col de l’utérus, un carcinome cutané non mélanomateux, un cancer localisé de la prostate, un carcinome canalaire in situ ou un cancer de l’utérus de stade I adéquatement traité -Les patients présentant des métastases actives connues du SNC (les lésions irradiées ou réséquées sont autorisées, à condition que les lésions soient intégralement traitées et inactives, que le patient soit asymptomatique et qu’aucun stéroïde n’ait été administré pendant au moins 30 jours)
